A follow-up study on chemoimmunotherapy (5-fluorouracil and BCG) in advanced gastric cancer.

Two hundred and seventeen patients with advanced gastric cancer were classified according to the resectability of the tumour into two groups: I, resectable (non-radical surgery), 99 patients, and II, non-resectable carcinoma, 118 patients. Within each group patients were randomly assigned to receive 5-fluorouracil (5-FU) + BCG, 5-FU, or no further treatment (surgery only). BCG was given by scarification. A 2-year follow-up is reported. The group of patients with resectable tumours and receiving chemoimmunotherapy had a statistically significant prolongation of survival compared with the 5-FU and surgery only groups. No differences in survival were observed between these treatment modalities in patients with non-resectable tumour. These observations indicate that chemoimmunotherapy may be of benefit for a selected group of patients with gastric cancer.

"Activated" monocytes in gastric cancer patients. I. Increased Fc receptor expression, antibody-dependent cellular cytotoxicity and NBT reduction.

The Fc receptor expression, antibody-dependent cellular cytotoxicity (ADCC), and nitro-blue tetrazolium (NBT) reduction of peripheral blood monocytes from 150 patients with different stages of gastric cancer was assessed and compared with results obtained in 77 normal persons and 104 patients with non-malignant diseases of the gut. Monocytes of cancer patients showed an increased ability to form rosettes with human 0, Rh + erythrocytes coated with D-specific antibody. ADCC and NBT reduction were also elevated but no correlation was found with the stage of disease. However, all these phenomena were related to the tumor load as elevated values were the same 4-6 months after surgery in the unresectable-tumor group, while they decreased in patients with resectable tumors. These observations suggest that monocytes of some cancer patients are functionally altered ("activated") in the course of disease.

"Activated" monocytes in gastric cancer patients. II. Suppressor and cytostatic activity in vitro.

The spontaneous nitro-blue tetrazolium (NBT) reduction of monocytes from patients with gastric cancer was assessed and compared with an in vitro monocyte-mediated cytostasis of tumor cell lines and their suppressor activity. The increased NBT reduction correlated with the ability of monocytes to inhibit mitogen-induced normal lymphocyte response and cytostatic activity against L-1210 murine lymphoma cell line. These observations suggest that "activated" monocytes of some cancer patients may play the role of suppressor cells and exert an anti-tumor effect in vitro.

The Fc receptors of normal and cancer patients monocytes.

The ability of human monocytes from normal donors and gastric-cancer patients to form rosettes with "O" Rh+(D) human erythrocytes coated with hyperimmune IgG anti-D antibody (EAhu) and to kill the same target in antibody-dependent cellular cytotoxicity (ADCC) were assessed. Trypsin pretreatment of normal monocytes decreased their ability to form rosettes with EAhu complexes, but their ADCC activity was unaffected. The Fc receptor (FcR) expression and ADCC activity of monocytes of cancer patients were elevated, and trypsin-treatment led to their further increase. The elevated values were related to the presence of the tumour. These results may suggest that human monocytes possess trypsin-sensitive and trypsin-resistant Fc receptors. The trypsin-resistant FcR seems to be involved in ADCC phenomenon and to be preferentially expressed on monocytes of some cancer patients.

Effect of 15(R)-15-methyl prostaglandin E2 (arbaprostil) on the healing of duodenal ulcer: a double-blind multicenter study.

A multicenter study was conducted on 173 patients with active, endoscopically proven duodenal ulcers (158 men, 15 women). They were randomly assigned, in a double-blind manner, to two groups: those receiving placebo capsules (91 patients) and those receiving capsules containing 100 microgram of 15(R)-15-methyl prostaglandin E2 (arbaprostil) (82 patients). Each drug was ingested four times a day (1 h before meals and at bedtime) for 28 days. Endoscopy was performed on days 0, 14, and 28 after the trial began. At each examination, the ulcer size was measured and whether the ulcer had healed was recorded. Arbaprostil increased the incidence of ulcer healing to approximately the same degree as reported in most extensive studies with cimetidine. At 14 days, three times as many patients were totally healed in the arbaprostil-treated as in the placebo-treated group (37% vs. 12%, p less than 0.001). At 28 days, 67% of patients receiving arbaprostil were healed compared with 39% in the group receiving placebo (p less than 0.001). Similarly, the ulcer size, measured endoscopically, was much smaller after arbaprostil administration than in the group receiving placebo after both 14 and 28 days (p less than 0.001). Side effects attributable to treatment consisted primarily of loose stools and diarrhea (34%). Smoking retarded healing in the placebo-treated group (p less than 0.05), but did not significantly retard healing in patients treated with arbaprostil. We conclude that arbaprostil markedly accelerates the healing rate of active duodenal ulcers. This effect may be due to inhibition of acid secretion as well as gastric cytoprotection.

[Results of subjective and endoscopic studies on the incidence of upper digestive tract pathology among industrial workers].

This paper has been aimed at an evaluation of the prevalence of upper alimentary duct pathologies in an industrial population, as well as at an attempt to evaluate the value of subjective symptoms in screening tests. Altogether 975 subjects have been examined, but only 235 of them underwent endoscopic studies. The data on subjective symptoms have been collected from all the subjects, by way of a special questionnaire. In the group of those subjected to endoscopic studies, 38,7% have been found to have developed various diseases of the upper alimentary duct. Those predominating have been ulcers and deformations of pylorus and hiatal herniae. Ulcers and deformations of duodenum in smokers have been nearly twice those in nonsmokers. The results demonstrated that the prevalence of upper alimentary duct diseases in the industrial population concerned, has been higher than expected. To account for this, further epidemiological analytical studies must be undertaken. It has been found that within the screening studies of populations, aimed at the detection of upper alimentary duct diseases, questionnaire on abdominal pain is more reliable than the questions on dyspeptic symptoms.
