Moxifloxacin versus ofloxacin plus metronidazole in uncomplicated pelvic inflammatory disease: results of a multicentre, double blind, randomised trial.

OBJECTIVE: This multinational, multicentre, prospective, randomised, double blind, parallel group, non-inferiority study compared the efficacy and safety of moxifloxacin monotherapy with ofloxacin plus metronidazole in women with uncomplicated pelvic inflammatory disease. METHODS: Women from hospitals throughout 13 countries received a 14 day course of either oral moxifloxacin, 400 mg once daily (n = 384), or oral ofloxacin, 400 mg twice daily plus oral metronidazole, 500 mg twice daily (n = 365). RESULTS: Of the 741 patients in the intent to treat (ITT) population, 564 (74.2%) were valid for the per protocol (PP) analyses; 112 (19.9%) of these were included in the microbiologically valid population (MBV). Clinical resolution rates in the PP population at the test of cure visit (TOC, 5-24 days post-therapy, primary efficacy end point) were 90.2% (248/275) for moxifloxacin and 90.7% (262/289) for ofloxacin plus metronidazole (95% CI: -5.7% to 4.0%). At follow up (28-42 days post-therapy), resolution rates in the PP population were 85.8% (236/275) and 87.9% (254/289) for moxifloxacin and comparator, respectively (95% CI: -8.0% to 3.1%). Bacteriological success rates in the MBV population at TOC were 87.5% (49/56) for moxifloxacin and 82.1% (46/56) for comparator (95% CI: -8.3% to 18.8%). Against Chlamydia trachomatis and Neisseria gonorrhoeae, bacteriological success rates with moxifloxacin were 88.5% (23/26) and 100% (13/13) and for comparator 85.7% (18/21) and 81.8% (18/22), respectively. Drug related adverse events occurred less frequently with moxifloxacin (22.5% (85/378)) versus the comparator (30.9% (112/363)) (p = 0.01). CONCLUSION: In uncomplicated PID, once daily moxifloxacin monotherapy was clinically and bacteriologically as efficacious as twice daily ofloxacin plus metronidazole therapy and was associated with fewer drug related adverse events.

Smoking induces oxidative stress inside the Graafian follicle.

BACKGROUND: A growing body of evidence indicates that pro-oxidant/antioxidant balance inside ovarian follicles plays an important role in folliculogenesis. Over 20% of women of reproductive age in Europe and the USA regularly smoke cigarettes. The impact of tobacco smoking on the intrafollicular markers of oxidative stress has not been fully elucidated. The objective of the present study was to test the hypothesis that cigarette smoking affects the intrafollicular redox milieu. METHODS: In follicular fluid samples originating from 108 IVF patients, lipid peroxidation was assessed by the thiobarbituric reactive substances method and total antioxidative capacity was quantified by the luminol enhanced chemiluminescence method. The level of patients' exposure to the cigarette smoke was evaluated by measuring the follicular fluid cotinine concentration by means of radioimmunoassay. RESULTS: Intrafollicular exposure to cigarette smoke metabolites was associated with a significant increase in follicular lipid peroxidation intensity (P < 0.001), which was accompanied by a significant decrease in the local antioxidative potential (P = 0.004). CONCLUSION: The results indicate that active smoking affects the pro-oxidant/antioxidant balance inside the pre-ovulatory ovarian follicle by inducing intrafollicular oxidative stress. This provides another possible explanation for impaired folliculogenesis in female smokers.

[The antioxidant-prooxidant balance in pregnancy complicated by spontaneous abortion].

The purpose of this study was to characterize prooxidant-antioxidant balance in patients with different clinical types of spontaneous abortion. Sixty previously untreated, pregnant women aged 16-38 years were included into this study. The gestational age ranged from 8 to 12 weeks. Patients were divided into three groups of 20 subjects each. The first group consisted of patients with inevitable abortion. The second group included women with threatened abortion in whom pregnancy was maintained until term. The control group consisted of women with uncomplicated pregnancy. In all the patients venous blood was collected immediately after the diagnosis was settled and before the treatment was commenced. Two biochemical markers of oxidative stress were quantified, namely: lipid peroxides (LP) using spectrophotometric TBARS method and total antioxidant status (TAS) by means of the commercially available kits (TAS: Randox Laboratories Ltd., U.K.). The obtained results indicate that spontaneous abortion is accompanied by a profound disruption of the prooxidant-antioxidant homeostasis towards oxidative stress.

[Influence of premature rupture of fetal membranes on kynurenic acid concentration in umbilical cord blood].

The aim of the study was to evaluate the influence of premature rupture of fetal membranes on kynurenic acid (KYNA) levels in venous and arterial umbilical cord blood of neonates. Statistically significant higher concentration of KYNA in umbilical arterial and venous blood in neonates of mothers with PROM lasting longer than 11 hours was observed. Increase of KYNA concentration in blood of such babies may be one of markers of developing infection.

[Cotinine levels in serum and colostrum of smoking puerperae].

In 50 women (25 smokers and 25 non-smokers), cotinine, a relatively stable metabolite of nicotine, was measured in the 3rd postpartum day in blood serum and colostrum by means of enzyme-enhanced chemiluminescence. Non-smokers had cotinine levels in serum and colostrum below 10 ng/ml. Colostrum cotinine content in smokers amounted to approximately 50% of the respective serum level and both these values were strongly positively correlated (p < 0.0014). Cotinine level in early breast-milk did not correlate significantly with the number of cigarettes smoked daily by a mother. Newborns breast-fed by smoking women are exposed to the tobacco smoke compounds not only by an environmental ("passive") smoking but also by the ingestion of nicotine metabolites present in mother's milk.

[The influence of smoking on the estradiol level in the preovulatory follicular fluid].

The aim of the present study was to test the relationship between the objectively measured intrafollicular exposure to the tobacco smoke compounds and the estradiol level in the preovulatory follicular fluid. In 30 IVF-ET patients (14 smokers and 16 non-smokers) levels of cotinine (C-FF) and 17 beta-estradiol (E2-FF) were quantified in the follicular fluid by means of radioimmunoassay. In smokers (C-FF from 62.21 to 595.00 ng/ml) the mean E2-FF was 85.07 nmol/l, while in the non-smokers (C-FF from 1.20 to 15.62 ng/ml) the respective value amounted to 109.19 nmol/l. The difference was statistically significant (p = 0.005). The obtained results support the growing body of evidence that tobacco smoking negatively influences endocrine function of the Graafian follicle.

[Menopausal medicine of the threshold of the third millennium].

The paper reviews most important trends in hormonal replacement therapy which have appeared in recent years. The tailored approach to HRT, introduction of new drugs and modification of established treatment regimes are the most probable directions in which menopausal medicine would develop in the years to come.

[Serum estradiol levels in early postoperative period after surgical castration--the influence of estrogen replacement therapy].

In 40 premenopausal women who underwent TAH&BSO serum E2 levels were measured by means of RIA in 3 time points: before the operation, in the postoperative day 2 and in the postoperative day 3. In 20 of the subjects the ERT (transdermal estradiol 0.1 g/24 h) was introduced in the 2 postoperative day. In the postoperative day 2 the average serum E2 decreased by nearly 90% as compared to the preoperative values. Twenty four hours after the application of the transdermal E2 patch, the estradiol level returned to the values insignificantly different from the preoperative ones.

[Blood serum laminin in gravidae with threatened abortion].

UNLABELLED: The aim of the study was to compare the serum laminin levels in patients with threatened abortion and healthy gravidae with uncomplicated pregnancy. MATERIAL AND METHODS: Twenty five pregnant women with menstrual arrest period ranging from 7 to 22 weeks, presenting with symptoms of threatened abortion (TA), were included into the study. Control group consisted of 25 healthy gravidae with uneventful course of pregnancy matched for the gestational age. Blood serum laminin was quantified by means of ELISA method using commercially available kits. RESULTS: Average serum laminin levels (S-L) measured in gravidae with threatened abortion was significantly higher than that of the control group. In patient with sonographically diagnosed trophoblast ablation the serum laminin was significantly elevated in comparison with the remaining patients of TA group, who had no sonographic abnormalities found. The patients with favorable outcome had mean serum laminin significantly lower than that of the patients with subsequent pregnancy loss. CONCLUSIONS: The observed difference in serum laminin level between patients with TA and patients with uncomplicated pregnancy seems to result from the release of laminin from trophoblast in cases when the integrity of the decidual/conceptual interface is disintegrated. This hypothesis is further supported by the finding of the high S-L in patients with the ablation of trophoblast. The obtained results suggest the prognostic value of S-L measurements in patients with imminent abortion.

The Graafian follicle is a site of L-ascorbate accumulation.

PURPOSE: Our purpose was to evaluate the L-ascorbate level in human preovulatory follicular fluid and to quantify the blood/follicle gradient for vitamin C. The effect of smoking on the follicular L-ascorbate concentration was studied. The correlations were tested between follicular L-ascorbate and follicle size and oocyte maturity. METHODS: In 65 women undergoing in vitro fertilization, samples of follicular fluid and blood serum were collected. Biochemical analyses included L-ascorbate determinations by a colorimetric method and cotinine measurements by a radioimmunoassay. RESULTS: The average follicular fluid:serum ratio for L-ascorbate was 1:68. Ascorbate levels in follicular fluid and serum were significantly correlated. The follicular L-ascorbate level did not correlate with the follicle size and the oocyte maturity grade. Insignificantly lowered follicular L-ascorbate levels were observed in smokers. CONCLUSIONS: The extracellular compartment of the Graafian follicle is a site of an ascorbate accumulation. Exposure to tobacco smoke does not significantly diminish the intrafollicular pool of L-ascorbate.

[Levels of lipid peroxidation products in blood serum and placental tissue of women with pregnancy induced hypertension].

The authors compared the concentrations of conjugated diens (CD), lipid hydroperoxides (HEPTE) and malonyl dialdehyde (MDA), in blood serum and placental tissue of 15 parturients with pregnancy induced hypertension (PIH) and 15 normotensive controls matched for gestational age. The CD and HEPTE were measured according to the Ward method, and the MDA level was quantified by means of the Ledwozyw method. In the placental homogenates of women with gestosis, levels of lipoperoxidation products were not significantly changed in comparison to the control group. In patients with PIH, the blood serum concentrations of CD, HEPTE and MDA exceeded the control values by 48.28%, 169.47% and 57.59%, respectively (p < 0.001). Lipid peroxidation seems to play an important role in pathogenesis of the pregnancy associated hypertensive disorders.

[Concentration gradient of cotinine between blood serum and preovulatory follicular fluid].

OBJECTIVES: The aim of the study was to evaluate the blood serum/follicular fluid gradient for cotinine levels in the IVF patients with different levels of the tobacco smoke exposure. MATERIAL AND METHODS: Cotinine levels were measured by means of the double antibody RIA in blood serum (S) and preovulatory follicular fluid (FF) of 45 IVF patients undergoing standard controlled ovarian hyperstimulation. Self reported smoking status was used for grouping purposes. RESULTS: Mean (range) cotinine concentrations (in ng/ml) in follicular fluid of nonsmokers (n = 10), passive smokers (n = 25) and active smokers (n = 10) were 3.71 (1.20-15.62), 29.65 (20.91-45.75) and 285.69 (62.21-595.00), respectively. The differences were highly significant (p < 0.001). No significant gradient of cotinine concentrations was found between blood serum and follicular fluid. Cotinine levels in serum and follicular fluid were strongly correlated (rs = 0.892; p < 0.001). The S/FF ratio for cotinine levels did not depend on the follicle's size or serum cotinine. CONCLUSION: Cotinine easily crosses the blood/follicle barrier.

[Infrared thermographic imaging of normal vulva and uterine cervix: a preliminary report].

OBJECTIVES: To evaluate in the standardized conditions the thermal emission by normal uterine cervix and vulva. MATERIALS AND METHODS: Infrared telethermography (ITT) was used to examine vulva and uterine cervix in 32 women aged 24-54 years without colposcopic and cytologic abnormalities. RESULTS: The measured temperatures differed between different topographic points of vulva and uterine vaginal portio. The inter-individual variability of temperatures determined at the same vulvar structures was relatively low. CONCLUSION: The obtained results constitute a basis for further studies on thermovisual definition of therapeutic targets in cases of vulvar and cervical lesions.

Antioxidative capacity of preimplantation embryo culture medium declines following the incubation of poor quality embryos.

Total antioxidative capacity (TAC), a measure of overall free-radical scavenging potential, was determined by enhanced chemiluminescence in preimplantation embryo culture medium (PECM; pre-equilibrated Ham's F-10 medium supplemented with 7.5% patient's serum). Changes were evaluated in PECM TAC following a 24 h incubation of 66 single human embryos, as was TAC of patient's serum alone. The PECM TAC averaged 8.1% of the same patient's blood serum TAC. The percentage decline of PECM TAC over an incubation period of 24 h ranged from 0.9 to 41.7%, with a median of 5.5%. The decline in PECM TAC in different embryo quality groups was also studied. Embryos were categorized as 'good', 'fair' or 'poor' according to a scoring system based on an assessment of both the morphological appearance and developmental speed of the embryos. Incubation of poor quality embryos was associated with a decline in TAC, which was significantly higher than that observed in 'good' and 'fair' embryos. The findings suggest that impaired embryo development may be associated with an increased generation of reactive oxygen species by the embryo.

Selenium dependent glutathione peroxidase activity in human follicular fluid.

The purpose of this study was to evaluate the follicular fluid selenium status of in vitro fertilization patients in relation to their individual characteristics and treatment outcome. A total of 135 samples of follicular fluid were collected from 112 patients during transvaginal oocyte retrieval. Selenium levels and glutathione peroxidase (GSHPx) activity were measured in follicular fluid (FF) and blood serum. A selenium dependent GSHPx activity in follicular fluid has been demonstrated. Patients with unexplained infertility had significantly decreased follicular selenium levels as compared with those with tubal infertility or male factor. The mean GSHPx activity in follicles yielding oocytes which were subsequently fertilized exceeded that of the follicles with non-fertilized oocytes. Tobacco smoking resulted in significantly diminished follicular GSHPx activity. The Se/GSHPx pattern of the FF seems to be significant for the evaluation of the follicular milieu.

Amnioinfusion. A review.

The recent obstetric literature reports on various applications of amnioinfusion that have been used in an attempt to improve maternal and neonatal outcome. This paper reviews the indications, techniques, clinical benefits and hazards associated with the use of this procedure.

[Successful pregnancy outcome in a patient with acute renal failure superimposed on chronic renal failure treated with repeated hemodialysis].

A case is presented of successful pregnancy in a patient with acute renal failure superimposed on chronic controlled renal failure. The cause which led to development of acute renal failure were severe water-electrolyte equilibrium disturbances due to diarrhoea. Appropriately conducted conservative treatment in the period of oliguria, and early institution of treatment with repeated haemodialysis (taking into account the minimal fluctuations of body fluids, and adequate heparinization) were decisive for successful outcome of pregnancy in this patient.

[Ultrasonographic criteria of diagnosing tubal pregnancy].

The results were subjected to analysis of USG examination of 11 patients with tubal pregnancy, verifying the found echographic anomalies with the intraoperative assessment done shortly after the USG examination. Of greatest usefulness for the preoperative diagnosis of tubal pregnancy is, according to the authors' opinion, coexistence of the following sonographic signs: enlargement of the uterus without echographic sing of pregnancy in the uterine cavity;-a mass of irregular outline, cystic-solid appearance, and axial dimension of above 50 mm, situated near the uterus;-fluid in the Douglas pouch. The experience of the authors shows that ultrasonography is a valuable supplementation of clinical examination and laboratory analysis in the diagnosis of tubal pregnancy, on condition of using objective criteria for sonogram interpretation.
