Mucoceles of ethmoid and sphenoid sinus with visual disturbance.

In the period from 1980 through 1989, we treated 25 (20 postoperative and five primary) cases of mucoceles of the posterior ethmoidal sinus and/or sphenoidal sinus accompanied by visual disturbances. The postoperative mucocele developed 15 to 25 years after initial surgery. Manifestations of mucoceles include headache, ophthalmalgia, double vision, and exophthalmos in addition to a decrease in visual acuity. The degree of improvement in visual acuity after an operation depends on visual acuity before the operation, the mode of development of the mucocele, and the time from onset of the disease until surgery. Opening the mucocele by an endonasal approach using an endoscope is recommended as the radical form of therapy.

Ultrastructural localization of calcium in mechanoreceptors of the oral mucosa.

Cytochemical localization of Ca2+ in Meissner corpuscles and Merkel cell-neurite complexes in the palatine mucosa of the Mongolian gerbil was studied by a combined oxalate antimonate-microwave irradiation procedure. The reaction products obtained were identified as calcium antimonate by EGTA solubility and X-ray microanalysis. Meissner corpuscles in the normal palatine rugae could be roughly classified into three types by amount and localization of Ca2+. Type I corpuscles were characterized by a high Ca2+ content in both the terminal axoplasm and caveolae of the lamellar plates, type II, by a low Ca2+ content in the terminal axoplasm and a high Ca2+ content in the lamellar cytoplasm. Type III corpuscles showed intermediate characteristics. Palatine rugae stimulated mechanically during fixation contained an increased number of type I corpuscles. On the other hand, two patterns were distinguished in the distribution of Ca2+ in Merkel cells in palatine rugae fixed under normal conditions. One showed abundant Ca2+ dispersed throughout the cell, while in the other, Ca2+ was specifically localized in the Golgi apparatus and mitochondria. Similar distribution patterns also were observed in palatine rugae that had received mechanical stimulus during fixation. Axon terminals of most Merkel cell-neurite complexes in normal palatine rugae were poor in axoplasmic Ca2+, whereas those in most Merkel cell-neurite complexes in mechanically stimulated palatine rugae contained abundant Ca2+ in their axoplasm.

Secretion and function of the third component of complement (C3) by murine leukocytes.

Our new finding of de novo synthesis and secretion of C3 by both murine peritoneal macrophages and polymorphonuclear leukocytes (PMN) was confirmed by the incorporation of [35S]methionine into C3 molecules and their complete inhibition by cycloheximide. The methods of secretion of C3 from these two types of cells were compared by examining the C3 contents in their culture supernatants. Completely different modes of secretion were observed, i.e. although macrophages synthesize and secrete C3 constitutively, PMN has a mechanism to store the already synthesized C3 in the cell and secrete it in response to stimuli. Protein kinase C (PKC) activators, e.g. 12-O-tetradecanoyl-phorbol 13-acetate, dioctanoyl glycerol, and mezerein, as well as calcium ionophore A23187 stimulate the secretion of C3 from PMN. These results suggest the involvement of PKC and the calmodulin pathway. A very sensitive method for measuring C3 activity was developed which enabled us to show for the first time that C3 secreted by PMN had opsonizing activity and that particles cultured with PMN were phagocytosed effectively.

Restriction of cicatricial pemphigoid antigens to the lamina densa: confirmation by indirect immunoelectron microscopy.

Circulating anti-basement membrane zone (BMZ) antibodies in a patient with cicatricial pemphigoid (CP) were examined using an indirect immunofluorescence test, indirect immunoperoxidase electron microscopy, and Western blot analysis. An indirect immunofluorescence test on salt-split skin revealed that the anti-BMZ antibodies reacted solely to the dermal side at the separating epidermal-dermal interface, and indirect immunoelectron microscopy on intact skin indicated localization of the corresponding antigens (CP antigens) over the lamina densa and within the lower half of the lamina lucida; there were no CP antigens beneath a melanocyte. Indirect immunoelectron microscopy on salt-split skin demonstrated that the CP antigens were partly dissociated from, but restricted to, the lamina densa. Western blot analysis showed no differences in molecular weight between the CP antigens and bullous pemphigoid (BP) antigens. CP antigens, as detected by this patient's serum, appear to be constituted of molecules quite similar to BP antigens, but with different epitopes. CP antigens may be shed from basal cells and locate in the area of anchoring filaments, where they play a role in connecting basal cells to the underlying lamina densa.

Erythema multiforme: pathomechanism of papular erythema and target lesion.

Skin lesions of erythema multiforme show time-dependent changes from early papular erythema to the late target lesion which consists of a peripheral elevated erythematous area and a central depressed area. We investigated the pathomechanism of erythema multiforme, by examining the papular erythema and target lesion separately. In the early papular erythema, a small number of polymorphonuclear leukocytes and nuclear debris were seen intermingled with mononuclear cells around the slightly swollen blood vessels, on which immunoglobulin and complement components were deposited. Circulating immune complex levels were occasionally elevated. Sera from the patients generated high levels of reactive oxygen species and nitroblue tetrazolium test revealed positive reaction on the infiltrating cells around the blood vessels. These findings suggest that the papular erythema develops via incomplete type III allergic reaction, followed by damage through reactive oxygen species. In the target lesion, the activity of histamine-N-methyltransferase, which is the major histamine-degrading enzyme, was markedly decreased in the peripheral elevated erythematous area and it was recovering in the central clearing area. ICAM-1 and HLA-DR antigens were expressed on the surfaces of the keratinocytes. An increased number of epidermal Langerhans cells and CD4 cell infiltration were observed in the peripheral elevated erythematous area, while a decreased number of epidermal Langerhans cells and CD8 cell infiltration in the central depressed area were observed. These findings suggest that impaired histamine metabolism and cellular allergic reactions play important roles in the development of the target lesion.

[Immunocytochemical and morphometric studies on the development of renin-immunoreactive cells in the rat kidney].

The development of renin-immunoreactive cells was immunocytochemically studied in Wistar-Imamichi rats. The renin-immunoreactivity was localized in the walls of the abdominal aorta, renal artery and arterioles in the kidney. The renin-immunoreactive cells converged with the progress of development from the renal artery via several arterioles to the afferent arteriole in the kidney. In the afferent arteriole, renin-immunoreactive cells appeared only in those of matured glomeruli. The matured glomerulus first appeared at 17 days of gestation followed by the initial appearance of renin-immunoreactive cells at 18 days. The juxtaglomerular index showing the relation between matured glomeruli and renin-immunoreactive cells increased rapidly until 20 days of gestation, shifted thereafter at a level similar to that at 20 days until 7 days after birth, and increased again until 20 days after birth. Although the area of the transverse section at the hilus of the kidney increased all through the period examined, the number of matured glomeruli increased rapidly from 17 days of gestation to 7 days after birth and maintained a fixed level thereafter. This led to the decrease in the number of matured glomeruli per unit area after 7 days after birth. The fine structure of renin-immunoreactive cells showed no difference according to their sites of localization.

New strategies to establish human monoclonal antibodies.

In order to establish human monoclonal antibodies to any sort of antigens efficiently, we have made following two approaches. Our first approach is to improve cell fusion frequency. By improving our previous method for production of human hybridomas, we obtained higher frequency (1/700 vs. 1/5500) compared with our previous method by adding irradiated myeloma cells to culture of fusion cells and modifying the selective medium. Our second approach is to use a SCID-hu mouse for immunization. Since the injection of human PBL can result in the stable long-term reconstitution of a human immune system in SCID mouse, we tried to immune SCID-hu mouse with KLH. In the serum of immunized SCID-hu mouse, we obtained human IgG antibodies to KLH. Additionally, we succeeded in establishing human B lymphoblastoid cell lines which produced antibodies specific to KLH. These methods will open new prospects for the detection and therapy of cancer.

A possible role of activated macrophages in the adoptive immunotherapy using CD4+ T lymphocytes.

Potent anti-tumor T lymphocytes with CD4+8- phenotype were obtained in peritoneal exudate cells by immunizing mice with irradiated tumor cells and OK-432. These effector cells were used in adoptive immunotherapy for tumor-bearing mice. Admixed administration of effector T cells with irradiated relevant tumor cells resulted in a marked enhancement of anti-tumor activity against local tumor and lymph node metastasis compared with the immunotherapy by effectors alone. The activating state of macrophages inoculated with viable tumor cells had much relevance with the implementation of immunotherapy. Innocent bystander lysis was not observed in this immunotherapy. Interleukin-2 given instead of stimulant tumor cells caused no enhancement, while interleukin-1 emerged stronger enhancement than stimulant tumor. In this case, activating state of macrophages had no relevance with the effectiveness of the therapy. These results suggest that macrophages in tumor play a role to secrete interleukin-1 to enhance anti-tumor activity of specific T cells.

The difference in immunological properties between lymph node metastatic and non-metastatic cell lines of MCA-induced fibrosarcoma of C4W mice.

To pursue the process of lymph node metastasis, i.e, the preferential tumor growth in lymph node, we have established the non-metastatic M2B cell line which was derived from 3-methylcholanthrene-induced fibrosarcoma of C4W mouse and a metastatic cell line, M2BLN-M+ which was obtained from metastatic lymph nodes of irradiated C4W mouse which was subcutaneously implanted with cultured tumor cells, because implanted tumor cells were derived from the spontaneous metastatic lymph node of the parental M2B tumor, but regressed in naive C4W mouse. We examined the characteristics of both tumor cell lines in terms of the immunological cellular interactions. M2BLN-M+ showed unexpectedly to be more susceptible to cytotoxicity of immune effectors (NK cell, macrophage and cytotoxic T lymphocyte) than M2B did. When cultured both tumor cells with these effector cells, the growth inhibition of M2BLN-M+ was greater than that of M2B. The regional lymph node of tumor-bearer, however, showed no effective cytotoxic activity as reported by others. On the contrary, when cultured both tumor cells with non-immune lymph node cells, to be surprised, the proliferation of M2B was markedly suppressed, while that of M2BLN-M+ was slightly inhibited. The lymph node cells of M2B-bearing mice showed stronger cytostatic activity to M2B. The results suggest that the cytostatic activity of lymph node cells will be a pivotal factor, concerning the establishment of lymph node metastasis.

The role of complement receptors in tumor cell destruction.

Murine peritoneal macrophages exuding early after stimulation with the activators of the alternative complement pathway are able to destroy some of the tumor cells. This destruction is inhibited by anti-Mac-1 and anti-C3. The tumor cell killing needs longer incubation time (> 15 hr), and anti-Mac-1 affects the reaction even if it were put into the reaction mixture at later time of incubation. The results suggest that complement produced by macrophage is deposited onto target cells, and the complement binding targets interact with macrophages through the complement receptor type 3 (CR3), which leads to the cell destruction. It is known that fully activated macrophages kill the tumor cells with the aid of antibody (ADCC) or lectin, but we show here another route of tumor cell destruction, that is, some sort of incompletely activated macrophages can kill some type of tumor cells in cooperation with endogenous complement and complement receptors (CR3) without participation of antibody.

Ultrastructural localization of Ca(++)-ATPases in Meissner's corpuscle of the Mongolian gerbil.

The cytochemical localization of Ca(++)-ATPases in Meissner's corpuscles of the palatine mucosa of the Mongolian gerbil was studied. Wachstein-Meisel's medium modified by SALAMA et al. (1987) was used for the demonstration of low-affinity Ca(++)-ATPase or Ca++/Mg(++)-ATPase, whereas the medium devised by Körtje et al. (1990) was used for the demonstration of high-affinity Ca(++)-ATPase. Orthovanadate-containing medium, Ca(++)- or/and Mg(++)-free medium, ATP-free medium and preheated sections were used as controls. The plasma membrane of axon terminals consistently showed a strong reaction for high-affinity Ca(++)-ATPase and a capricious reaction for Ca++/Mg(++)-ATPase. In contrast, plasma membranes of the caveolae of lamellar plates showed a strongly positive reaction for Ca++/Mg(++)-ATPase.

A simple and improved method to generate human hybridomas.

By improving our previous method for production of human hybridomas, we developed a simple and remarkably efficient method for production of human hybridomas. We reformed our previous method in the following three points: (1) we added irradiated (30 Gy) myeloma cells as feeder cells to culture of fusion cells; (2) ouabain concentration in the selective medium was reduced from 2 x 10(-6) M to 1 x 10(-6) M; (3) selective medium (GIT-HAT-OL) was added to the culture after overnight cultivation of fusion cells. Consequently, we obtained higher fusion frequency (1/700 vs. 1/5500) compared with our previous method. By our present method, only so small number of human B cells (EBV-LCL) is required, so that the time necessary to establish human hybridomas is reduced.

Active recombinant C3a of human anaphylatoxin produced in Escherichia coli.

DNA sequence coding for the complete human C3a with 77 amino acids was divided into three portions, synthesized separately and constructed for expression in Escherichia coli. High expression of the recombinant C3a was achieved by an expression system using T7 polymerase. Purified recombinant C3a showed the same activities of ileum contraction and platelet aggregation of guinea pig as C3a purified from human serum.

Mechanoreceptors of the hard palate of the Mongolian gerbil include special junctions between epithelia and Meissner lamellar cells: a comparison with other rodents.

As a part of our comparative morphological study of mechanoreceptors in rodent oral mucosae, the hard palate of the mongolian gerbil (a kind of sand rat) was investigated by light and electron microscopy. Subepithelial connective tissue papillae of palatine rugae contained a dense distribution of lamellated corpuscles that showed an ultrastructure similar to that of Meissner corpuscles of primates. The corpuscles in the palate of the mongolian gerbil, however, frequently formed compound large corpuscles with a palisade-like extension. Antemolar rugae predominated in containing such compound "Meissner" corpuscles. Superficial cytoplasmic lamellae of the corpuscles closely adhered to overlying epithelial cells without an intervening basal lamina. Although a small number of simple corpuscles was seen in the first antemolar rugae, no similar corpuscles were encountered in other parts of the palate. On the other hand, numerous Merkel cell-nerve endings were seen in epithelial pegs and sometimes in epithelial ridges of palatine rugae. Quinacrine labelling showed that intermolar rugae contained a rather dense distribution of Merkel cells in comparison with antemolar rugae. The present and previous data indicate that the pattern of palatine mechanoreceptors of the mongolian gerbil is analogous to that of the mouse, but not of the rat.

Antigenic heterogeneity of the reticular meshwork in the white pulp of mouse spleen.

Monoclonal antibodies against cellular components of reticular meshworks were produced by immunizing rats with heterogeneous stromal-cell population of mouse spleen. Immunohistochemical screening selected two antibodies, WP-1 and RPSC-2. WP-1 proved to immunostain the meshwork of the B area densely, leaving the marginal zone unstained; it also reacted sparsely with the meshwork of the T-cell region. In contrast, RPSC-2 selectively immunostained the meshwork of the T region. Immuno-electron microscopy clearly visualized, for both antibodies, reaction products being deposited along the cytomembrane of the fibroblastic reticulum cells, along their abundant cytoplasmic processes that were densely intertwined with lymphocytes. Double immunostaining with RPSC-2 followed by WP-1 clearly divided the white pulp into the T and the B domains. The meshwork in the T-cell region proved to be immunostainable with both WP-1 and RPSC-2. Thus, the fibroblastic reticulum cells of the T- and the B-cell areas, while indistinguishable by routine microscopy, are at least partially heterogeneous.

US and CT evaluation of cervical lymph node metastasis from oral cancer.

A comparison was made of clinical diagnosis by ultrasound (US) and CT examination in 52 patients who had undergone radical neck dissection. The site, size, depth and other patterns concerning the nodes which could not be detected by physical examination was examined. US and CT examination proved superior to conventional clinical examination in detecting metastatic nodes. US and CT revealed that the disease stage in nine necks had increased from N1 to N2b and in seven of 11 necks from N0 to N positive. The characteristic features of US and CT diagnosis of metastatic lymph nodes were elucidated. Specifically, the diagnosis of cervical metastatic nodes by a combination of US and CT was found to be the most reliable method for detection of subclinical lymph nodes.

Oral contraceptive-induced lupus erythematosus in a Japanese woman.

A case of oral contraceptive-induced lupus erythematosus (LE) was reported. Erythematous skin lesions were noticed on hypothenor sites of palms and acral sites of pedes. Abnormal laboratory findings included an elevated ESR, CRP and weakly positive anti-nuclear antibodies. Skin biopsy specimens from involved skin showed Clq deposits at the dermo-epidermal junction. LE-like symptoms were considered to be induced by oral contraceptives, because her symptoms disappeared after the oral contraceptives were discontinued.
