Remarkable responses of metastatic renal cell cancer in multiple organs treated with alpha-interferon.

We report a case of renal cell carcinoma with metastases in the lungs, humerus and brain, which we treated with alpha-interferon. After a transperitoneal nephrectomy the patient received a series of intramuscular injections of alpha-interferon every 2 days. Two months after the initial therapy complaints of pain in the humeral bone gradually decreased and the palpable mass in the right brachial region disappeared completely. A computerized tomography scan revealed only the scar of the intracranial metastasis. Most of the pulmonary lesions disappeared 4 months later except for the metastasis at the apex of the left lung.

[A case of giant cell glioblastoma].

The pathological entity of intracranial giant cell tumor remains controversial, which has been considered a glioma (giant cell glioblastoma) by some and a sarcoma (monstrocellular sarcoma) by others. The authors present a case in which CT scan shows a wedge-shaped high density area at the left parietal lobe. This tumor expands explosively only a month on serial CT scan. The microscopic speciemen demonstrates the bizarre irregular giant cells with large vesicular nuclei, and glial proliferation and abundant reticulin fibers. Then we reviewed and discussed with emphasis on the historical aspect of giant cell tumor and also radiologic, therapeutic problem.

[A case in which alpha-interferon showed remarkable effect against the brain, bone, and lung metastases arising from the renal cell carcinoma].

A 37-year-old man was sent to our department from the Radiology Department on July 19, 1984 with the diagnosis of right renal cell carcinoma with metastases to the right humerus and bilateral lungs. He was nephrectomized immediately and administered 10 X 10(6) units of alpha-interferon i. m. every other day for 3 months during the admission and 30 X 10(6) units once a week for more than 3 months after discharge. Although in addition to humeral and lung metastases, brain metastasis was found a week after the operation, interferon showed a remarkable effect and produced get a complete response in the humeral and brain metastases, and partial response in the lung metastasis. We think this is, perhaps, the first case of CR in brain metastasis so far. We conclude that interferon can be the first choice drug as an adjuvant chemotherapy against renal cell carcinoma.

Surgically confirmed myelographic classification of congenital intraspinal lipoma in the lumbosacral region.

Twenty-four cases of histologically confirmed congenital intraspinal lipoma of the lumbosacral region were studied by means of myelography with metrizamide. The findings were compared with intraoperative observations. Myelography with metrizamide clearly revealed the detailed intrathecal structures and allowed a classification of intraspinal lipomas into four types, in terms of their insertion into the conus medullaris: dorsal type, either with direct or indirect (via an intrathecal stalk) insertion of the extrathecal lipoma into the dorsal aspect of the conus medullaris; caudal type; combined type; and filar lipoma. Based on our surgical experience in untethering and decompression of the lesions, the classification was found to be useful in designing a safe and effective surgical procedure which minimized all possible trauma to the intrathecal neural structures.

Transplacental induction of myeloschisis associated with hindbrain crowding and other malformations in the central nervous system in Long-Evans rats.

Nine groups of 66 pregnant rats, grouped by gestation days 11 to 19, were subjected to a single, intragastric administration of ethylenethiourea (ETU). Cesarean section was performed on gestation day 20. No dam died following the ETU treatment, although a mortality as high as 21.2% was noted among the fetuses in this group; the remaining live fetuses were found to suffer from a high incidence of myeloschisis associated with hindbrain crowding. Exencephaly and abnormally enlarged head with occipital bossing due to the herniation of the mesencephalic tectum, with and without dilation of the mesencephalic and 4th ventricles, were induced among the fetuses of the dams given ETU at gestation days 12 and 13. Various degrees of hydranencephaly and dysplastic hydrocephalus were found among the fetuses of dams treated by ETU from gestation days 14 to 18. From the histological features of these malformations of the central nervous system (CNS), a possible mechanism in the induction of myeloschisis with hindbrain crowding induced by ETU is discussed, and compared with the previously reported similar malformations induced by trypan blue.

Intracranial epidermoids--concerning the low absorption value on computerized tomography.

The authors analysed the contents of two intracranial epidermoids showing low density on computerized tomography. Although both cases had approximately similar composition, 85, 83% for water, 12, 12.5% for protein, 3, 4.5% for total lipid including cholesterol, respectively, the absorption values differed from each other. Both tumours were identified to communicate with CSF, one by delayed metrizamide CT scan and the other by direct aspiration during operation. The low absorption values in both cases resembled or were slightly higher than that of CSF in each case, and not a single pixel showed negative values for lipid. These findings suggest that it is the CSF content within the anatomical interstices or structural crevice of the tumour, rather than cholesterol, which is responsible for the low absorption values which appeared in the CT scan.

Cerebral gumma. Case report.

A case of focal cerebral syphilitic gumma of the right temporal lobe is reported. Angiography showed moderate focal hypervascularity with stretched vessels, and irregularity of the vessel walls. Plain computerized tomography revealed an area of low density that enhanced strongly after intravenous administration of contrast medium.

[The use of 99mTc-dimercaptosuccinic acid renoscintigraphy in the evaluation of differential renal function].

We studied the total and differential renal function by 99mTc-dimercaptosuccinic acid (DMSA) renoscintigraphy and present a formula to estimate the renal depth for the Japanese and the attenuation coefficient which influenced renal uptake. Total renal uptake of 99mTc-DMSA correlated well with creatinine clearance and with the PSP test, and there was a close correlation between its relative uptake and relative function as determined by 99mTc-diethylenetriaminepentaacetic acid (DTPA) renography. Therefore differential renal function test with 99mTc-DMSA renoscintigraphy was found to have clinical utility. We also demonstrated 99mTc-DMSA renoscintigraphy provided useful morphological information.

[A case of pycnodysostosis].

Pycnodysostosis is a term introduced by Maroteaux and Lamy in 1962 to describe characteristic features such as dwarfism, separated cranial sutures, open fontanelles, hypoplasia and absence of mandibular angle, partial aplasia of the terminal phalanges of the fingers and toes, generalized condensation of bone shadow, bone fragility, and autosomal recessive inheritance. We reported a such case in a 13-years old boy associated with an epidural hematoma. Four weeks after removal of the hematoma, lumbar puncture was performed with the initial pressure of 200mm H2O in an awake state and the plateau waves were observed several times during sleep. Such increased intracranial pressure may have caused the open fontanelle and cranial sutures in addition to dysostosis. Thus such patients require careful attention in order to prevent major bleeding from the superior sagittal sinus directly beneath the separated sagittal suture.

[Effect of etoposide for urogenital tumors].

Ten patients with urogenital tumors were treated with Etoposide. The dose schedule was 100 mg i.v. daily for 5 days and was repeated after 4-5 weeks. One patient with ureteral tumor experienced partial remission for 60 days and minor regression was achieved in 3 patients. The major side effects were leukopenia (60%) and thrombocytopenia (40%). Mean WBC nadir of 1050/mm3 occurred at 13.5 days and recovered quickly after a further 7.5 days.

[Total cystectomy for urinary bladder cancer: clinicopathological study of 95 cases].

Total cystectomy was performed on 95 patients with primary urinary bladder cancer between 1973 and 1983. Histopathological and prognostic studies were reviewed according to the general rules for clinical and pathological studies on bladder cancer. The cancer histological type were transitional cell carcinoma in 87 cases, squamous cell carcinoma in 5 cases, adenocarcinoma in 2 cases, and undifferentiated carcinoma in 1 case. The overall 5-year actuarial survival rate was 36.0%. As for the growth pattern of the bladder cancer, the 5-year survival rates for the patients with papillary non-invasive type (PNT), papillary invasive type (PIT), and non-papillary invasive type (NIT) were 100%, 25.8% and 34.8% respectively. As for the stage, the 5-year survival rates for the patients with pTa, pT1, pT2, pT3a, pT3b, and pT4 were 81.8%, 64.7%, 40.1%, 30.5%, 22.6% and 6.7% respectively. Of 87 patients with transitional cell carcinoma, the 5-year survival rates for the patients with grade 1, grade 2 and grade 3 were 100%, 43.0% and 32.1% respectively. Intramural lymphatic invasion and vascular invasion and intramural histopathological mode of spread were significant indicators of prognosis.

[Nine cases of bladder cancer occurring in occupational dye users].

Workers in the dye manufacturing industry have a high risk of urinary bladder cancer. There may also be a high relative risk of bladder cancer in occupational dye users. Nine occupational dye users were found to have bladder cancer. The period of engaging with dye work ranged from 5 to 40 years. Seven patients had bladder cancer and the other 2 patients had lesions both in the bladder and in the renal pelvis. Histopathology of all cases was transitional cell carcinoma. Three cases were classified into grade 1 and 6 cases into grade 2. One patient died of bladder cancer. The other 8 patients are alive. Four cases recurred after TUR therapy. It is necessary to establish systemic mass screening examination of occupational dye users for the early diagnosis of bladder cancer.

Rathke cleft cysts.

Three cases of Rathke cleft cyst are presented. The clinical symptoms were disturbances of visual acuity and visual fields, and a functional disorder of the hypophysis. In two cases, there were associated meningeal signs. This meningitis was considered to be aseptic and ascribable to leakage of the cyst contents into the subarachnoid space. In one patient, the cyst extended to the suprasellar cistern; a computed tomography scan revealed an enhanced capsule, high-density spotting in a low-density area inside the cyst, and enhanced areas. In another patient, the tumor was localized in the sella turcica; high-density spotting was seen. Based on this finding, we suggest that computed tomography scanning facilitates differentiation from other sellar lesions to some degree. Electron microscopic studies of a cyst with cilia revealed the presence of a coating material around cells with microvilli. On the basis of this finding, Rathke cleft cysts may be considered to be namely, endodermal, inclusions from the upper respiratory tract.

Cavernous hemangioma with bone formation in a child: case report.

A 13-year-old girl suffering from seizures who was found to have hemangioma calcificans in the frontal lobe was surgically treated. The favorable postoperative course of our patient as well as reported cases suggest early excision of the lesion if it is located in an accessible area. Histological examination confirmed ossification with marrow in the lesion. The literature regarding ossification in the brain is discussed briefly.
