Element composition of tubule cells in the inner stripe of the renal outer medulla.

To obtain further insight into renal medullary function, element concentrations were determined in individual tubule cells of the outer medulla in the rat kidney using electron microprobe analysis on freeze-dried cryosections. In the cells of the thick ascending limb of Henle's loop the Na, P, Cl, and K concentrations (means +/- SEM) were: 9.5 +/- 0.6, 158.4 +/- 6.2, 25.6 +/- 1.2, and 135.3 +/- 4.8 mmol/kg wet weight, respectively. While similar Na, P, and K concentrations were observed in light and dark cells of the medullary collecting duct, Cl was markedly higher--55.0 +/- 2.8 mmol/kg wet weight--in the dark cells. The electrolyte concentrations of the thick ascending limb cells seen in the present study are in good agreement with ion activities reported for the isolated perfused thick ascending limb. The low cell Na and Cl concentrations provide a favorable driving force for passive cell entry of Na, Cl, and K across the apical membrane via the Na-2Cl-K cotransporter even at low tubule fluid NaCl concentrations. Although in hydropenic rats interstitial tonicity of the inner stripe is above isotonicity, electrolyte concentrations of inner stripe cells did not differ from those obtained in cortical tubule cells. This finding suggests that, similar to papillary cells, osmoadaptation of outer medullary cells is, at least partially, accomplished by organic osmolytes.

Interactions between sodium salicylate and acetyl salicylic acid evaluated using ADP induced platelet aggregation and bleeding time.

1 g acetyl salicylic acid orally significantly enhanced the initial rate of platelet aggregation induced by 1 mumol/l and 2.5 mumol/l ADP. Sodium salicylate was without effects on the platelet aggregation and specifically it did not prevent acetylsalicylic acid from inhibiting the secondary aggregation. Sodium salicylate was without effect on the bleeding time and did not inhibit the prolongation induced by acetyl salicylic acid. Our study does not lend support to the concept of an important interaction in vivo between acetyl salicylic acid and its first metabolite salicylate in man.

Decreased platelet sensitivity to prostacyclin (epoprostenol) during hemodialysis.

Evidence is accumulating that activation of platelets occurs during conventional hemodialysis. In ten uremic patients, the ADP-induced platelet aggregation and the platelet sensitivity to prostacyclin (PGI2) were examined before and after 120 and 240 minutes of dialysis. The ADP-induced aggregation was enhanced during dialysis reaching statistical significance after 240 minutes compared to predialysis values. The platelet sensitivity to PGI2 was significantly diminished during dialysis. Our results are in agreement with the concept of platelet activation during hemodialysis and lend support to the clinical use of PGI2 during hemodialysis.

Cigarette smoking shortens the bleeding time.

The cutaneous bleeding time was shortened after smoking high nicotine cigarettes while not after smoking nicotine free cigarettes. The ADP induced primary platelet aggregation was not enhanced. The number of circulating platelet aggregates did not change due to smoking.

Nitroglycerin prolongs the bleeding time in healthy males.

Bleeding time was prolonged when measured 6 minutes after the administration of 0.5 mg nitroglycerin (NTG) sublingually in 15 men aged 25-40 years. The prolongation was not caused by a general inhibition of platelet aggregation. 4 g acetylsalicylic acid inhibited the effect of NTG on the bleeding time with a time course similar to the vessel wall cyclooxygenase. This may indicate that the effect of NTG is mediated by prostacyclin.

Thrombosis of the subclavian vein--a rare complication of transvenous cardiac pacing.

Thrombosis of the central veins as a complication of transvenous cardiac pacing fortunately is rare. A case is presented in which symptomatic thrombosis of the subclavian vein arose seven weeks after implantation of a new electrode. The frequency, pathogenesis, clinical picture, complications and treatment of the condition are briefly discussed.

Lactate dehydrogenase isoenzyme 1 in the diagnosis of myocardial infarction.

A new immunochemical method for the measurement of lactate dehydrogenase isoenzyme 1 (LDH-1) was used on 113 consecutively admitted patients with suspected myocardial infarction. Using WHO's criteria and our routine program 49 were classified as having myocardial infarction and 64 as having no myocardial infarction. LDH-1 was better than total lactate dehydrogenase is discriminating between patients with and without myocardial infarction (p less than 0.02), and the efficiency (the number of correct classifications divided by the total number of analyses) on day 2-3 equalled that from the literature for CK-MB on day 1. Total lactate dehydrogenase and LDH-1 are supplemental to creatine kinase (CK) and CK-MB, while aspartate aminotransferase may be omitted in the diagnosis of myocardial infarction. Determination of the ratio of LDH-l to total lactate dehydrogenase offered to no advantage.
