A simple maneuver to replace dislodged laparoscopic port.

Inadvertent dislodgment of laparoscopic port can occur despite preventive measures. We describe a simple maneuver to replace a dislodged port using a palpation probe. The principle is analogous to that of using a laparoscopic grasping forceps. It is easy to perform, quick, reliable, safe, and particularly useful for the laparoscopists in training.

[The urology department].

The Department of Urology of Erasme Hspital, University of Brussels, has participated to the development and implementation of new technologies that have significantly transformed the specialty in the last 25 years. The minimally invasive treatment of benign prostatic hypertrophia was developed by the use of Trans-Urethral Needle Ablation (TUNA) and new pharmacological treatments. Treatment of urinary stones by extracorporeal shockwaves lithotripsy and Endourology has allowed to avoid operating hundred of patients each year. In the field of prostate cancer, an original laparoscopic prostatectomy technique by the extraperitoneal approach was developed in our unit. The prevention of prostate cancer and the influence of various nutritional factors and its early diagnosis by the use of different markers have been the subject of numerous publications and have contributed to improve our knowledge in this field. Different prognostic factors of bladder cancer have been evaluated and in particular their place with the use of intravesical BCG. A better understanding of the different mechanisms involved in erectile dysfunction has been the subject of numerous studies during the last 20 years and the department is internationally recognised as a reference centre in this field.

Extraperitoneal laparoscopic radical prostatectomy. Results after 50 cases.

INTRODUCTION: After an initial experience using transperitoneal laparoscopic radical prostatectomy as described by Vallancien and Guillonneau, we developed a pure extraperitoneal approach. This approach seems more comparable to the open technique and avoid potential risks of specific complications due to the transperitoneal approach. We evaluated the perioperative parameters (blood loss, operating time, transfusion rate) and postoperative results (oncological results, continence and potency) after our first 50 cases. MATERIAL AND METHOD: Between September 1999 and September 2000, we performed 50 laparoscopic radical prostatectomy. On average, patients were 63.3 years old (range 47-71), had preoperative mean PSA values of 9.14 ng/ml (1.1-23). Median Gleason score was 6 (4-10) with 2.5 (1-6) positive biopsies for a mean prostate volume of 40 cm(3) (17.5-95.0). Clinical stage was T1, T2a, T2b and T3 in 46.3, 41.5, 9.8 and 2.4% of the cases, respectively. We used a pure extraperitoneal approach and we performed a descending technique starting with the dissection at the bladder neck. The seminal vesicles dissection is comparable to the open approach. RESULTS: 42 extraperitoneal and 8 transperitoneal procedures were performed (2 in the initial experience, 3 because of previous abdominal surgery and 3 because of incidental peritoneal opening). Mean operative time was 317 min, mean blood loss 680 cm(3), transfusion rate of 13%. 1 patient/50 was converted to an open procedure. Pathological stage was pT1a, pT2a, pT2b, pT2c, pT3a and pT3b in 2.2, 8.5, 42.5, 2.2, 34 and 10.6% of cases, respectively. Positive surgical margins were observed in 22% of cases. The potency rate after neurovascular bilateral bundle preservation was 43% at 3 months (n = 7) and 67% at 6 months and (n = 6) without any further treatment. The continence rate (no pad) was 39% at 3 months and 85% at 6 months. Detectable postoperative PSA at 3 month was observed in 2 patients only. Two major complications occurred: one acute transient renal failure one uretrorectal fistula at day 20. CONCLUSIONS: The extraperitoneal laparoscopic radical prostatectomy results seem comparable to transperitoneal laparoscopic radical prostatectomy or open surgery. This approach is reproducible and seems to avoid the potential risks of intraperitoneal injury. Long-term follow up and comparative series are however necessary to further evaluate these new techniques.

[A new approach in techniques to treat urinary incontinence: TVT (tension free vaginal tape)].

The indications and the surgical technique of a newly developed operation for the correction of female urinary incontinence (the Tension free Vaginal Tape) are described. This technique is based on a new etiological concept of urinary incontinence and ensures the patient a reduced morbidity (local or regional anaesthesia) and hospital stay (theoretically adapted for a one day clinic). Mid-term results (84% success rate after a median follow up of 3 years) are promising but must be confirmed at the long term. We relate our experience of 30 cases with a success rate of 93% but entached with a high rate of postoperatory urinary urgency. This leads us to emphasise a strict adherence to the indications and an absolute respect of the technique as originally described.

Improving the prognostic value of histopathological grading and clinical staging in renal cell carcinomas by means of computer-assisted microscopy.

The present work aims to refine prognosis in cases of renal cell carcinoma (RCC) by integrating a variety of parameters with the prognostic information provided by histopathological grading and clinical staging, carried out on a series of 97 RCCs. To this end, Feulgen-stained RCC cell nuclei were characterized by means of 38 variables describing nuclear DNA ploidy levels and morphology. All of these data were subjected to a principal components analysis. On the basis of this multivariate analysis, Fuhrman grade II was subdivided into grades II- and II+, and Fuhrman grade III into grade III- and III+. The same kind of subcategorization was performed in the case of the T2 and T3 clinical stages. The results show that the classification into grade II- and III- RCCs correspond to a more favourable prognosis than grade II+ and III+, to which shorter survival periods were attributable. Similar results were obtained for the subcategorization of the T2 and T3 clinical stages. Very simple biological characterizations of these grade- or stage-related RCC groups were obtained by means of a decision tree approach applied to the cytometry-generated variables. The resulting classification rules were validated on a new series of 18 patients and enabled very accurate predictions of survival.

The importance of measuring the prostatic transition zone: an anatomical and radiological study.

OBJECTIVE: To assess the accuracy and reliability of measurements of the volume of the transition zone (TZ, representing the hypertrophied benign component) and whole prostate by TRUS in patients with BPH or cancer, by comparing the radiological with pathological findings after surgery. PATIENTS AND METHODS: The study comprised 36 patients with prostate cancer undergoing radical prostatectomy and 34 patients with symptomatic BPH treated using retropubic adenectomy. The weights of the radical prostatectomy specimens and of the enucleated adenomas were correlated with the corresponding volumes of the TZ and of the whole prostate, respectively, measured by TRUS using the prolate ellipsoid method. RESULTS: The mean (sd) TZ volume measured by TRUS was 36.9 (25.48) mL, whereas the weight of the enucleated specimen was 42.7 (33.58) g (correlation coefficient r=0.95, P<0.001). The TRUS estimate of the volume of the whole prostate was 29.2 (9.24) mL, while the radical prostatectomy specimens weighed 34.5 (10.76) g (r=0.78, P<0.001). The variability in the TZ volume estimate ranged from -17% to +18%, whereas the variability for whole prostate was -21% to +30% (P<0.05). CONCLUSIONS: Measurements of the TZ of the prostate by TRUS are more accurate than those for the whole prostate. An assessment of the TZ volume may be sufficiently reliable to be used in the clinical management of BPH and to detect prostate cancer using the prostate-specific antigen density of the TZ as a marker.

[PSA level of the transitional zone: a new marker especially reliable for the detection of prostatic cancer].

The extensive use of serum prostate specific antigen in early PCa diagnosis has been limited by the far from perfect sensibility and specificity of this test. The value of prostate specific antigen density (PSAD) to enhance prostate cancer detection in patients with PSA serum levels below 10 ng/ml is limited due to controversial results. We have compared the value of PSAD of the transition zone (PSA-T) dividing serum PSA by transition zone volume to the PSAD for early prostate cancer prediction in patients with PSA levels under 10 ng/ml. PSAD and PSA-TZ were calculated in patients with histologically proved located prostate cancer or with benign histology. We have defined a cut-off value of 0.35 ng/ml/cc for PSA-TZ with both a sensitivity and a specificity in predicting prostate cancer of nearly 90%. For PSAD around 0.15 ng/ml/cm3, PSA-TZ value was significant in prostate cancer. This study showed that PSA-TZ could be a new accurate parameter for prostate cancer prediction in patients with PSA below 10 ng/ml.

Conservative management of ureteric stones.

The authors review the current conservative management of ureteric stones. The physiopathology of the renal colic is analyzed with its implications in the medical treatment. The role of NSAID is enhanced. Stone size and stone location are to be considered when evaluating the possibility of spontaneous passage of the stone. Stones less than 3 mm in diameter of the lower ureter will pass spontaneously in 90% of the cases while stones more then 6 mm in the upper ureter will not pass in most cases. The role of stone dissolution in uric acid and cystine stones is discussed. SWL is not controversial in the management of upper stones less than 15 mm in size. The authors report their experience with SWL of ureteric stones in upper, middle or lower ureteric stones with a success rate of respectively 87%, 65% and 84%.

[Medical treatment of ureteral calculi].

Antiinflammatory drugs are the first choice in the treatment of the acute nephretic colic. This is due to their fast and direct action on the ureteral wall. The use of antispasmodics are still controversial and opioïds are not indicated. During the acute crisis, an hydric restriction should be associated to the medical treatment. After the crisis an increase of diuresis could help to "wash out" the stone. A spontaneous elimination can be expected, especially if the stone is small and located in the third part of the ureter. The ureteral rupture is rare but serious and must be treated by antibiotics and some time to be drained. The rapidity of a more aggressive treatment is function of numerous factors.

Acute Disease↗

[Calculi: interventional treatment].

Extracorporeal shock wave lithotripsy is actually the most popular treatment in the urolithiasis. However, different factors influence the choice of the best treatment. Great progresses appeared in the conception of performing endoscopic material which led to minimize morbidity. Percutaneous approach is very popular but require expensive material and trained operator. Indications for open surgery were reduced but didn't completely disappear. We review in the next paragraph the literature of the last five years.
