[Percutaneous ultrasound-guided ethanol sclerotherapy of autonomous thyroid nodules].

Percutaneous ethanol injection therapy for autonomously functioning thyroid nodules has been performed in 53 patients. 36 patients suffered from hyperthyroidism, and 17 patients had subclinical hyperthyroidism. Ethanol was administered under ultrasonographic guidance in 2-6 sessions depending on the size of the nodule Local neck pain was the most often adverse effect. Transient dysphonia occurred in 3 patients. A subacute granulomatous thyroiditis-like reaction within 1 week after the last session occurred in 4 patients. During a 10-day steroid administration this reaction was stopped. After ethanol sclerotherapy reduction of thyroid nodular volume can be achieved. The reduction of the nodules was between 36 and 75% (mean 55 +/- 15%) of the pre-treatment volume at 6 week after therapy. In 27 of 36 hyperthyroid patients the FT4- and T3-levels became normal. Repeated sclerotherapy was successfull in 6 of the remaining 9 hyperthyroid patients. No relapse of hyperthyroidism was observed. The scintiscan showed a complete cure in 10 of 23 patients one year after PEI-therapy, while in 11 patients partial normalization of the scintiscan was observed. In 2 of 23 patients the scintiscan remained unchanged. Indication of ethanol sclerotherapy is not clear. The method appears an effective alternative procedure in patients with large nodules at high surgical risk. Under special circumstances (pregnancy or iodine-induced hyperthyroidism) ethanol sclerotherapy may be a practical alternative for toxic autonomously functioning thyroid nodules.

Diagnostic value of AgNOR method in thyroid cytopathology: correlation with morphometric measurements.

A silver staining technique was applied to 51 thyroid smears. The numbers of silver-stained nucleolar organizer regions (AgNORs) were counted, and the mean AgNOR and nuclear area per cell were determined with an image analyzer. The mean AgNOR count per cell was significantly higher in malignant than in benign lesions, but there was a considerable overlap. The mean AgNOR area and the mean SD of the AgNOR area per cell were significantly higher in carcinomas than in benign lesions (P = 10-9 and P = 5 x 10-10, respectively) and there were only two and one benign cases, respectively, of overlap. A strong correlation was observed between the mean AgNOR area and the mean nuclear area (r = 0.88), the former being a better discriminator between benign and malignant lesions. The AgNOR technique may contribute to routine thyroid cytopathology.

Study on the glycoprotein nature of chicken IFNs by chemical cleavage and inhibition of glycosylation.

The considerable molecular heterogeneity of chicken IFNs suggested the possible glycoprotein nature of these IFNs. The carbohydrate-specific oxidation and cleavage by neuraminidase indicated that fibroblast IFN may contain sugar moieties. Among the antimetabolites preventing glycosylation, monensin diminished IFN production. However, since it had the same effect on unglycosylated human leukocyte IFN, the drug very probably blocks intracellular transport. It rather proved that chicken IFNs are also secretory proteins. Tunicamycin diminished the formation of leukocyte IFN, but the decrease in titre of this IFN is due to the inhibition of protein synthesis. It is suggested that chicken fibroblast IFN is a glycoprotein, while leukocyte IFN lacks a sugar moiety. The IFN produced on induction with mitogen is closely related to leukocyte IFN, and therefore it seems that the chicken IFN system does not involve the conventional gamma type.

[Anaplastic transformation of papillary carcinoma of the thyroid].

Anaplastic transformation of well-differentiated carcinomas of the thyroid is not common. The authors present a case of a 61-year-old woman, on whom, 17 years earlier, a near-total thyroidectomy was performed with a unilateral selective lymphatic dissection of the neck because of a papillary carcinoma of the thyroid. The operations were followed by a course of external irradiation. For 17 years the patient had no complaints, then she was treated again for recurrence of tumor in the residual thyroid tissue. By biopsy next to papillary carcinoma the authors diagnosed an anaplastic tumor with giant cells, which led to the patient's death 6 months after the operation. Surveying the data in literature they draw attention to the role of external irradiation in anaplastic transformations after the first operation.

Naltrexone potentiates 4-aminopyridine seizures in the rat.

The effects of a pharmacological blockade of the mu opiate receptors on the manifestation of tonic-clonic seizures were investigated in freely moving animals. 4-aminopyridine, a specific blocker of the neuronal K+ channels was used to produce generalized convulsions. After pretreatment of adult rats with 1 mg/kg naltrexone HCl, 3, 5, 7, 9, 14 mg/kg 4-aminopyridine was injected intraperitoneally, and the latencies of the symptoms generated by 4-aminopyridine were measured. Naltrexone HCl decreased these latencies and enhanced the seizures significantly. The experiments provided further evidence for the existence of a tonic anticonvulsant opioid system in the brain.

Changes and correlations of antioxidant enzymes, lipid peroxidation and serum neutral lipids due to haemodialysis treatment in chronic uraemic patients.

A study was made of the changes in the activities of the antioxidant enzymes superoxide dismutase and catalase in the blood of chronic uraemic patients before and after haemodialysis. Changes in serum lipids (primarily neutral lipids), lipoproteins and lipid peroxidation were also followed. Before haemodialysis, antioxidant enzyme activities decreased, while lipid peroxidation increased both in the plasma and in the RBC haemolysates. Quantitative changes in the lipids increased the risk factors. Haemodialysis moderated these risk factors and a tendency to improvement was observed.

Diagnostic accuracy of fine needle aspiration cytology of the thyroid: impact of ultrasonography and ultrasonographically guided aspiration.

OBJECTIVE: To determine the impact of ultrasonography on the diagnostic accuracy of fine needle aspiration cytology (FNAC) of the thyroid. STUDY DESIGN: We compared two FNAC series. In period 1 we used neither ultrasonograph nor ultrasonographically-guided FNAC in the evaluation of thyroid nodules, while both tools were routinely applied in period 2. Moreover, in the former period all cellular follicular lesions were encountered among suspicious findings, while in period 2 we advised regular follow-up examinations instead of immediate surgery for patients with no significant atypia. The basis of follow-up examinations was ultrasonographic volumetry of the nodule and repeat FNAC on growing nodules. RESULTS: The number of FNAC, positive predictive value of FNAC, number of malignancies, sensitivity, the specificity and diagnostic accuracy were higher in period 2 as compared with period 1: 3,446 vs. 1,448; 45% vs. 16%, 39 vs. 29, 92% vs. 76%, 65% vs. 87% and 66% vs. 87%, respectively. CONCLUSION: The introduction of ultrasonography permits higher diagnostic accuracy of the evaluation process. This is achieved not only by lessening false negative reports but, more important, by decreasing false positive results.
