Acupuncture in the treatment of bladder instability.

Twenty patients with lower urinary tract symptoms attributable to bladder instability or sensory urgency were treated with traditional Chinese acupuncture; 77% of patients with idiopathic detrusor instability were symptomatically cured. Urodynamic assessment after treatment showed no consistent changes and instability was abolished in only 1 patient. Bladder capacity rose in those who responded. Acupuncture is safe and free of complications and side effects. In patients with diurnal symptoms associated with idiopathic bladder instability it offers a therapy as effective as other non-invasive treatments.

Late failure of vasectomy after two documented analyses showing azoospermic semen.

From April 1970 to December 1980, 14 047 men underwent vasectomy for sterilisation under local anaesthetic at this clinic. In each man sterility was confirmed by two analyses of semen showing azoospermia. Allowing for a minimum follow up of three years, the wives of six of these men subsequently became pregnant between 16 months and three years after vasectomy. Analyses of semen confirmed recanalisation of the vasa deferentia in all six men. Only five similar cases initially fulfilling the same criteria for sterility have previously been reported. Full account of the rare possibility of recanalisation should be taken both when couples are counselled preoperatively and when pregnancy occurs after the male partner has been confirmed to be sterile.

Complications of vasectomy: review of 16,000 patients.

Sixteen thousand, seven hundred and ninety-six men underwent vasectomy between 1970 and December 1983 and have been reviewed. Post-operative side effects were few and significant complications were reported in 0.9%. Failure to achieve sterility occurred in 72 men, 69 of whom have been analysed. The early recanalisation rate was 0.36%. This rate was not influenced by the operative technique used, but varied markedly between individual surgeons. Experience and care with technique should result in a failure rate of 0.2% or better. There were six cases of late recanalisation in men previously thought sterile by two consecutive azoospermic analyses 4 months after vasectomy.

Inflammatory obstruction of oesophageal tubes.

Two patients, intubated for inoperable carcinoma of the oesophagus, recently presented with their tubes blocked by granulation tissue. This complication has not been previously reported. The tissue macroscopically resembled normal mucosa in one and recurrent tumour in the other. The presentation, aetiology, and significance of this granulation tissue is discussed.

Cooling: a new approach to bladder denervation.

Cooling tissues to temperatures close to zero preferentially destroys nerve. In vivo intracavity cooling of the urinary bladder of rabbits to temperatures between 0 degrees and 5 degrees C produced partial denervation of intramural nerves, assessed by silver staining techniques. All other tissues appeared normal histologically. Detrusor muscle function, assessed in vivo by cystometry and in vitro pharmacologically, was unaffected by this procedure, but 11 out of 18 rabbits showed long-term large increases in conscious bladder capacity following cooling.

The urodynamic characteristics of multiple sclerosis.

Urodynamic assessment was performed in 52 patients with urinary symptoms positively diagnosed as suffering from multiple sclerosis. Fifty-one patients had unequivocal bladder instability on filling cystometry the exception had inappropriate relaxation of the urethral sphincter at a small bladder capacity and low bladder pressure. Results of sphincter electromyography and synchronous cineradiography of voiding in a number of our patients led to the conclusion that many of these patients also suffer detrusor sphincter dyssynergia. There was no correlation between any measurable urodynamic parameter and the severity of the physical disability.

Radiographic patterns of pulmonary involvement in acute mycoplasmal infections.

The radiological appearances and the progress of the pulmonary manifestations of serologically-proven acute Mycoplasma pneumoniae infection were studied in 29 patients. The radiological changes seen at and after admission were not helpful in establishing an aetiological diagnosis except in three patients, in whom a "classical" pattern of mycoplasma pneumonia, based on the radiological appearnaces at different histopathological stages of the disease, could be recognised retrospectively. The patterns seen were varied and generally non-specific, and were not related to the age of the patient or to the duration of disease prior to admission. Specific anti-mycoplasma therapy may have prevented the progress of the disease after admission, although five of six patients in whom radiological deterioration occurred after admission were receiving such treatment.

The radiographic patterns of acute mycoplasma pneumonitis.

Twenty-two cases of serologically proven pulmonary mycoplasma infection have been reviewed. All showed abnormality on plain chest radiographs. A definitive diagnosis could not be made on initial or individual films. However, studies of sequential films enabled diagnostic patterns to be distinguished.

The operative cholangiogram: its interpretation, accuracy and value in association with cholecystectomy.

The value and errors of one method of operative cholangiography were assessed prospectively with the aim of reducing further the incidence of unnecessary explorations of the common bile duct and the occurrence of residual stones. Operative cholangiograms were obtained using a cystic duct cannula in 174 cases, the technical failure rate being 2%. All cases were followed up using intravenous cholangiography where necessary. Of 129 judged normal by the operator, 2 were found to have common bile duct stones-false negative rate 1%. Of 45 judged abnormal by the operator, 10 had negative explorations- false positive rate 6%. Anomalies were discovered in 12% of patients. It is concluded that meticulous technique and accurate interpretation are essential for the best results. The high incidence of anatomical variations present is of interest and this alone emphasises the necessity of carrying out the investigation and the need of experienced interpretation.
