[Transcranial duplex scanning in estimation of cerebral hemodynamics in patients with secondary cochleovestibular neuritis].

The authors present pilot experience with transcranial duplex scanning (TCDS) of the cerebral vessels in 17 patients with secondary cochleovestibular neuritis combined with chronic leptomeningitis (primarily of the posterior cranial fossa). This method has determined extra- and intracranial hemodynamics, characterized arterial and venous blood flow qualitatively and quantitatively, outlined possible genetically determined factors in development of dyscirculatory disorders. Finally, hemodynamic defects of two types were revealed in 12 patients. Type 1 (3 patients) was characterized by asymmetric circulation in the territory of the middle, anterior and posterior cerebral arteries, by the absence of venous congestion. Type 2 (9 patients) was characterized by impaired venous outflow manifesting as higher speed of the blood flow and its phase response along the intracranial veins and sinuses. In 7 patients these defects combined with asymmetric circulation along brain stem arteries. These patients were diagnosed to have perilymphatic labyrinthine hydrops.

[Chronic tonsillitis: therapeutic and diagnostic approaches].

The article is a brief review of clinical and topographic anatomy of the palatine tonsils. Their role in formation of general and local immunity is shown. Etiological and pathogenetic aspects of chronic tonsillitis are considered: most frequent causing agents, underlying pathogenetic and immunological mechanisms. B.S. Preobrazhensky's and V.T. Palchun's clinical classification of chronic tonsillitis is presented. New approaches to etiopathogenetic treatment of chronic tonsillitis are described.

[The study of aerobic bacterial background and antibiotic-sensitive isolated strains in the ENT department].

Aerobic microbes in ENT departments were studied. The following strains were isolated: Staphylococcus, Streptococcus, Enterococcus, Acinetobacter, Klebsiella, Pseudomonas, Flavobacter, Citrobacter, Enterobacter, Escherichia, Aspergillus, Candida. 54.5% of the isolated strains were of hospital origin. Comparative evaluation of antibiotic resistance has shown a wide spectrum of resistance of the isolated microorganisms to antimicrobial drugs, among those to the drugs used in the ENT departments.

[Ligenten treatment of sinusitis].

Efficacy of local treatment with ligenten which was introduced after irrigation of the nasal sinuses was tried versus control intranasal introduction of 1% dioxidin solution. 35 and 25 patients with acute sinusitis or aggravation of chronic purulent sinusitis were treated, respectively. Subjective and objective responses in the study and control groups assessed with the use of sensor-analogue and visual-analogue scale demonstrated higher efficacy of ligenten.

["Otinum" in the therapy of middle ear diseases].

Otinum, a new drug made in Poland, was used to treat inflammatory diseases of the middle ear of 40 patients. The drug was applied topically as drops or tampons inserted into the external acoustic meatus. As controls, 16 patients with identical problems, who were treated with 3% boric acid alcohol, were used. Otinum had a distinct pain-killing and anti-inflammatory effect, as well as a mild keratolytic effect. Otinum is recommended to be used for acute otitis media and exacerbated chronic otitis media in the absence of pathogenic microorganisms.

[Correlations between changes in hearing and cerebral hemodynamics in patients with chronic alcoholism].

Seventy eight chronic alcoholics, aged 19 to 50 years, were exposed to audiologic and rheoencephalographic examinations. Tone hearing impairment was found in all subjects. The recruitment or greater than normal increment of loudness was primarily identified in patients of Group 1 that comprised those who consumed alcohol from 1 to 10 years. Rheoencephalographic examinations showed decreases of the rheographic index as well as the tone of large and medium caliber arteries in all patients. Group 2 patients who consumed alcohol for more than 10 years displayed greater changes in the vessel tone than Group 1 patients. Analysis of rheoencephalographic waves demonstrated that peripheral and venous tone was decreased in Group 1 patients and increased in Group 2 alcoholics. Visual analysis of rheoencephalographic waves showed lability in the vessel tone of Group 1 patients and a trend for a vessel spasm in Group 2 patients. The results give evidence that Group 1 chronic alcoholics develop toxic lesions of receptors of the hearing organ which is confirmed by the occurrence of recruitment, sound perception impairment, close to normal speech discrimination and insignificant changes in cerebral hemodynamics. Group 2 alcoholics develop marked disorders in cerebral hemodynamics leading to changes in central and intermediate compartments of the hearing organ.

[Syndromology and differential treatment of secondary cochleovestibulitis in patients with early clinical forms of cerebral circulatory insufficiency].

Altogether 136 patients with cochleovestibulitis and early clinical forms of cerebral circulation insufficiency were examined. Two types of syndrome were identified that were associated with the pathogenetic development: vascular and metabolic. Different diagnostic criteria and therapeutic approaches as applied to each type of syndrome were developed.
