[Clinical anophthalmos. Cosmetic outcome after 2 years therapy with an orbital expander for stimulating orbital growth].

BACKGROUND: Congenital anophthalmos is a rare condition in which intervention in an early age can stimulate orbital expansion. The therapeutic goal was to allow retention of a suitable prosthesis and to maximise facial symmetry. METHODS AND PATIENTS: We report on 3 anophthalmic newborn male patients, of which one presented with microphthalmos in the other eye as well as associated systemic disorders. Solid moulded shapes were adapted and manufactured out of methylmethacrylate to increase expansion of orbital soft tissue and bone. The moulding of the orbita was performed by silicone made of two components under general anaesthesia during the first two years of life. The following day the solid shapes were fitted in the outpatient department. They were increased in size when they started to rotate within the orbit or when the eyelids were relatively loose around the solid shape. The initial insertion of the orbital expander was performed in the first weeks after birth and was repeated every 3 to 6 weeks within the first year of life. The degree of orbital expansion was determined by measurements of the horizontal eyelid length, the volume of the solid shapes, by measuring the volume in cm3 of water displaced by the shape after submerging it in a cylinder of water and assessment of the cosmetic aspect. RESULTS: Within 8 months of therapy a horizontal eyelid lengthening to 10 mm and an increase of the volume of the orbital expander from 1.5 cm3 to 6 cm3 was achieved in patient 1. After 20 months of therapy patient 2 showed symmetry of the face and the horizontal eyelid length. In patient 3 therapy started 3 years late and after 2 years only a horizontal eyelid lengthening of 3 mm and a suboptimal symmetry of the face was achieved. CONCLUSION: Our own experience suggests that early prosthetic fittings are necessary for an ideal cosmetic outcome. Motivation and cooperation of the parents is an important factor to achieve optimal results.

[Mathematical investigations of the functional tooth axis].

The functional tooth axis is--contrary of the anatomical tooth axis--defined as follows: The effect of force in its direction causes a maximum of desmodontal fibres being stressed on drawing. After measure of an upper caninus, an upper and a lower molar mathematical models of the teeth and the desmodont of force are calculated with a computer simulation. A variety of functional tooth axes which are not identical with the anatomical ones, result for each tooth. The possible functional axes yield a variety which signifies the biological tolerance. This tolerance causes that many individual forms of occlusion don't have any pathological effect on the desmodont.

[Thermal dimensional stability of dental registration wax].

Three bite registration waxes were tested for their thermic dimensional behavior under several heating and cooling conditions. Aluwax showed the smallest changes in length. Difficulties of application are expected because of its low softening temperature. There were greater dimensional changes with Beauty Pink Hard-Wax, but Kerr No. 8-Wax showed the greatest changes. The cooling method had a small, the heating temperature, however, a strong influence on the thermic dimensional behavior. These are the recommendations for the clinical application of the tested waxes: The bite registration material should not be heated more than necessary for achieving a sufficiently plastic quality for the registration. It should be stored in ice water to be resistant enough to deformation during the model mounting.

[Recording pattern of occlusal indicators. I. Influence of indicator thickness, pressure, and surface morphology].

There are no data in the literature about the recording pattern of occlusal indicators and the relevant methods. We therefore studied the recording patterns of four foils, six paper materials, and four silk qualities at different pressures and surface morphologies. Foils are the thinnest indicator materials and give more accurate readings than paper and silk. However, under smaller loads their marking capacity is worse. This means that higher pressures must be applied for the clinical use of foils. The drawbacks of the more intensively marking papers and silk are their greater thickness and less flexible base material. This leads to a greater number of pseudo contact markings.

[Precision of semiadjustable articulators].

The precision of 11 semiadjustable articulators was examined. The results indicate that for reproducible centric position it is necessary to practise centric lock. Neither guidance of the condylar element into centric relation by hand nor devices like feathers do secure against deviation. When putting usual pressure on the articulators all showed considerable differences.

[Computer tomography of the TMJ versus clinical and instrumental analysis of function].

Data obtained by computer tomography of the TMJ were compared with those of clinical and instrumental analyses of function. 25 females and 37 males aged between 19 and 47 years (mean age, 24.8 years) were examined. None of the patients complained of functional problems subjectively, but clinical analysis of function showed several signs of stomatognathic dysfunction to be present. Results of CT did not correlate with those of clinical and instrumental analysis.

[Recording occlusal indicators. Introduction to occlusal surface materials and roughness].

The quality of 4 foils, 6 papers and 4 tapes being used for marking occlusal contacts are examined. The influence of surface material and the roughness of surfaces are demonstrated. Indicators on plastics colour best, those on ceramics worst. As a rule unpolished surfaces show better results than those being extremely polished. Foils are the most exact markers. But they are less apt for smooth surfaces of ceramics and amalgam as well as for the rough surface of gold-alloys. In those cases thin articulating papers have preference.
