Parasellar and orbital apex syndrome caused by aspergillosis.

Patients with orbital aspergillosis commonly present with unilateral proptosis and associated sinus disease. However, neither of these manifestations was observed in a 62-year-old woman who had an orbital apex syndrome with visual loss, complete ophthalmoplegia, and corneal hypoesthesia. Pathologic examination of specimen from a granulomalike mass removed at left frontotemporal craniotomy showed branching hyphae with the characteristic appearance of Aspergillus.

Retinal atheromatous plaques: their recognition by elevating the intraocular pressure.

Recognition and further study of existing retinal atheromatous plaques in the central retinal artery may be enhanced by pressure on the globe. This simple maneuver should be used when patients with transient ischemic attacks (cerebral or ocular) are examined with or without atheromatous disease of the carotid artery. Previously unseen or suspected plaques may be brought into view. Plaques already visible may appear larger or vary greatly in both color and texture giving indication of their composition. The recognition of these atheromatous plaques is important in the diagnosis of atheromatous disease.

Ross' syndrome (tonic pupil plus).

Two cases of tonic pupil, hyporeflexia, and segmental hypohidrosis (Ross' syndrome) are reported. The relationship of this syndrome to other autonomic dysfunction is discussed. Those symptoms (emotional instability, loss of sweating, orthostatic hypotensive symptoms, and signs of bilaterality of the tonic pupil) which should alert the clinician to more extensive disease states are noted. It is suggested that these conditions may represent a continuum or spectrum of disorders with a widespread degree of severity and progression.
