Neural stimulation for spinal spasticity.

The knowledge of neural plasticity is used to improve motor function of the spinal cord by chronic stimulation of the dorsal columns. We achieved this with an improvised technique. The electrodes are exteriorized and stimulated by using a Grass stimulator. Five paraplegic patients with different aetiologies causing their paraplegia were treated by this method, and the details are enumerated. Four had good improvement and relaxation was maintained for varying periods after cessation of the stimulation. Mode of action is discussed.

Carbohydrate metabolic studies during twelve months of treatment with a low-dose combination oral contraceptive.

Carbohydrate metabolism was evaluated in twenty healthy women volunteers using a low-dose combination oral contraceptive (OC) containing 30 micrograms of ethinyl estradiol and 500 micrograms of dl-norgestrel by measurement of serum glucose and insulin levels during 3-hour oral glucose tolerance test (OGTT) before and after 3, 6 and 12 months of medication. There were no significant differences in body weight or blood pressure between pretreatment and posttreatment. Fasting serum glucose levels were slightly reduced, though not significantly, during all periods of treatment. But serum glucose levels were increased at 1, 2 and 3 hours in association with high insulin responses during OGTT in all periods of OC therapy, indicating mild to moderate insulin resistance. These data suggest that the low-dose combination OC used in the study exerts alterations on carbohydrate metabolism in women during one year of OC use.

Stereotactic cingulumotomy for drug addiction.

This paper analyzes the results of surgical treatment of 73 patients with drug addiction. They were treated by lesions in the anterior cingulum. Morphine, Pethidine (meperidine) and alcohol were the common drugs of addiction. Selection of cases, rationale for surgery and operative technique are discussed. Follow-up varied from 1 to 6 years. Results of surgery indicate this procedure as the most promising one for cure of drug addiction.

Post-traumatic middle cerebral artery occlusion: case report and review.

Occlusion of extracranial carotid and vertebral arteries following closed craniocervical trauma is well known. However occlusion of intracranial arteries following closed head injuries is infrequently reported. In fact only 25 cases of post-traumatic middle cerebral artery occlusion have been adequately documented. This entity bears re-emphasis not only because of its rarity but also because of its clinical similarity to other intracranial mass lesions.

Stereotactic surgery of the limbic system in epilepsy.

During the routine follow-up of results of stereotaxic surgery in the patients treated for behaviour disorders, it was found that some of them had a bonus relief from seizures, Out of 260 amygdalotomies, 54 hypothalamotomies and 5 thalamolaminotomies performed for behavioural problems, 107 patients suffered from associated seizure disorder. 76 of them (71%) showed considerable relief from seizures following surgery. This is detailed. It is hypothesized that probably these nuclei of the "limbic system" either initiate and/or propagate seizure activity. This view is supported by the "improvement" in EEG in addition to the clinical improvement seen in these patients.

Why hemispherectomy?

A series of ten cases of infantile hemiplegia with behaviour disorders and with of without fits treated by unilateral stereotactic amygdalotomy is described. Out of the eight cases with fits good releif was seen in six and amelioration of the seizures in two. The behavioural disorders were well controlled in all the patients. When this procedure is compared with hemispherectomy, it is felt that operation of stereotactic amygdalotomy can be done as the procedure of first choice. Some hypotheses have been advanced to explain the modus operandi of the operation.
