Evaluation of human bladder tumors by ultrastructural morphometric techniques.

To estimate the malignancy of the human urinary bladder tumors, various cellular components of normal and neoplastic bladder epithelial cells were examined using morphometric techniques at the electron microscopic level. Nuclear to cytoplasmic ratios were found to be 1:5 in normal epithelial cells, 1:4 in superficial grade 1 (G1) tumor cells, 1:3 in superficial grade 2 (G2) tumor cells and 1:2 in invasive grade 3 (G3) tumor cells. Nuclear volumes were largest in G3 tumor cells, but no differences were seen in cell volumes between normal epithelial and tumor cells. The volumes and surface areas of the rough-surfaced endoplasmic reticulum (rER) and lysosomes per cell decreased progressively from G1 to G3 tumor cells. The volume density of tonofilaments was increased in G1, but decreased in G2 and G3 tumor cells. The cisternal thickness of the rER and Golgi complex was also increased in G1 tumor cells. From these results, it was concluded that morphometric analysis may be useful in evaluating the degree of differentiation and invasive potential of human bladder tumor cells in the low and intermediate risk groups.

Immunocytochemical demonstration of S phase cells by anti-bromodeoxyuridine monoclonal antibody in human prostate adenocarcinoma.

Using a monoclonal antibody to bromodeoxyuridine and immunohistochemistry, we measured the incorporation of this thymidine analogue into the deoxyribonucleic acid of human prostate adenocarcinoma cells exposed in situ. Fifteen patients with prostate cancer were given an intravenous infusion of 500 mg. bromodeoxyuridine at needle biopsy to label tumor cells in the deoxyribonucleic acid synthesis phase (S phase). The tumor specimens were fixed with 70 per cent ethanol, embedded in paraffin, sectioned and stained by an indirect immunoperoxidase method using anti-bromodeoxyuridine monoclonal antibody as the first antibody. The results showed that this method demonstrated bromodeoxyuridine-labeled nuclei satisfactorily in tissue section. The bromodeoxyuridine labeling index, S phase fraction, was determined by counting the number of bromodeoxyuridine-labeled cells in the tissue sections. Grade 3 tumors averaged 4.37 +/- 0.48 per cent labeling versus 2.41 +/- 0.49 per cent in grade 2 tumors, and grade 1 tumor in the series had an S phase fraction of 1.36 +/- 0.39 per cent. The average S phase fractions for single gland, cribriform, fused and medullary were 1.16, 2.30, 3.74 and 4.95 per cent, respectively. The results obtained with S phase fraction measured with bromodeoxyuridine labeling proved to be comparable to the results of histological grade and growth pattern. Thus, the higher S phase fraction may indicate biological malignancy. Moreover, the degree of heterogeneity concerning S phase fraction distribution within prostate cancer tissue could be compared to the morphological appearance. Our preliminary results suggest that the measurement of bromodeoxyuridine labeling index in prostate cancer may prove to be a new objective and quantitative assay of biological potential of individual tumor.

[Detectability and characteristics of the primary adrenal tumor by ultrasonography--comparison with CAT scan and adrenal scintigraphy].

From March 1978 to May 1987, 22 cases of primary adrenal tumors were examined by ultrasonography. The patients ranged in age from 21 to 79 years with an average of 42.6 year, including 11 cases of pheochromocytoma, 6 cases of primary aldosteronism, 4 cases of Cushing's syndrome and 1 case of ganglioneuroma. Visualization of tumors was successful in 19 of the 22 patients (86.4%). The detecting rate of right adrenal tumors was 100% (12/12), while the rate of the left was 80% (8/10). The sizes of detected adrenal tumors were larger than 10 mm on the right and larger than 20 mm on the left. Obesity and bowel gas degraded the image of left adrenal tumors, resulting in a lower detective rate of the tumors. The suprarenal area and small lesions were best demonstrated by a lateral intercostal approach using longitudinal scanning plane and by a right subcostal approach using transverse scanning plane. In pheochromocytoma, cystic lesions (echo free space) existed, and the fact that no cystic lesion in other adrenal tumors was helpful for diagnosis of pheochromocytoma. It is concluded that ultrasonography is a very useful method for diagnosis of adrenal tumors as an initial imaging procedure, because it is totally non-invasive, rapid and less expensive than all of the other techniques.

S phase fraction of human bladder tumor measured in situ with bromodeoxyuridine labeling.

A total of 18 patients with transitional cell bladder cancer was given a 0.5-hour intravenous infusion of bromodeoxyuridine at the time of endoscopic biopsy or transurethral resection to label tumor cells in the deoxyribonucleic acid synthesis phase (S phase). The tumor specimens were fixed with 70 per cent ethanol, embedded in paraffin, sectioned and stained by an indirect immunoperoxidase method with anti-bromodeoxyuridine monoclonal antibody as the first antibody. The bromodeoxyuridine labeling index, S phase fraction, was determined by counting the number of bromodeoxyuridine-labeled cells in the tissue sections. All grade 1 tumors had an S phase fraction of lower than 10 per cent. The average S phase fractions for noninvasive (11 cases) and invasive (7) tumors were 9.8 and 20.0 per cent, respectively. Two distant metastatic bladder tumors showed an average S phase fraction of 25.3 and 30.0 per cent. Thus, transitional cell bladder cancers with an S phase fraction of greater than 10 per cent appears to grow faster and be more invasive more often than those with an S phase fraction of less than 10 per cent. The higher S phase fraction may indicate greater biological malignancy. Our preliminary results suggest that measurement of the bromodeoxyuridine labeling index in bladder tumors may be a new objective and quantitative assay of biological potential of individual tumors.

[Subrenal capsule assay for testing the effectiveness of anticancer drugs].

This study compared the histological changes induced by human bladder tumor xenografts following their implantation in the subrenal capsular space of immunocompetent mice (CDF1, 6-10 weeks age). Ten human bladder tumors were studied by light microscopic and electron microscopic methods. All materials were obtained from cold-cup biopsy using endoscopic procedure. Single fragments (1 mm X 1 mm X 1 mm) were implanted on Day 0 under the renal capsule of each mouse. Animals were serially sacrificed from day 2 to day 6 and in some cases as late as day 10. All kidneys were sectioned up to 3 levels at the xenograft and stained with appropriate histological reagents. The results of this study were; 1) infiltration of the tumor with mononuclear cells and neutrophils began on day 4, 2) pretreatment with 200 mg/kg cyclophosphamide before implantation reduced cell infiltration into the xenograft, 3) complete rejection of the tumor on day 8, 4) electron microscopic study showed minimum changes of intracellular organs on day 2, 5) but microscopic architecture was preserved in the tumor explants removed on day 4. This morphological study suggested that histological evaluation on day 4 might be a suitable method of subrenal capsule assay using immunocompetent mice. We therefore tested genitourinary carcinomas to determine their sensitivities to commonly use chemotherapeutic agents using histological criteria. Groups of tumor-bearing mice were treated daily for 3 days. Sensitivities for assays were assessed on day 4 by comparison of histological score. Of the 15 cases evaluated, 20% demonstrated significant oncolytic activity. These data also suggested that a clinically relevant chemosensitivity assay might enhance the selection of appropriate therapy.

[A case of malignant paraganglioma within the renal sinus].

A case of malignant paraganglioma within the renal sinus is presented. A 51-year-old woman underwent left transabdominal nephrectomy for renal tumor. Pathologically, the tumor was malignant nonchromaffin paraganglioma. This tumor seemed to originate from parasympathetic paraganglions around the left renal artery. Paraganglioma is rare in the literature. Especially, this may be the first report of such a lesion in the renal sinus.

[A case of gas gangrene in a vesico-vaginal fistula].

A case of gas gangrene is presented. The patient was a 78-year-old woman who was admitted with the diagnosis of vesico-vaginal fistula. Cystography revealed a vesico-intestinal fistula and leakage of contrast medium into the prevesical space, in addition to the vesico-vaginal fistula. Right ureterostomy was performed. Two weeks postoperatively, she complained of severe pain and swelling of her right thigh. The swelling grew rapidly and general condition became worse. A diagnosis of gas gangrene was made 8 days after her first complaint by demonstrating subcutaneous and intramuscular gas formation in X-ray. She died the next day. By the bacteriological examination, this case was non-clostridial gas gangrene.

Survival of patients with tumors of the renal pelvis and ureter: a retrospective study.

A review of 27 patients with renal pelvic tumors and ureteral tumors admitted to the Tsukuba University Hospital was performed retrospectively. Nineteen cases of renal pelvic tumors and eleven cases of ureteral tumors were submitted to the study. These patients were treated mainly by surgery. The rates for 1-year, 3-year, 5-year and 10-year survival were 88.5%, 57.6%, 40.3% and 20.2%, respectively. No obvious relationship between prognosis of renal pelvic tumor and that of ureteral tumors was seen. Pathologically, there was a correlation between the grade and the stage of the tumor. Prognosis was related to the stage of the tumor, but not to the grade of the tumor. Clinically, the patients with positive urinary cytology had a poorer prognosis. It was considered that no relationship existed between the preoperative clinical stage and the postoperative pathological stage. Consequently, the former is not considered a prognostic factor. We have been performing mainly radical nephroureterectomy with regional lymphadenectomy since 1975. The patients who received the operation with regional lymphadenectomy since 1975. The patients who received the operation with regional lymphadenectomy or with adjuvant chemotherapy had a better prognosis. It is concluded that lymphadenectomy is a valuable operation for improving prognosis.
