Ebstein's anomaly with non-patent ductus arteriosus aneurysm and bradycardiac atrial fibrillation in an 80-year-old man.

An 80-year-old man with Ebstein's anomaly and ductus arteriosus aneurysm is reported. He was admitted with bradycardiac atrial fibrillation and right ventricular failure. For the control of brady-arrhythmia, a permanent pacemaker was implanted. Two-dimensional echocardiogram revealed distal displacement of the septal tricuspid valve. Aortography and computed tomography showed ductus arteriosus aneurysm. This is the first report of the association of Ebstein's anomaly and non-patent ductus arteriosus aneurysm.

A hypothesis of epiarachnoidal growth of vestibular schwannoma at the cerebello-pontine angle: surgical importance.

AIMS: The purpose of this study is to clarify the rearrangement of the arachnoid membrane on the vestibular schwannoma during its growth in relation to adjacent neurovascular structures for a better understanding of dissecting plane of arachnoid during surgery. METHODS: Arachnoid membrane over the tumour was investigated during surgery with suboccipital transmeatal approach in twenty-six tumours. All microsurgical procedures were recorded with a video and reviewed. The tumour growth was classified into five stages depending upon the tumour diameter in the cerebello-pontine (CP) angle: Stage 1; purely intracanalicular (2 cases), Stage 2; less than 5 mm (2 cases), Stage 3; > or = 5 and <15 mm (8 cases), Stage 4; > or = 15 and <25 mm (9 cases) and Stage 5; > or = 25 mm (5 cases). Rearrangement of the arachnoid on the tumour was conceptualised throughout all stages. RESULTS: All tumours of Stage 1 and 2 were entirely located in the subarachnoid space of the cerebello-pontine cistern without arachnoidal rearrangement, while all tumours of Stages 3 to 5 were enveloped, in the CP angle, with invaginated arachnoid membrane consisting of cerebello-pontine cistern except two surfaces; the medial pole and the tumour surface under the facial and cochlear nerves near the porus. CONCLUSION: The tumour originates subarachnoidally within the internal auditory meatus (IAM) and grows epiarachnoidally in the CP angle. Rearrangement of the arachnoid begins with its adhesion on the medial pole of the tumour along the porus, resulting in the arachnoidal invagination into the cerebello-pontine cistern with further growing of the tumour.

Effect of the alpha-adrenoceptor antagonist urapidil on the pacemaker activity of rabbit sino-atrial node preparations.

We studied the effects of the alpha-adrenoceptor antagonist urapidil on the sino-atrial node pacemaker activity of the rabbit. Urapidil (above 10 microM) decreased the spontaneous firing frequency and the maximum rate of rise of the action potential, and prolonged the action potential duration at half-amplitude. Above 100 microM, urapidil also reduced the rate of the diastolic depolarization and the action potential amplitude. On the current systems, urapidil depressed the slow inward current and the time-dependent potassium outward current. The hyperpolarization-activated inward current was also reduced with only a high concentration (300 microM). Furthermore, the agent decreased the current oscillations induced by elevating [Ca]o. It is concluded that urapidil depresses the sinoatrial node pacemaker activity, mainly due to a reduction of Ca2+ influx through the cell membrane and it also eliminates the excessive intracellular Ca2+ load of nodal cells.

Growth characteristics of murine B16 melanoma multicellular spheroids: a model for invasion and effects of doxorubicin treatments.

Multicellular spheroids, derived from murine B16 melanoma cells, showed unique growth characteristics: when they reached about 500 microns in diameter, their morphology changed rapidly and they became amoeba-like irregular-shaped aggregates. This morphological characteristic closely resembled that of invasive cancer, and may serve as a model for local invasion. To test the possibility that the changes mentioned above can be inhibited by a drug, spheroids were treated with 0.8 microgram/ml of doxorubicin for one hour and their morphology was observed temporally. Although this concentration of the drug decreased the survival of the melanoma cells in monolayer to about 10(-3), the growth was not delayed nor were the "invasion"-like changes inhibited in the spheroids. We believe this system of multicellular spheroids is a useful model to study the mechanisms of tumour invasion, although doxorubicin could not inhibit "invasion"-like changes in this system.

Ki-1-positive large cell anaplastic lymphoma diagnosed by urinary cytology. A case report.

Cytomorphologic and immunocytochemical findings in urinary cytology in a case of Ki-1-positive large cell anaplastic lymphoma in the left cervical region in a 42-year-old man are reported. Smears stained with Papanicolaou or Wright-Giemsa stain showed a cytomorphologic spectrum that varied from immunoblastlike cells to large neoplastic cells with pleomorphic nuclei and vacuolated cytoplasm. Neoplastic cells with multilobulated, bizarre nuclei and mitosis were also observed. Immunocytochemistry on urinary sediments showed that the tumor cells were of a "null" phenotype and strongly expressed the Ki-1 antigen and "activation" markers of HLA-DR, Tac (CD25) and OKT9 (CD71). Histology and immunohistochemistry confirmed these findings.
