A study of hyperthermia was performed after injection of 0.5 mg/kg of MMC into feeding artery of Walker 256 implanted 4 days (early stage) or 8 days (advanced stage) earlier into the s.c. dorsum side of hindpaw of Wistar rats. The tumor growth curves on 6 days after treatment by warming tumor in hot water (44 degrees C) for 60 minutes were inhibited in advanced stage group which formed tumor vessels, but not in the early stage which had not still formed them. An intraarterial injection of warmed 0.01 to 0.05% Noradrenaline solution (44 degrees C, 10 ml) after chemotherapy (0.5 mg/kg of MMC a.i.), by which the temperature did not rise in the tumor tissue but rose to 40 degrees C for 5 to 10 minutes in the tumor vessels, showed severe cytotoxic damage in advanced stage group. The warming target of the tumor for hyperthermia can be considered to be the tumor vessels. Warmed 5% glucose using 0.1 mg of Noradrenaline (500 ml, 44 degrees C) injected into the tumor feeding artery after chemotherapy (2 to 6 mg of MMC or 250 mg of 5-FU a.i.) was tried in advanced cancer patients (liver tumor 4 cases, pancreas tumor 1 case, metastatic bone tumor 1 case, and multicentric tumor in liver, gall bladder and pancreas 3 cases). Efficacy was noted in 8 cases at the rate of 72.7% (8/11). It was concluded that warmed Noradrenaline solution delivered to the tumor vessels after chemotherapy for advanced cancer resulted in good efficacy rate as a from of chemo-thermotherapy.
The purpose of this study was to evaluate the influence of minor surgery conducted in the oral cavities of children. Changes in blood pressure and pulse during minor surgery were measured using a Pulse-Wave Korotokoff Sound Record Meter GP-303S (PARAMA). The subjects consisted of 28 children (15 boys and 13 girls) who underwent minor surgery at the pediatric dental clinic of Iwate Medical University. An examination of 16 of the 28 children to determine the relationship between their physique, personality, and blood pressure was made. The following results were obtained: 1) The maximum blood pressure formed a peak at the time of the impacted supernumerary teeth extraction. This was more conspicuous in boys than in girls. The pulse rose to the highest values with the infiltration of the anesthesia, the incision, and stripping. However, no significant values based on sex difference were demonstrated. 2) The maximum blood pressure changed more intensely during surgery in boys than in girls. This was distinctly observed during extraction of the impacted supernumerary tooth. 3) Both the maximum blood pressure and pulse increased with the progress of the surgery. After the surgery, it returned nearly to the presurgical level. With extraction of a supernumerary tooth and marsupialization, the return of the blood pressure to the preoperative level was more delayed in boys than in girls. 4. The changes in blood pressure and pulse seemed to be mostly induced by mental disquietude and stress such as anxiety and fear of surgery. 5. The physique and personality were related to the changes in blood pressure and pulse during minor surgery in the oral cavity.
We report an anesthetic experience of a 63-year-old female patient with blue rubber bleb nevus syndrome who underwent an open reduction of the fracture of the right femur. The syndrome is characterized by bluish nevus scattered throughout the skin of the whole body and angiomas of the gastrointestional tract causing serious bleeding. Anesthesia was induced with thiopental followed by intravenous injection of succinylcholine chloride and maintained with neuroleptanesthesia in nitorous oxide and oxygen. The course of anesthesia and emergence from anesthesia were uneventful. Anesthetic management of patients with blue rubber bleb nevus syndrome was also discussed.
Case histories of three elderly patients with a malignant lymphoma of the small intestine are described. Case No. 1 involved a 80-year-old male with a diffuse malignant lymphoma of medium size that was associated with a jejunal perforation. On the 64th day after surgery, he died of heart failure during the course of chemotherapy. Case No. 2 was a 76-year-old male with a diffuse malignant lymphoma of medium size in the ileocecum. He left hospital after surgery had relieved his symptoms, although 8 months after he developed active pulmonary tuberculosis during the course of chemotherapy. Case No. 3 involved a 76-year-old female with a diffuse malignant lymphoma of the mixed cell type in the terminal ileum. She died of heart failure complicated by a pulmonary disease on the 36th postoperative day. The therapeutic results of treating the malignant lymphoma in these 3 aged patients were all unfavorable. Thus, it is felt that special care should be taken in treating aged patients with this kind of disease.
A case of disseminated trichosporonosis associated with Candida infection of the urinary bladder is presented. Coffee bean shaped or crescent shaped yeast-like elements are characteristic of Trichosporon and useful in differentiating Trichosporon from Candida but such histological features are less efficient than the immunohistochemistry in identifying mixed fungal infection. In the present case, an application of avidin-biotin-complex (ABC) method with anti-Trichosporon antiserum and anti-Candida antiserum enabled us to diagnose a mixed infection by both fungi.
An 81-year-old woman was admitted to our clinic due to gross hematuria. A large bulky pedunculated mass was found in the bladder by cystoscopic examination. Subtotal cystectomy and bilateral cutaneostomy was performed on January 12, 1987. Histologically the tumor was composed of carcinomatous and sarcomatous elements. The carcinomatous element was composed fundamentally of grade 2, transitional cell carcinoma with numerous foci of squamous metaplasia. The sarcomatous element was composed of myxosarcomatous, chondro-sarcomatous pattern and non-differentiated malignant spindle cell component. Immunohistochemical examination demonstrated the presence of cytokeratin and epithelial membrane antigen in the spindle cell and more obvious carcinomatous regions, using the avidin-biotin conjugated immunoperoxidase technique. The patient died 3 months after operation. Autopsy findings showed multiple organ metastasis which were composed of carcinomatous and sarcomatous elements.

A female-specific late-lethal mutation, fle(3)100, of Drosophila melanogaster was examined on its effects in XX-XO mosaics, and its interaction with various alleles of sex-determination genes tra-2 and dsx. Mosaic flies homozygous for fle(3)100 appeared at a high frequency (more than 80%) compared to the control heterozygous flies. Mosaics, however, rarely appeared when they had relatively large XX tissues. A significant reduction of the frequency of XX genotype was observed in the second and third abdominal sternites, suggesting the presence of a focus or foci of primary gene action in the internal organ or tissue near these structures on the blastoderm fate map. Chromosomal females doubly homozygous for fle(3)100 and tra-2 or dsx were not rescued. Females homozygous for fle(3)100 as well as those doubly homozygous for fle(3)100 and tra-2 died as pharate adults. Unexpectedly, chromosomal males doubly homozygous for fle(3)100 and tra-2 showed variations in viability ranging from about 20% to 100% depending on the alleles of the tra-2 locus examined. These males also died as pharate adults.
In our previous paper, we reported a marked advantage in using collagen gel matrix available for cell culture in combination with a silicone tube as an effective environment for axonal sprouting during peripheral nerve regeneration. In the present experiment, collagen film was substituted for the silicone tube because of its non-toxicity, biocompatibility and better availability. Also, the surgical procedure was simplified, using a fibrin adhesive system instead of suturing. In the second postoperative week, severed proximal and distal stumps became joined together concomitantly with absorption of the collagen matrix and film. On size-frequency histograms, the diameters of both myelinated and unmyelinated axons at 8 weeks after surgery had recovered to their normal ranges. These findings demonstrate that this procedure of enveloping a collagen matrix and severed nerve stumps in bioresorbable collagen film would be an ideal way of forming a perfect perineurium. The regeneration of peripheral nerve axons resulted in normal thickness of the original nerve bundle without exception, unlike the axons on the control side.
A 44-year-old male patient under treatment for a duodenal ulcer for the past 9 years visited our clinic because of a sudden pain in the epigastric region. He was diagnosed as having a perforated duodenal ulcer, so an emergency laparotomy was performed. In this operation, the coexistence of a gastric carcinoma was clarified, and a subtotal gastrectomy with an R2 dissection was done at the same time. The coexistent gastric carcinoma was localized to a size of 20 x 12 mm within the gastric proper mucosa. The infiltrated range of the cancer was as wide as 120 x 80 mm, extending to the proper tunica muscularis. This case was seen as manifesting a relative early stage of scirrhous carcinoma.

A new experimental method to test the effect of drugs on tumor-induced osteolysis using a bladder tumor in mice has been designed. The method consisted of inoculating MBT-2 tumor cells s.c. over the calvaria in mice, resulting in a local tumor causing fragmentation of the bone. This was accompanied by adjacent osteoplastic changes, which were evaluated by X-ray and histological examination. Etidronate disodium (EHDP), a diphosphonate derivative, at a dose of 3 mg/kg to 30 mg/kg s.c. protected the bone by decreasing the extent of osteolysis as judged by the same criteria. Therapy with EHDP prolonged the survival period. This inhibition was obtained with no apparent effect on the growth of the MBT-2 tumor.
The present study is a chronological morphological examination on the effects of collagen gel matrix on regeneration of severed sciatic nerves. The nerves (5 mm length) were resected, and both the distal and proximal stumps were inserted into a silicone tube with 5 mm gap in between. In the test side, the gap in the tube was then injected with liquid collagen which gels in the tissue when reconstructed with a certain buffer solution. The gap space in the tube of the control side was left empty. In a chronological examination of the tissue in the tube, considerably more rapid growth of sprouting axons toward the distal stump in the test side was revealed in comparison with the control side. The cells, including both fibroblasts and larger Schwann cells, were less in number. More orderly directions were observed in the collagen matrix than in the control tube. The result indicates that regeneration of the peripheral nerves in the silicone tube can be improved, by using appropriate exogenous fine materials, collagen matrix.

The Wenckebach phenomenon of entrance block in intermittent atrioventricular junctional parasystole is reported for the first time in a 40-year-old woman. In this case the presence of dual pathways in the atrioventricular junction is demonstrated. One of them is a pathway without conduction disturbance. The other is a pathway containing both the ectopic focus and the site of second-degree entrance block. This site is located a considerably long distance above the focus. Entrance block occurs also at some site below the ectopic focus, which is a part of the pathway containing the focus. It is suggested that entrance block in this lower site might exist during the whole ectopic cycle.
