Hemodynamic response to ibopamine in acute myocardial infarction.

To determine the effects of ibopamine on hemodynamic parameters, 9 patients with cardiac dilation after acute myocardial infarction were studied. Although there were no significant changes in mean blood pressure, heart rate and rate pressure product after administration of ibopamine, cardiac index and urine volume increased significantly. These data indicate that ibopamine exerts a positive inotropic effect on the myocardium and vasodilating effect on renal vasculature without increasing myocardial oxygen consumption. However, pulmonary capillary wedge pressure increased significantly at 30 min after ibopamine and decreased to the control value at 120 min. A significant increase in heart rate was observed in 3 patients which was followed by chest pain. In addition to increase in cardiac contractile force and decrease in subendocardial perfusion from elevated left ventricular end diastolic pressure, increase in heart rate would decrease diastole time, while increase in oxygen consumption results in aggravation of ischemia especially in patients with multivessel coronary disease. From these findings it is considered that ibopamine should be administered with care to patients with multivessel coronary disease.

Oxygen delivery, oxygen consumption and hemoglobin-oxygen affinity in acute myocardial infarction.

The interrelations of oxygen delivery (DO2), oxygen consumption (VO2) and hemoglobin-oxygen affinity assessed by P50 were investigated in 43 patients with acute myocardial infarction. As DO2 declined due to low cardiac output, a significant decrease in VO2 (r = 0.75, p less than 0.001) and a significant increase in P50 (r = -0.74, p less than 0.001) were observed. In the DO2 range between 300 and 450 ml/min/m2, in which the DO2 of 32 survivors and 11 nonsurvivors overlapped, the P50 of nonsurvivors was significantly higher than that of survivors (31.5 +/- 1.6 vs 27.9 +/- 1.5 torr, p less than 0.001), but there were no significant differences in any other oxygen transport variable. As a result of differences in P50, VO2 in this range was significantly higher in nonsurvivors compared to survivors (169 +/- 17 vs 148 +/- 13 ml/min/m2, p less than 0.001). These data suggest that a normal or increased VO2 alone does not ensure survival in patients with acute myocardial infarction, and increased P50 leads to an increase in VO2. Nevertheless, the interpretation of an increased P50 in patients with acute myocardial infarction must be made with caution, even with adequate DO2 and VO2, because it may imply a precarious oxygen transport/requirement balance in peripheral tissue and, hence, a fatal outcome.

Impact of a family history of hypertension on blood pressure levels during early and late adolescence.

The aim of this study was to examine the impact of a family history of hypertension (FHH) on blood pressure levels in Japanese rural adolescents. The study population consisted of 458 and 553 subjects, aged 13 and 16 years, respectively. Particular attention was focussed on subjects with a FHH (13 years old, n = 38; 16 years old, n = 46) and those without a FHH (13 years old, n = 83; 16 years old, n = 97). Significant associations were found between FHH and systolic blood pressure for boys and girls at 16 years old and diastolic blood pressure for girls at 16 years old. The impact of a FHH on blood pressure mainly affected systolic blood pressure in late adolescence (16 years of age). In early adolescence (13 years of age), different physical maturation status may have masked the real impact of a FHH on blood pressure.

[Molecular forms of plasma atrial natriuretic peptide (ANP) after three kinds of ANP releasing stimulation].

This study analyzed by high performance gel permeation chromatography (HPGC) molecular forms of atrial natriuretic peptide (ANP) in plasma from anesthetized dogs stimulated by balloon inflation at the main pulmonary artery (Group A, n = 4), right atrial pacing (Group B, n = 4) and low-molecular dextran injection to right atrium (Group C, n = 4). Mean pulmonary arterial (PA) pressure, mean right atrial (RA) pressure, mean arterial pressure, heart rate and ECG were monitored by polygraph system Mean PA pressure in Group A, heart rate in Group B and mean RA pressure in Group C increased significantly by the stimulation (p less than 0.01, p less than 0.001 and p less than 0.05 respectively). Other parameters were not changed significantly in the three groups. There were significant increases in ANP concentration after the three stimulations (Group A: 52.4 +/- 5.4 (SD) pg/ml----86.4 +/- 12.2 (SD) pg/ml, Group B: 43.8 +/- 6.7 (SD) pg/ml----72.0 +/- 14.1 (SD) pg/ml, Group C: 42.7 +/- 8.8 (SD) pg/ml----69.3 +/- 10.0 (SD) pg/ml; each p less than 0.01). Gel filtration profiles showed that the increased form of plasma ANP in the three groups was alpha-ANP. These results suggest that alpha-ANP may be a main molecular form in plasma from dogs stimulated by the stretch of pulmonary arterial wall and right atrial wall, and atrial pacing.

Disparate difference in preload reserve between myocardial hypertrophy due to essential hypertension and hypertrophic cardiomyopathy.

To investigate the left ventricular preload reserve in essential hypertensives, the legs-up procedure was performed and the data were compared with those in patients with non-obstructive hypertrophic cardiomyopathy and in normal subjects. Stroke volume index and the following systolic time intervals were measured before and after the legs-up procedure: left ventricular ejection time, pre-ejection period and the ratio of pre-ejection period to left ventricular ejection time (PEP/ET). No changes in blood pressure, heart rate or plasma catecholamine levels were noted before and after the legs-up procedure in all groups. A shortening in the pre-ejection period, a prolongation of ejection time, a decrease in the PEP/ET ratio and an increase in the stroke volume index were observed in essential hypertensives and in normal subjects. However, the changes in these parameters were not observed in patient with hypertrophic cardiomyopathy. The legs-up procedure is known to increase left ventricular end-diastolic volume. These results suggest that myocardial hypertrophy due to essential hypertension preserves the same preload reserve as in normals, but hypertrophic cardiomyopathy does not.

[A case of spontaneous spinal epidural hematoma presenting Brown-Séquard syndrome].

Although there have been a few reports of spontaneous spinal epidural hematoma, it may be difficult to make a correct diagnosis of the lesion because of atypical clinical symptoms. The authors reported a case of spontaneous spinal epidural hematoma presenting Brown-Séquard syndrome, with a review of literature. This 75-year-old male experienced a sudden onset of severe dorsal neck pain during sleep associated with right hemiparesis which deteriorated quickly to hemiplegia, without loss of consciousness, nausea, vomiting and vertigo. The patient has a history of hypertension and has been treated for chronic hepatitis for one year. On admission, neurological examination revealed right hemiplegia with normotensive deep tendon reflexes, and loss of pain and temperature sensations below the level of C5 on his left side. Position sense of fingers and toes was diminished on his right side, and hyperesthesia was recognized at the area of C4 level. Consciousness disturbance, cranial nerve signs, and urinary incontinence were not seen. Right retrograde vertebral angiograms showed no abnormal vascular shadow, even though anterior spinal artery was visible at the level of C8 through C5. Cervical CT scan revealed a left-side dominant extradural high density mass which situated dorsally at around the upper part of C2 body changing it's position to dorsolaterally as it descended to the lower part of C5. On contrast enhancement study, only the margin of the lesion was partially enhanced, which might be the hematoma membrane. On the third day after the onset, laminectomy from C2 to C5 was performed and the epidural hematoma was evacuated.(ABSTRACT TRUNCATED AT 250 WORDS)

Total lipodystrophy with heart failure and renal failure: report of a case.

A 38-year-old female was admitted to our hospital because of dyspnea. The diagnosis of total lipodystrophy was made by following findings: (1) gaunt appearance; (2) insulin-resistant diabetes mellitus; (3) hyperlipidemia; (4) fatty liver. Chest X-ray demonstrated cardiomegaly, pulmonary edema and pleural effusion. Echocardiogram was characterized by left ventricular hypertrophy with asymmetrical septal hypertrophy and left ventricular dysfunction. Renal biopsy revealed focal glomerulosclerosis. We reported a patient with total lipodystrophy combined with heart failure and renal failure, which have been rarely associated with the disease.

Vitiligo with primary hypothyroidism and hypoacusis: a case report.

A 48-year-old female patient with vitiligo developed primary hypothyroidism and hypoacusis. The signs and symptoms of hypothyroidism improved with thyroid hormone replacement therapy but hypoacusis did not. The coexistence of vitiligo and thyroid disease is uncommon and the additional association of hypoacusis is very rare. There may be a common autoimmune pathogenesis for the three disorders. Vitiligo may be a diagnostically useful sign which suggests the presence of multiple autoimmune diseases.

Determination of total dietary fiber in Japanese foods.

Total dietary fiber was determined in Japanese foods by the Prosky-AOAC method. To accomplish the analyses of unsuitable samples, we introduced a few minor modifications to the versions for (i) seaweed and fruits, (ii) cereals, and (iii) fish and meats. These modified methods were used together with the standard method to obtain results with reasonably good relative standard deviation for 231 foods and 21 groups of mixed foods. In this study, dietary fiber was defined so as not to exclude the nondigestible polysaccharide portions of animal foods. A method was proposed which could estimate more accurately the fiber components of animal foods by measuring the "nondigestible protein" of the fiber sample of the fiber sample by the Biuret colorimetric method, instead of the Kjeldahl method, to avoid deducting the values for aminopolysaccharides. In Japanese diets, the amount of fiber obtained from animals foods was less than 5% of the total intake of dietary fiber.
