[Single-stage exposure of the fistula in surgical treatment of the peri-proctitis abscess].

117 anorectal abscesses has been operated by one operateur. 68x a fistulectomy was done. 2x there has been a high fistula. Of the remaining 47 patients 44 have had a followup for 15 months. In this time 37 showed a fistula-in-ano. Procedure and the position of the fistulous tracks are shown. The assumptions and dangers of the single-stage approach are demonstrated.

Histometric features for the grading of prostatic carcinoma.

Histometric features for the objective grading of prostatic adenocarcinoma in histologic specimens were analyzed in five cases each of well, moderately and poorly differentiated lesions. Tissue sections from the selected cases were stained by the Feulgen method and digitized by a video-based microphotometer. Twenty total fields were recorded for each grade: ten at high resolution (an image sampling of 0.5 micron per pixel) and ten at low resolution (0.8 micron per pixel), with two fields per case recorded at each resolution. The images were segmented by an automated expert system-guided scene segmentation procedure. The performance of that procedure was measured by comparing the automated counts of nuclei in the segmented fields to the visual counts made by a pathologist in the same fields. For well, moderately and poorly differentiated cases, respectively, the nuclear counts made by the expert system at high resolution were 2.7%, 4.2% and 4.7% higher than the visual counts (as estimated from a total of 6,628 nuclei), but 1.2%, 2.5% and 1.1% lower at low resolution (10,329 nuclei). High-resolution features and tissue textural features were computed for each case. The high-resolution features showed good separation between the three groups of cases. The tissue textural features showed consistent separation between well and moderately differentiated cases. The relaxation of the spatial resolution (to 0.8 micron/pixel spacing) did not affect the selection of features, but led to less separation between the data from different grades. In conclusion, the automated system performed satisfactorily in distinguishing sections of prostatic tumors of varying degrees of differentiation.

Reversal of the cardiotonic and action-potential prolonging effects of DPI 201-106 by BDF 8784, a methyl-indol derivative.

1. We studied the interaction of the cardiotonic compound DPI 201-106 (4-[3'-(4''-benzhydryl-1''-piperazinyl)-2'-hydroxypropoxy]-1H-indole-2- carbonitrile; DPI) and its derivative BDF 8784 (2-methyl-4-[3'-(4''-benzhydryl- 1''-piperazinyl)-2'-hydroxypropoxy]-1H-indole; BDF) in isolated right ventricular papillary muscles of guinea-pig heart. 2. In contrast to the cardiotonic DPI, the methyl-indole derivative lacked a positive inotropic effect and even caused negative inotropic effects in concentrations above 1 microM. At 10 microM BDF significantly reduced the force of contraction and dV/dtmax, but did not affect action potential duration (APD). 3. Pretreatment of papillary muscles with BDF prevented the positive inotropic action of DPI in a concentration-dependent, but non-competitive fashion. At 10 microM, BDF prevented the inotropic effect of racemic DPI and shortened the DPI-induced prolongation of action potential duration. BDF similarly affected the inotropic and APD-prolonging effects of the sea anemone polypeptide ATX II. 4. In cardiac myocytes, DPI induced a tetrodotoxin (TTX)-sensitive, slowly inactivating inward current. The slow decay of this current was enhanced by BDF. In cells pretreated with BDF, DPI was not effective. BDF alone depressed the sodium and the calcium current. 5. In conclusion, the non-inotropic methyl-indole derivative BDF interacts with DPI noncompetitively at the sodium channels to abolish the inotropic and APD-prolonging effects of DPI, emphasizing the importance of the substituent in position 2 of the indole moiety for this effect.

[The piriformis syndrome].

A rare mentioned and therefore and often overlooked cause of low back pain with sciatica. The piriformis muscle arises from the anterior surface of the sacrum and inserts on the superior border of the greater trochanter. Functionally, the piriformis muscle is an external rotator and abductor of the thigh. Muscle spasm or on inflammatory process set into motion by the spasm cause an epineurial irritation of the sciatic nerve. The symptoms present almost identically to lumbar disk syndrome, except for the consistent absence of true neurologic findings. Diagnosis is accomplished by rectal palpation of myofascial trigger points within the piriformis muscle, also pain and weakness on resistant abduction-external rotation of the thigh. The essential therapy is the injection of local anesthetic and a small dose of corticoid into the precise focal point of hyperirritability deep in the belly of the muscle. Recurrences are uncommon.

[Robin-Silverman-Smith (fetal face) syndrome].

Report of a case of the fetal face syndrome associated with severe oligophrenia and septum pellucidum cyste in a male patient aged 30 years. The case demonstrates the good prognosis of the hypogenitalism resp. small genitalia; the correct diagnosis in this syndrome can the patients save a plastic of the penis.

Comparison of single- and dual-energy quantitative computed tomography for therapy control under antiosteoporotic treatment.

Lumbar spine bone mineral density was determined in 50 female patients (mean age: 55.4 +/- 12.3 years), who were treated because of osteoporosis or osteopenia. Bone mineral density was assessed using single-energy QCT with 80 and 120 kV tube voltage and with dual-energy QCT. Compared to single-energy QCT, dual-energy QCT showed much higher variability of the bone density values obtained. Therefore we conclude that single energy QCT is a method that allows precise and reproducible determination of spinal bone mineral content under clinical conditions which cannot be further improved by the use of dual-energy QCT.

Computed tomography in the determination of leg geometry.

In 286 adults with misalignment of the lower limb the length and torsional angles of the femur and tibia were determined using a method based on computed tomography. Depending on the location of the axial scan values for femoral antetorsion changed substantially. Normal values obtained in non injured extremities by this technique were an internal torsion of 23.5 +/- 8.6 degrees (mean +/- SD) in the femur and an external torsion of 34.9 +/- 8.6 degrees in the tibia. Mean intraindividual side differences in the femur and tibia were 4 degrees, the 95% confidence interval was 11.0 degrees in the femur and 12.3 degrees in the tibia. CT-methods can only be used for the determination of torsional angles in the femur and tibia if reproducible and standardized locations for the acquisition of axial scans are defined.
