Viability and versatility of arterialized venous perfusion flaps and prefabricated flaps: an experimental study in rabbits.

The purpose of this study was to investigate the viability of two types of unconventional flaps: 1) the arterialized venous perfusion (AVP) flap; and 2) the prefabricated flap. Four experimental groups were studied: an AVP flap group with assessment of the viability of single and paired flaps nourished by the same vascular pedicle; a prefabricated flap group with the abdominal flap pedicled on the epigastric artery and vein; a prefabricated flap group in which the flap was supplied by an arterialized vein graft (A-V shunt), and paired flaps of different designs, but based on the same vascular pedicle, were investigated; and a free composite graft group. Survival of the skin flaps exceeded 92 percent in each group, except in the free composite group which showed complete necrosis. Results of the study validated that flap viability was independent of flap size (large or small), type (AVP flap or prefabricated flap), and the number of flaps on each vascular pedicle (single or paired).

Congenital alveolar adhesions.

We report an infant girl with congenital alveolar adhesions and a cleft palate. The mucosal bands were resected the day after birth. Stretching exercises of the mandible improved the range of movement at the temporomandibular joint. Two weeks of therapy were required before full mouth opening was possible. Previously reported patients and theories of embryogenesis are reviewed.

Upper extremity aneurysm in infants.

We report a 2-month-old female infant with a false aneurysm of the brachial artery and an 8-month-old male infant with a true aneurysm of a common digital artery. These traumatic aneurysms in the upper extremity are extremely rare in infants. In the former infant, reanastomosis of the brachial artery was performed. In the latter infant, simple excision was successful.

Binder's syndrome: peculiarities in Japanese patients.

We have seen 4 Japanese patients with Binder's syndrome or maxillonasal dysostosis. The diagnosis of Binder's syndrome was very difficult, based on facial appearances alone. Although the flattened nasal tip was quite unusual in Japanese individuals, the midface retrusion and flattened nasal bridge would be considered within normal limits. Consequently, the diagnosis of Binder's syndrome is only rarely made in Japanese persons. The peculiarities of this syndrome in Japanese patients are discussed and our surgical methods are shown.

[Delayed midsternal wound reconstruction for infants without secondary sternal closure].

Three infants, aged 11 days, 19 days and 48 days, underwent two Jatene operations and one modified Norwood operation. The sternum was left open and the skin defect was covered with a silicon sheet in all three patients. Delayed sternal closure was impossible because of hemodynamic deterioration in all three patients. Consequently delayed midsternal wound reconstruction was applied. One rectus abdominis muscle flap was turned up and the defect between the split sternum was filled with this muscle flap. Bilateral axillary incision was made to decrease the skin tension and the midsternal wound was closed with cutaneous advancement flaps. Bilateral axillary defects were covered with mesh skin implantation. All three patients recovered after this procedure. We propose this technique for the cases in which the delayed sternal closure is impossible.

Heterotopic gastrointestinal cyst of the oral cavity.

We report a heterotopic gastrointestinal cyst of the oral cavity. Heterotopic islands of gastrointestinal mucosa have been found in the esophagus, the small bowel, and particularly in Meckel's diverticulum. However, their occurrence in the head and neck is extremely rare. Previously reported cases and the theories of embryogenesis are reviewed.

Treatment of severe microstomia caused by swallowing of caustic soda.

Treatment of severe microstomia caused by swallowing of caustic soda is presented. The oral cavity was severely constricted because of mucosal adhesions. We used a free forearm flap for reconstruction of the oral cavity and vermilion flaps at the oral commissure, with satisfactory results. A technique is presented, and the problems with respect to the reconstruction of the oral cavity are discussed.

First branchial cleft sinus in hemifacial microsomia.

In this paper we present a case of first branchial cleft sinus in hemifacial microsomia. An external opening was placed about 2 cm infero-posterior to the microtic ear remnant. The literature is reviewed and discussed.

The question mark ear (congenital auricular cleft): a familial case.

We describe here 2 patients with question mark ears in one family. They had the peculiar auricular deformities bilaterally. Both patients had a cleft between the helix and earlobe respectively. The upper portion of the auricle showed marked prominence. The mother also showed notching between the helix and earlobe. The literature is reviewed and discussed.

Cleft palate in the Beckwith-Wiedemann syndrome.

Patients with the Beckwith-Wiedemann syndrome have numerous anomalies, varying somewhat from case to case. The most common presentation is exomphalos, macroglossia, and somatic gigantism. Although cleft palate in association with this syndrome has been rarely reported, we have observed 6 patients with cleft palate in 10 patients who were diagnosed with the Beckwith-Wiedemann syndrome. The literature is reviewed and discussed.

Osteogenic capacity of vascularised periosteum: experimental study using rib periosteum in rabbits.

The osteogenic capacity of vascularised periosteum was investigated in rabbits using island rib periosteum nourished by intercostal vessels as an experimental model. Twenty-three adult white female rabbits were used. Excellent bone formation was observed after 2 postoperative weeks in each case. In the early stages of bone formation, extensive chondral ossification was observed. Newly formed bone became mature after 4 to 8 postoperative weeks, on histological examination.

The sequential evaluation of bone scintigraphy: an analysis of revascularised bone grafts.

Technetium-99m Methylene Diphosphonate (Tc-99m MDP) has often been used to assess revascularisation of bone, because positive uptake of Tc-99m MDP reflects patent anastomoses and viability of the grafted bone. However, the sequential evaluation of bone scintigraphy has seldom been reported, especially in clinical cases. We carried out sequential evaluation in 17 patients with revascularised bone grafts. In this series, intense or maximal uptake in the grafted bone was observed in the early days after surgery (1-2 weeks postoperatively), with considerable reduction after 4 weeks. Furthermore, it was found that there was no uptake of Tc-99m MDP when the anastomosed vessels were occluded, even after 6 weeks, and these three cases all developed fracture or sequestrum. We therefore believe that bone scintigraphy is reliable in the postoperative monitoring of revascularised bone for at least 4 or 6 weeks postoperatively.

Experimental study of vascularized bone: quantitative analysis of bone scintigraphy and histology.

The purpose of this study was the sequential evaluation of vascularized bone by quantitative analysis of bone scintigraphy with Tc-99m methylene diphosphonate (MDP). The correlation between the uptake of Tc-99m MDP and the morphologic behavior of vascularized bone was investigated, simultaneously with bone labeling, contact microradiogram (CMR), and histology of an undecalcified sample. In this experiment, 41 adult female rabbits were used. An island tibia pedicled with the popliteal vessels was used as the model of vascularized bone. The maximum uptake of Tc-99m MDP was observed at two to four postoperative weeks. When bone marrow was partially exposed, the uptake was significantly large. On the basis of this experimental study, the degree of Tc-99m MDP uptake seemed to depend primarily on the osteoblastic activity of vascularized bone. The newly-formed bone was observed mainly at periosteal sites.

Hereditary occurrence of lateral cervical fistulas.

Three cases of hereditary lateral cervical fistulas are reported. The patients had lateral cervical fistulas bilaterally and different types of auricular malformations. The mothers of the patients also had lateral cervical fistulas. Moreover, one mother had bilateral microtias and the other mothers bilateral preauricular fistulas. The literature is reviewed and discussed.

Branchial cyst in the Beckwith-Wiedemann syndrome.

Patients with the Beckwith-Wiedemann syndrome have numerous anomalies, varying somewhat from case to case. A 2-year-old boy with the Beckwith-Wiedemann syndrome who had a branchial cyst is reported. The literature is reviewed and discussed.

Oral and pharyngeal reconstruction using the free forearm flap.

Recent progress in myocutaneous and free flaps has facilitated the primary closure of various complex defects resulting from ablative surgery of head and neck cancers. The musculocutaneous flap (such as the pectoralis major flap or the latissimus dorsi flap) has in particular proved its greater versatility. The forearm free flap for intraoral defects can provide thin and pliable skin and has great potential in the reconstruction of oropharyngeal defects. Between November 1982 and August 1985, we achieved 74 free forearm flap transfers for patients with head and neck cancers. Herein we present several clinical applications of the free forearm flap and discuss its advantages and disadvantages, especially hand morbidity after flap elevation.

The development of bone after perichondrial grafting: an experimental study using ear and rib perichondrium in rabbits.

The development of bone after perichondrial grafting was investigated using rabbit ear and rib perichondrium. Sixty-four white adult female rabbits were used. Both free and vascularised perichondrial grafts were undertaken. In each case the chondrogenic potential of perichondrium was proved. Furthermore, when the perichondrium was vascularised or grafted in recipient sites having good blood circulation, the development of large areas of bone was observed around the regenerated cartilage.

Accordion flap.

The accordion flap is proposed as a means of covering a rhombic defect. A prominent feature of the method presented is in the preparation of a flap on one side of the skin defect, resulting in little dog ear.
