[Diagnostic and clinical value of Doppler sonography of fetal blood vessels].

Fetal Dopplersonography is a not generally applicable additive method in prenatal care. Its main value is the non-invasive access to the fetal cardiovascular system. Previous studies show a diagnostic gain in sonographically and clinically pathologic pregnancies, mainly those with intrauterine growth retardation. The validity of a general screening of all pregnant women is poor. The examination of fetal physiology, pathophysiology as well as of the influence of drugs on fetuses and pregnant women will profit from Dopplersonography and fetal blood sampling.

[Destructive bladder mole--follow-up by human chorionic gonadotropin (HCG), ultrasound and digital subtraction angiography (DSA)].

A case report of a mola destruens is presented including the clinical monitoring and the therapeutical conclusions. Control parameters were the HCG, ultrasound and DSA. The DSA in a new and improved technique, provided helpful information during a critical phase of the disease, when changing from mono- to polychemotherapy was necessary. Furthermore, the excellent vascular mapping enables planning of radiological interventions. In case of tumour bleeding hysterectomy can be avoided by embolisation. There may thus be more time for conservative treatment by chemotherapy.

[Molecular biological demonstration of parvovirus infection in fetal tissue].

We report on the morphological findings in 16 fetuses with serologically confirmed maternal parvovirus B19 infection. Typical routine morphological findings of the hydropic fetuses were the presence of abnormal erythroblasts with typical nuclear inclusions. These infected cells positively stained immunohistochemically with antibodies against a recombinant virus protein, as well as they ultrastructurally contained virus particles. Using in-situ hybridization techniques, we were able to demonstrate the presence of parvovirus B19 genome in the infected cells, while no other cell type was shown to contain virus genome. According to our results the differential diagnosis of fetal parvovirus B19 infection should be considered in each case with hydrops fetalis of unknown origin. The careful routine microscopic examination of fetal tissue may provide evidence for parvovirus infection which should be confirmed by in-situ hybridization analysis.

Successful intrauterine treatment of fetal hydrops caused by parvovirus B19 infection.

The infection with parvovirus B19 during pregnancy causes in 1/3 of the cases an aplastic crisis in the fetus and consecutively generalized fetal hydrops as a result of severe anemia. Some cases of fetal hydrops and intrauterine death are reported. In our two cases, the fetal therapy by intrauterine intravasal transfusion was successful and the children developed normal. The techniques and indications for fetal blood sampling and the method of intrauterine intravasal transfusion are explained.

[Studies on dichlorvos (DDVP). III. Testing of dichlorvos for cocarcinogenic activity in mice].

In order to clarify whether or not Dichlorvos (DDVP)--which did not exert carcinogenic effects in mice in our experiments--is cocarcinogenic, male and female mice of the strain C57Bl/6/Bln were subcutaneously injected with the carcinogen N-Nitrosodiethylamine (NDEA) and received in addition DDVP orally. For comparison NDEA and DDVP alone was administered to other groups of mice. The combined application (NDEA + DDVP) did not result in increased incidences of tumors and preneoplastic lesions as compared with the NDEA-treated groups of mice. The incidence of focal (transitional cell) hyperplasias of the urinary bladder epithelium was increased in the groups treated with NDEA + DDVP as compared to the groups with single compound treatment. There were no development of tumors and no differences in the latency periods of tumors which could be attributed to the combined treatment with NDEA + DDVP. Under these experimental conditions DDVP was not cocarcinogenic in mice.

[Pregnancies in females following kidney and pancreas transplantation].

Uraemic patients are in general infertile. Ovarian function is, however, restored after successful renal transplantation, thus making conception possible. We followed up 14 patients after renal transplantation involving 16 pregnancies. Two patients became pregnant twice, one with twins and the other following renal and pancreatic transplantation--the first recorded in the world. Caesarean section was performed in all patients due to increasing serum creatinin levels, avoid pre-eclampsia or premature rupture of membranes. Both, mother and child in all cases progressed without complications, although these pregnancies are associated with high risk for both. Therefore, a close co-operation between the mother, the nephrologist, the transplantation centre, the gynaecologist and the paediatrician is a prerequisite for a possible favourable course.

[Sonographic diagnosis of a case of type 1 achondrogenesis in the 2d trimester].

The authors report on a prenatal ultrasonic diagnosis of lethal osteochondrodysplasia achondrogenesis type I (Parenti-Fraccaro) in the 17th week of pregnancy. The prenatal findings were confirmed by necropsy after termination of the pregnancy. The possibility to recognize lethal skeletal disorders early in pregnancy is discussed as well as the patho-anatomical criteria and possible patho-physiological mechanisms of achondrogenesis.

[Clostridium infection in the puerperium following cesarean section].

Clostridium perfringens infections in the puerperal period are rare. A 22-year old patient, after caesarean section at another hospital, was admitted to our clinic showing clinical signs of haemolysis, slight uraemia, a crepitation of tissue and sonographical signs of air bubble formation in the uterus. Since clostridium perfringens infections show a high mortality rate, early operative measures under high-dose Penicillin treatment are indicated. In this case, hysterectomy and salpingectomy were performed. Both ovaries were unaffected and could be conserved. In addition, a peritoneal lavage was done. Our patient was discharged as cured after a postoperative course without any complications. There is no evidence in the literature for the efficacy of either antitoxin treatment or a high oxygen therapy.

Pena-Shokeir phenotype with major CNS-malformations: clinicopathological report of two siblings.

Clinical and pathological features of two siblings of opposite sex with the Pena-Shokeir phenotype are reported. A detailed account of the prenatal and dysmorphological findings is given in one case. A broad range of deformations regarded as secondary to fetal hypokinesia was present, including a number of yet unreported findings. One case showed additional endocrine hyperplasia and left lung trilobation. Both siblings displayed extensive, highly similar CNS-abnormalities. The type and convergence of these malformations differ from previously reported cases and characterize a new familial subtype of the Pena-Shokeir phenotype.

[Microbial mutagenicity testing of N-nitrosoiminostilbene and N-nitrosoiminodibenzyl, the nitrosation products of the drugs carbamazepine and trimipramine hydrochloride].

The active agents of the drugs Finlepsin and Herphonal, carbamazepine and trimipramine, were nitrosated under simulated human gastric conditions. For both formed N-nitroso compounds, N-nitroso iminostilbene (N-nitroso-5H-dibenz(b,f)azepine) and N-nitroso iminodibenzyl (N-nitroso-10,11-dihydro-5H-dibenz(b,f)azepine), tests for mutagenic potency in the Ames-test gave negative results.
