Prednimustine v cyclophosphamide-vincristine-prednisolone in the treatment of non-Hodgkin's lymphoma with favorable histopathology: results of a national cancer care program in Sweden.

In a nationwide multicenter study, single-agent treatment with prednimustine (PM) was compared with combination chemotherapy with CVP (cyclophosphamide, vincristine, and prednisolone) in patients with stage III-IV non-Hodgkin's lymphoma (NHL) with favorable histology (Rappaport groups nodular lymphocytic poorly differentiated, nodular mixed, and diffuse lymphocytic poorly and well-differentiated). From January 1979 to May 1982, 217 evaluable patients were randomized, and at present analysis, the median follow-up time is 42 months. Overall response rate in the PM group was 63% (complete, 40%; partial, 23%) compared with 66% in the CVP group (complete, 32%; partial, 34%). There was no significant difference in relapse-free survival (RFS) or survival between the two groups, median RFS being 30 months and median survival being 42 months, respectively. Reclassification of the lymphomas according to the Kiel system (possible in 80%) revealed high-grade malignant NHL in 22 patients (12 in PM group, 10 in CVP group). Within this small group, there was a marked difference in survival in favor of CVP-treated patients. However, treatment with PM was less toxic and better tolerated than the CVP regimen. It was concluded that for patients with low-grade malignant NHL treatment with PM is equally effective and less toxic than the CVP regimen.

Cancer care programmes: the Swedish experience.

The present organization of oncological care in Sweden is described. In each health care region, there is an oncological centre with specific functions, including responsibility for running a regional tumour registry and for developing cancer care programmes for different types of tumours. The regional tumour registry is population-based, operates in close contact with the national central cancer registry and has, in addition to cancer registration, the key function of coordinating and evaluating cancer care programmes. Cancer care programmes are defined as agreed means of managing patients with a specific disease which are effective throughout a region and at all levels within a health care system. They represent the right of all patients with cancer within a health care region to equal, standardized care, regardless of their domicile. Such standardized management also forms an excellent basis for performing clinical trials within different cancer care programmes. Current cancer care programmes in Sweden are listed, together with some examples of patient accrual in trials within regional and national programmes.

CVP-remission-maintenance in stage I or II non-Hodgkin's lymphomas: preliminary results of a randomized study.

The effect of adjuvant combination chemotherapy when given to non-laparotomized patients in remission after radiotherapy in stage I or II non-Hodgkin's lymphoma was studied in a prospective randomized multicenter study. Locally extended field radiotherapy was given to a target absorbed dose of 40 Gy in 20 fractions. Fifty-five patients who were in complete remission 6 weeks after conclusion of radiotherapy were randomized to either no further therapy or to 9 cycles of CVP (cyclophosphamide + vincristine + prednisolone). The relapse-free survival at 30 months was 41% for patients without and 86% for patients with adjuvant chemotherapy (p = 0.02). The survival was the same for both treatment arms, being 90% at 30 months. Fifteen patients have relapsed, 14 of them with extensions and 1 with a recurrence within the radiation target volume. Analysis of subgroups showed that adjuvant chemotherapy in the present series significantly prolonged the relapse-free survival in diffuse histiocytic lymphoma.

An on-line method for the acquisition of medical information for computer processing as applied to radiotherapy.

Based on a structured medical record, specially designed for patients with malignant disease, an on-line data capture system has been developed. This enables the collection of virtually any type of information contained in the patient's case notes. The structure of the record is described, with actual examples. The record is typed on a typewriter terminal linked to a mini-computer. Data is recorded as code + heading + value string. The headings are identified automatically, and an internal code generated, describing the type of information. Record keeping according to the principles described was introduced in clinical routine at the department in 1971. Data collection was implemented later that year, using an off-line magnetic tape encoder (IBM MT72). The system has been developed further and converted to a versatile on-line system. The data base, collected with these systems, now contains data on about 20,000 patients.

Early oral contraceptive use as a prognostic factor in breast cancer.

The survival of 193 premenopausal breast cancer patients was investigated in relation to their history of early use of oral contraceptives. The women were born in 1939 or later and diagnosed in the southern health care region of Sweden. Women, who had started their oral contraceptive use (OC-use) before 20 years of age had a significantly lower survival rate as compared with those who had never used OC and late users (p = 0.02 and = 0.04 respectively, generalized Wilcoxon test). For women who started OC-use between 20 to 25 years of age, a tendency for a shorter survival was seen in comparison with women who had never used OC (p = 0.18). For all patients simultaneously, the relative risk adjusted for age at diagnosis increased for earlier OC-start. When only stages II and III were considered in a stratified multivariate model, a significantly elevated risk was seen for early users of OC irrespective of age or of adjuvant treatment given. The estrogen and progesterone receptor concentrations of the primary tumor were significantly lower among early users (p = 0.001 and p = 0.05 respectively).
