Understanding patient perceptions of asthma: results of the Asthma Control and Expectations (ACE) survey.

In spite of support among UK healthcare professionals for asthma guidelines, studies continue to show that many patients fail to reach the suggested management goals. Patient expectations and poor communication may be factors in this failure. This survey assessed patients' asthma control, expectations in respect of asthma and communication with healthcare professionals. A structured questionnaire, designed for self-completion, was developed and distributed to asthma patients at participating pharmacies. A total of 1031 questionnaires were returned. Most of the respondents, even among those feeling well, reported lifestyle restrictions because of asthma. These restrictions were not generally discussed with healthcare professionals. One-third did not perceive the benefits of inhaled corticosteroids. Low expectation may be a major contributor to the poor control seen in this survey. The failure to discuss lifestyle restrictions and symptom levels with healthcare professionals gives little scope for recommendation of appropriate and adequate treatment. Improved communication between healthcare professionals and patients may help such understanding and raise patient expectations.

Management of asthma in pre-school children.

The management of asthma in pre-school children often presents physicians with challenging problems. This article addresses the diagnostic criteria by which the diagnosis may be made, discusses the prognosis of untreated asthma and states the principles underlying the treatment of asthma in this age group. The management according to a stepwise protocol is discussed with reference both to maintenance therapy, and the treatment of acute severe asthma. The methods by which appropriate medication may be delivered are also described.

Rapid response to budesonide (Pulmicort) inhaled via the Nebuhaler in asthmatic children.

Acute reversible airways obstruction in children, especially those aged under one year, who may respond poorly to inhaled beta 2-stimulants, is difficult to treat in general practice. In an open study, 48 episodes in 29 children, half aged under one year, were treated with budesonide (Pulmicort) 0.1-2.09 mg used to charge a Nebuhaler fitted with a facemask. In 45 instances, clinical improvement was observed, and with doses of 0.8 mg and above, this usually occurred within 10 minutes. This study suggests that budesonide via a suitable delivery system can evoke rapid improvement in reversible airways obstruction in young children.

Improved method for selenium determination in biological samples by gas chromatography.

A gas chromatographic assay employing electron-capture detection for the determination of selenium in biological samples is reported. A calibration curve of 4-nitro-o-phenylene-diamine derivative of selenium as a function of peak area was linear from 5-1000 pg. The limit of detection for the electron-capture detector was approximately 0.5 pg. Recoveries of selenium added to various biological materials ranged from 95-105%. This procedure reduces the number of transfers thereby reducing errors associated with losses or contamination. One advantage of the present method is that interfering compounds occurring in previously employed chromatographic methods are eliminated. This procedure can be used for routine microanalysis of selenium. Samples containing less than 2 ng selenium in 200 microliter of biological fluid can be routinely analyzed using this method.

Training vocational rehabilitation counselors who work with chronic mental patients.

Because of the inefficacy of disparate psychiatric and rehabilitative approaches to psychosocial restoration of chronic mental patients, the authors designed the New England Psychiatric Rehabilitation Training Program for vocational rehabilitation counselors who work with these patients. The program extends Erikson's epigenetic sequence to development of work capacity and emphasizes a comprehensive, multiaxial approach to psychopathology and vocational rehabilitation. After 6 weeks of seminars, clinical work, and supervision, counselors return to their agencies and receive an additional 8 weeks of part-time support from field faculty instructors.
