Insulin-like growth factor I-dependent tyrosine kinase activity in stromal cells of human endometrium in vitro.

The study was undertaken to identify and characterize insulin-like growth factor I (IGF-I) receptors in human endometrial stromal cells in culture and to examine whether these receptors are modulated by estradiol (E2) and/or progesterone (P). We found that partially purified plasma membrane proteins from these cells contain specific high-affinity binding sites for IGF-I (10 fmol/micrograms protein). Chemical cross-linking with 125I-labeled IGF-I and autophosphorylation with [32P]ATP-labeled proteins of relative molecular weight 135,000 and 95,000 correspond to the known Mr values of the alpha- and the beta-subunits of IGF-I receptors. Receptor autophosphorylation and phosphorylation of the substrate poly(Glu,Na4Tyr1) was stimulated in vitro by IGF-I (half-maximally at 1 nM, maximally at 100 nM). After stimulation of intact cells with IGF-I (5 nM) and subsequent partial purification of receptors in the presence of phosphatase inhibitors, a 2.5- to 3.6-fold stimulation of the kinase activity toward poly(Glu,Na4Tyr1) was found. Preincubation of the cells for 16 h with E2, P, and E2 + P did not modify the IGF-I binding characteristics nor the effect of IGF-I (5 nM) on tyrosine kinase stimulation in intact cells. This suggests that, in isolated humans, endometrial cell modulation of IGF-I receptor function by estrogen and P does not occur.

[Current possibilities in reproduction medicine. 1. Diagnosis].

The methods employed by assisted conception, presuppose a rational diagnostic evaluation. The endocrine work-up concentrates on demonstrating adequate ovulation. Today, apart from vaginal ultrasonography, this largely means LH determination in the urine in the middle of the cycle, together with progesterone measurements in the luteal phase. Recording the basal body temperature can offer no more than an orientation. The clarification of tubal patency is ideally served by laparoscopy with pertubation. With the aid of new sonographically visible contrast media, tubal flow can also be investigated using sonography. For the male work-up, classical evaluation of the ejaculate has declined in importance. As a means of establishing a fertility prognosis, forward progression, lateral movements of the head of the sperm, and functional sperm analysis (hamster oocyte penetration test) would appear most suitable.

Fallopian Tube Patency Tests↗

[Current possibilities in reproduction medicine. 2. Spectrum of therapeutic procedures].

As measures of assisted conception, GIFT, ZIFT, TV test and insemination require patent Fallopian tubes. In the majority of cases, intra-uterine insemination is attempted before the invasive methods. Intratubal gamete transfer is employed in particular in the case of infertility that has resisted treatment for years, some forms of male subfertility, and in patients with endometriosis or other forms of genital pathology. In addition, ZIFT and the TV test permit an assessment of in vitro fertilization. In the case of IVF, the classical indication of microscopically non-treatable tubal occlusion has been expanded considerably. Although GIFT is associated with a pregnancy rate of between 30 and 40%, IVF is increasingly being used in women in whom GIFT is indicated, thus avoiding the need for laparoscopy. The alternative techniques of transvaginal GIFT or transvaginal embryo transfer cannot yet be definitively evaluated for clinical routine use. In view of the wide range of therapeutic possibilities, individual counselling of the patient is an essential requirement.

[Pregnancy with an extremely rare P blood group with anti-PP1Pk].

The authors report on a successful pregnancy in a young Turkish patient with p and anti-PP1Pk-antibodies. Anti-PP1Pk is considered to cause haemolytic diseases in the newborn and habitual abortions. The patient was treated via plasma exchange and substitution with intravenous immunoglobulin. At 31 weeks of gestation the patient underwent Caesarean section due to growth retardation and pathological signs in the fetal CTG. The child has developed well up to two years after the delivery. Based on our data and a review of the literature, the effectiveness of therapy by plasmapheresis and/or immunoglobulin is discussed.

Cyclic ovarian function in recreational athletes.

In 17 female recreational athletes, ovarian function was monitored using daily hormone measurements and serial ultrasound determinations. Whereas 11 out of 13 women of a control group showed estradiol (E2) maxima beyond 470 pmol/l, progesterone (P4) maxima of 19 nmol/l or more, and a luteal phase length of 9 days or more, only 10 out of 17 athletes satisfied these criteria. Six athletes showed disturbed follicular development, and one athlete showed luteal phase disturbance. Both athletes with disturbed menstrual function (n = 7) and athletes fulfilling the above-mentioned minimal criteria (n = 10) had lower E2 concentrations in all phases of the menstrual cycle (P less than 0.05). P4 concentrations were significantly decreased in the group with disturbed menstrual function (P less than 0.05). Maximal aerobic capacity in the two athlete groups was similar. Neither athlete group showed the expected increase in caloric intake compared with the sedentary controls. It is concluded that recreational running is associated with altered ovarian function. Inadequate nutritional adaptation may be a contributing factor.

[Immunologic assessment in habitual abortion].

Since 1985, after excluding nonimmunological reasons for habitual abortion, we have been performing an MCL, a cross-match, and an HLA typing in these patients. Those with a low MLC response and a negative cross-match are considered to be immunological aborters. Only three out of 42 examined couples showed a low response and in 21 of 24 tested couples the cross-match was negative. Of 36 patients with a high response, 10 had successful pregnancies; seven of them shared more than two HLA antigens. An immunological treatment is indicated in patients with low response and a negative cross-match. High responders should have another pregnancy without stimulation. HLA typing is without any consequences.

Dieting causes menstrual irregularities in normal weight young women through impairment of episodic luteinizing hormone secretion.

Thirteen healthy, normal weight young women were studied throughout a control cycle and a diet cycle, during which they lost 1 kg per week on a vegetarian 800 kcal diet. Blood was sampled daily in the morning, and at weekly intervals, collected at 10-minute intervals for 6 hours. Follicle growth was monitored by ultrasonic measurement. All subjects showed normal cyclic gonadal function during the control cycle. Cyclic gonadal function remained unaltered in two subjects during the diet cycle. No dominant follicle developed in seven others, while another four showed apparently normal follicular development but impaired progesterone secretion by the corpus luteum. Comparison of both cycles revealed that episodic luteinizing hormone (LH) secretion during the follicular phase was altered by dieting. Average LH concentrations and the frequency of episodic secretions were significantly reduced during the follicular phase but not during the luteal phase. Follicle-stimulating hormone was unaltered.

[The value of hormonal and sonographic parameters in the diagnosis of disordered and normal early pregnancy].

In 112 clinical pregnancies after IVF/ET or GIFT, the importance of beta-HCG progesterone and 17 beta-estradiol was evaluated. Additionally, we performed a vaginosonography to confirm an early intrauterine pregnancy. The aim of the study was to define, when a clinical pregnancy can be detected as early as possible and when the differential diagnosis of intact or abnormal pregnancy can be made. By measuring E2 in an HCG substituted cycle, the diagnosis of a clinical pregnancy is possible as soon as day +12 after induction of ovulation. When considering beta-HCG levels alone, the same diagnosis can be made at day +18. Single determinations of HCG level do not offer a satisfactory diagnosis, because of intra-individual variations. A progesterone drop always demonstrates a disturbed early pregnancy. Using a combination of hormonal serial measurements of HCG, E2, progesterone and vaginosonography, the diagnosis of an intact intrauterine pregnancy should be possible as early as day +27 after ovulation induction.

[Prenatal diagnosis of non-viable fetuses caused by developmental disorders--an indication for abortion for fetal reasons?].

Between October 1st 1983 and January 31st 1986 we diagnosed various anomalies on 116 foetuses at the Department of Obstetrics and Gynaecology, Klinikum Grosshadern, Munich. In 103 cases we made a correct diagnosis. This group of patients included 45 non-viable foetuses. Severe anomalies of the foetal central nervous system and lethal dwarfism were diagnosed correctly. On the other hand, antenatal diagnosis of bilateral nonfunctional kidneys and non-correctable cardiac malformations poses problems. We feel that in the case of a definitive prenatal diagnosis of an anomaly for which there is no known medical treatment and which is not compatible with life, the decision as to further procedure should be made at this stage. Before 24 weeks gestation, termination of pregnancy on foetal grounds should be discussed with the parents. Thereafter, preterm labour is preferable to the dangers of obstructed labour at term and should be conducted without foetal monitoring.

[Significance of sonographic diagnosis of fetal abnormalities of the urinary tract].

Out of 5,209 fetuses, routinely screened at our prenatal clinic 56 fetuses with malformations of the urinary tract were diagnosed. In 37 cases dilatation of the urinary tract was found. Five fetuses presented with unilateral multicystic kidneys whereas 3 fetuses suffered from bilateral polycystic kidney disease and 11 were found to have bilateral non-functioning kidneys. Prenatal diagnosis of dilatation of fetal urinary tract and of unilateral multicystic kidneys allows referral of these newborns for further urological evaluation before symptoms are present. Refinement in prenatal diagnosis of bilateral polycystic kidney disease and bilateral non-functioning kidneys helps evaluating fetal viability.

[Course of pregnancy following in vitro fertilization and embryo transfer].

Worldwide analysis of pregnancies after IVF/ET allows the evaluation of the pregnancy outcome compared to normal population. The mean age of IVF-patients is significantly higher. The incidence of clinical abortions after IVF/ET is within normal range, correlating rather with age than with treatment modalities. The rate of multiple gestations is increased after IVF/ET as well as GIFT or hMG-stimulation, leading to more high risk pregnancies. Gestosis is seen in 7% of singletons and 17% of triplet pregnancies. Preterm deliveries before 36 weeks of gestation occur in 17% of singleton and 52% of twin pregnancies after IVF/ET (control 7.3%). This leads to an increasing rate of caesarean section. The global malformation rate is 2% and in a comparable range to normal population. A presumably higher incidence of neural tube defects has to be proven in a larger number of IVF pregnancies.

[Value of endogenous uterine contraction stress test by breast nipple stimulation].

In a prospective study we performed a breast-stimulation-test (BST) in 136 patients. Sufficient contractions could be achieved in 89.7%. A hyperstimulation was seen in 5.8% of the patients. We could not find a false negative BST-result. The duration of the BST was significantly shorter compared to the OBT. The new noninvasive method was well accepted by patients and staff-members, leading to a new management of fetal surveillance in patients beyond 40 weeks of pregnancy.
