[The rights of the hospital patient: the knowledge and perception of their fulfillment on the part of the professional. The Group in Catalonia of the Spanish Society of Care for the Health Services User].

BACKGROUND: The Customer Service Department includes the protection of the patient's rights. To analyze the work performed in this setting, we planned to quantitate and evaluate the knowledge and perception of fulfillment by medical staff. METHODS: Multicentric cross-sectional study. The population included health care professionals in nine Catalonian hospitals. The sample was selected at random with reposition and was segmented according to professional category. Data collection was carried out by personal interview supported by a questionnaire. Differences according to institution size were analyzed. RESULTS: A total of 1,014 professionals were interviewed; 84.4% reported to know the patient's rights and 64.4% to observe them. Significant differences (p < 0.05) according to institution size were observed. CONCLUSIONS: Discussion and diffusion of patient's rights is an useful tool to improve knowledge and evaluate the perception of its fulfillment by health care professionals.

The increased skeletal muscle protein turnover of the streptozotocin diabetic rat is associated with high concentrations of branched-chain amino acids.

Experimental streptozotocin-induced diabetes resulted in important changes in body weight which were associated with abnormalities in water and food intake. In addition, diabetic rats showed a clear muscle atrophy involving a decrease in both skeletal muscle size and protein content. This was accompanied by a marked loss of total carcass nitrogen. These changes were related to important alterations in protein turnover in skeletal muscle. Thus, the diabetic animals showed changes in the fractional protein rates of both synthesis (decreased by 37%) and degradation (increased by 140%). The increased protein degradation observed in the muscle of the diabetic animals was associated with important changes in the concentration of both circulating and muscle amino acids. Interestingly, the diabetic animals did not show important changes in either liver or kidney protein turnover rates, in spite of having a clear increase (over 50%) in kidney mass. In addition, and although the total amino acid concentration was not affected by the diabetic state, the chemically induced diabetic animals showed important elevations of branched-chain amino acids (leucine, isoleucine, and valine) in both blood and skeletal muscle. Similarly, important decreases in the blood concentrations of glutamate+glutamine, alanine, glycine, proline, serine, and threonine were also observed. These observations reinforce the idea of the association between muscle protein wasting, increased protein turnover, and alterations in branched-chain amino acids previously proposed by our group.

[Stroke incidence in Asturias, 1990-1991].

BACKGROUND: Stroke, with a rate of 121/100,000 was the second cause of death in Asturias, Spain. Here we communicate the results of a population based stroke register in the oriental health districts of Asturias. METHOD: The study took place between October 1, 1990 and May 30, 1991, covering a population of 652,549. Surveillance was both active and passive and included the public and private hospitals, and the primary health care. Death certificates could not be checked due to legal reasons. We defined case following the WHO criteria. RESULTS: During the eight months of study 708 case occurred. The crude incidence rate of first stroke was 132.4/100,000 and that of any stroke, 162.7/100,000. The age adjusted incidence to the world population was, respectively, 61.3/100,000 and 73.6/100,000. There were no differences in the incidence across sexes. Age showed a strong effect in cerebral infarctation and less marked in intracerebral hemorrhage Infarcts are very rare in women before 50. Distribution of stroke type was: infarct: 77%; intracerebral hemorrhages: 14%: subarachnoid hemorrhage: 4% and unspecified stroke: 4%. Almost 50% of the strokes occurred in the morning hours, from 6:00 am to 12:00 am also during the cold season, winter and fall, the proportion of stroke was higher than during the warm season. CONCLUSION: Stroke is an important cause of morbi-mortality in Asturias. The magnitude of mortality is larger than the expected from the incidence. It maybe explained by a high case/fatality ratio in Asturias due to proportion of hemorrhagic stroke, the ageing of the population and the distribution of competing causes of death. However, we cannot rule out a small incomplete registration of cases, mostly in patients over 85. We estimated the incidence of any stroke, given underegistration, to be 176/100,000 for any stroke.

[Study of the relationship dengue-pregnancy in a group of cuban-mothers].

The current worldwide situation of dengue infection, and particularly in the American continent, is analyzed taking into account the increasing risk for infection during pregnancy which exists in endemic areas; the consequences of an eventual vertical transmission of the virus are practically unknown yet. We studied 59 women who gave birth to their children between 5 and 9 months after the "peak phase" of the type 2 dengue virus epidemic which broke out in Cuba in 1981. Sera of the mothers and newborns were studied through various serologic techniques in order to detect antibodies; in some cases we tried to isolate the virus, without success, from serum samples taken from the newborns; among these, we found 4 cases with positive IgM. These newborns showed neither apparent fetal damage at birth nor further alterations in a follow-up carried out when they reached 5 years of age. The need of carrying out further studies contributing to establish an adequate management of pregnant women infected by dengue virus is discussed.

[Histological evaluation of cryosurgery in high grade intraepithelial neoplasia (CIN-III)] of the uterine cervix].

Sixty patients, having high grade cervical intraepithelial neoplasia (CIN III) are analyzed. They are all treated primarily with cryosurgery followed, approximately 12 weeks thereafter, by cervical conization, in order to histologically evaluate the response to treatment. The mean follow up of the whole series was 36 months. Ninetieth five percentage of the patients were included in the series, having an abnormal PAP smear, suggesting presence of a CIN. Colposcopically directed biopsy prior to cryosurgery showed grade CIN III, with no glandular involvement in 61.7% of the cases. In 38.3% the endocervical glands were involved, cryotherapy was done in 60 patients, with single freezing in 26 (43.3%) and double freezing in 34 (53.7%) patients. On analyzing the cone specimen, 71.7% of the cases showed no residual disease. The group that received double freezing did better than the single one (79 versus 61%). As well, the group with out gland involvement did better than the group showing such condition (83.8 versus 52.2%). Cryosurgery is a good therapeutic modality in cases with CIN III, following street selection criteria and having a good and reliable follow up.

[Fish silage from shrimp by catch fish: I. Preparation and biological evaluation].

Fish silage was prepared from some fish species of the shrimp by-catch caught in Golfo Triste, Carabobo, Venezuela. Fish were ground until a particle size that would guarantee the liquefaction process was achieved. The process was accelerated by the incorporation of a 3.5% w/w acid mixture, formed by diluted sulfuric acid (1:3) and concentrated formic acid, in a proportion of 1:4. The silage process occurred spontaneously at room temperature during 17 days. Chemical, physical and microbiological tests were conducted in order to characterize both the raw material and the final product. The raw material used had adequate levels of freshness, so that it was suitable for use, thus producing a first-grade silage. A first biological evaluation was carried out in rats in order to estimate the protein quality of the silage by means of certain biological parameters such as PER, NPR and apparent digestibility percentage of the protein. The results of the first experimental period demonstrated that the fish silage protein was of optimal quality, since the biological response in rats was similar to that of the control group fed the casein diet.

[Fish silage prepared from fish species of shrimp by-catch. II. Biological test in broilers].

The purpose of this study was the nutritional evaluation of proteins in fish silage. A biological test was conducted in chicks (broilers) to evaluate two levels of inclusion of fish silage (2.5% and 5%, respectively) in diets normally used for feeding them. Chicks were evaluated in weight gain, food consumption, and feeding conversion index. Finally, flavor and acceptability tests of meat from broilers fed the experimental diet were conducted. Results of the biological assay indicated that the best biological response was that of the animals fed the 5% fish silage diet. Sensory tests revealed that the meat of chicks fed the experimental diets had favorable acceptance by the consumers.

[Cleft lip and palate in Campeche Mayas].

It has been suggested that among American Indians, as in some genetically-related Asiatic ethnic groups, incidence of cleft lip and/or cleft palate is higher than among people of Caucasian extraction. Such hypothesis, plus growing demand for services observed at a center for the surgery of cleft lip and cleft palate in Campeche state, led the authors to undertake research among the Maya residents of that region. However, neither careful review of case histories nor field research performed in several Indian communities could confirm the hypothesis of a higher incidence among this ethnic community.

Ultrastructural study of glycogen containing cells in colonic adenocarcinomas and precancerous polypoid lesions.

A previous report of members of our group demonstrated, in most adenocarcinomas and some adenomatous polyps with areas of malignant transformation, the presence of glycogen in cells that failed to show the synthesis of ordinary mucosubstances. The presence of glycogen in non-differentiated epithelium of the digestive tract of embryos, where it precedes ordinary mucus secretion, has lead some authors to suggest that the changes to glycogen secretion that occurs in malignant colonic cells, reflects the general regression to a less differentiated level of structure and function. It is well established that patients with carcinomas of the colon classified as less differentiated or higher grade tumors have significantly less chance of cure and consequently a worse prognosis than patients with lower grade tumors. The present ultrastructural study of the morphological differentiation characteristics of the glycogen containing cells previously encountered, revealed a strong association of glycogen to undifferentiated cells of the colonic adenocarcinomas and precancerous polypoid lesions studied. This undoubtedly seems to encourage the testing for the presence of glycogen in routine procedures as a possible marker of diagnostic and prognostic significance.

Histochemical and ultrastructural study of human colon preneoplastic stages.

Fine structure and function are closely related in the cells of the different tissues. For this reason a comparative study combining histochemical techniques and scanning electron microscopy can result a potentially useful tool in the search for cell markers of preneoplastic stages of the colon mucosa. With this purpose, samples of normal colon mucosa, adenomatous polyps and adenocarcinomas obtained by colonoscopy were studied by histochemical techniques for the detection of epithelial mucosubstances and by scanning electron microscopy. Differences were shown in the architectural arrangement of the colon epithelium between the normal mucosa, the adenocarcinoma and the polyps studied, while between tubular an tubulovillous adenomas no marked differential ultrastructural features were found. Acid mucosubstances distribution pattern showed that sialomucin predominated in the colon mucosa harboring a carcinoma and in the tubulo-villous polyps as well as in the mucosa remote from adenocarcinoma and from the two cases of familial polyposis coli histologically considered as tubulo-villous adenomas. On the other hand tubular adenomas and normal mucosa showed a predominance of sulfomucin.

Burkitt's lymphoma in Cuba. I. Clinical and morphological features and EBV association.

The first 7 cases of histologically and cytochemically confirmed Burkitt's lymphoma have recently been studied in Cuba. Some of them presented with facial bone involvement and high anti-Epstein-Barr virus (EBV) antibody titers characteristic of endemic African cases. Others showed features of nonendemic cases. The distribution of patients in these two groups may be in relation to the behavior of the healthy Cuban population with respect to the EBV virus.
