[Mono-radiculopathy multiplex--multiple infarction of the cauda equina caused by intravascular lymphomatosis].

A 55-year-old man had felt numbness of the bilateral peroneal sides of legs for 6 months. Then hepatosplenomegaly, anemia, body weight loss and fever developed, and a diagnosis of malignant histiocytosis (MH) was made by revealing the presence of innumerable atypical histiocytes with hemophagocytosis in the bone marrow. Soon later, sensory disturbance of bilateral peroneal sides of legs (right side dominant) developed and aggravated with painful dysesthesia and weakness of the legs for the last 2 weeks before death. Electrophysiologically, sensory conduction velocity of the sural nerve was normal and somatosensory evoked potentials from tibial nerve were normal before P15 but were not evoked at all after the lumbar potential, suggesting lumbosacral radiculopathy. Autopsy showed multifocal ischemic lesions and secondary degeneration of the lumbosacral nerve roots associated with necrosis and fibrosis of the radicular vessels and intravascular infiltration of atypical mononuclear cells which were positive for B cell markers. The neurological manifestations and the distribution of ischemic lesions, which were similar to those of vasculopathic mononeuropathy multiplex, would deserve the name of "monoradiculopathy multiplex".

[A study of trace elements in deciduous teeth. The differences of Cd, Zn, Pb, and levels in prenatally and postnatally formed deciduous teeth].

It has been reported by some investigators that deciduous teeth could be useful materials for the analysis of the trace elements available to the human body burden. In this study, an attempt was made to define that uptake of trace elements (Cd, Zn, Pb and Cu) take place during formation of the deciduous tooth. Samples were prepared from exfoliated human upper deciduous central incisors divided into two or three sections at incremental lines includes neonatal line. Samples were dissolved with nitric acid and dried at 80 degrees C. Each element was separated chromatographically using acetone-HCl gradient solution with a cation exchange resin and then analyzed by flameless atomic absorption spectrophotometry. The results were as follows: 1. Ca, P, Ca/P ratio levels showed no significant differences between prenatally and postnatally formed teeth. 2. In the enamel, (1) Cd level showed a tendency to be higher in the postnatally formed enamel, but not significant. (2) Zn and Pb levels were found to be significantly higher in the postnatally formed than in the prenatally formed enamel. (3) Cu level was reversed significantly. 3. In the dentin, (1) Cd and Cu levels in the postnatally formed dentin I were significantly less than prenatally formed denin, and these were almost equal to the postnatally formed dentin II. (2) Zn level in the postnatally formed dentin I was almost equal to the prenatally formed dentin, but that was significantly less than the postnatally formed dentin II. (3) Pb level in the postnatally formed dentin I was significantly less than the prenatally formed dentin and the postnatally formed dentin II. Therefore, the results suggest that the accumulation of Cd and Cu to the deciduous tooth mainly occur in the prenatal and the accumulation of Zn and Pb to the deciduous tooth occur not only in the prenatal but also continuously in the postnatal, and the deciduous tooth can be a useful materials for environmental contamination recorder.

[A study of development and lesion of dental hard tissue. Quantitative analysis of trace elements (Cd, Zn, Pb and Cu) in different geological areas of Japan].

Trace elements (Cd, Zn, Pb and Cu) levels were determined in lower deciduous incisors which were collected from four areas (Sapporo, Yokosuka, Nagano and Kitakyushu) in Japan. Samples were divided into enamel and dentin, and dissolved with nitric and dried at 80 degrees C. Each element was separated chromatographically using an acetone-HCL gradient solution with a cation exchange resin and then analyzed by atomic absorption spectrophotometry. The results were as follows: 1. Ca, P, Ca/P ratio levels showed no significant differences among four areas. 2. In the enamel, (1) Cd levels of each area were 0.593 micrograms/g (Yokosuka) greater than 0.315 micrograms/g (Sapporo) greater than 0.261 micrograms/g (Nagano) greater than 0.222 micrograms/g (Kitakyushu). (2) Zn levels of each area were 174.912 micrograms/g (Nagano) greater than 157.473 micrograms/g (Yokosuka) greater than 150.971 micrograms/g (Kitakyushu) greater than 128.980 micrograms/g (Sapporo). (3) Pb levels of each area were 8.412 micrograms/g (Kitakyushu) greater than 7.925 micrograms/g (Yokosuka) greater than 5.426 micrograms/g (Sapporo) greater than 4.285 micrograms/g (Nagano). (4) Cu levels of each area were 9.276 micrograms/g (Kitakyushu) greater than 6.928 micrograms/g (Nagano) greater than 6.178 micrograms/g (Yokosuka) greater than 4.389 micrograms/g (Sapporo). 3. In the dentin, (1) Cd levels of each area were 0.214 micrograms/g (Sapporo) greater than 0.184 micrograms/g (Kitakyushu) greater than 0.145 micrograms/g (Nagano) greater than 0.119 micrograms/g (Yokosuka). (2) Zn levels of each area were 124.152 micrograms/g (Yokosuka) greater than 112.242 micrograms/g (Nagano) greater than 111.952 micrograms/g (Sapporo) greater than 111.456 micrograms/g (Kitakyushu). (3) Pb levels of each area were 3.584 micrograms/g (Kitakyushu) greater than 3.019 micrograms/g (Yokosuka) greater than 1.819 micrograms/g (Sapporo) greater than 1.523 micrograms/g (Nagano). (4) Cu levels of each area were 3.734 micrograms/g (Kitakyushu) greater than 2.221 micrograms/g (Nagano) greater than 2.098 micrograms/g (Yokosuka) greater than 1.664 micrograms/g (Sapporo). Therefore, the results suggest that the trace elements levels in deciduous teeth depended on the environmental condition of each area and the deciduous teeth could be a useful material as an environmental contamination indicator.

[Two cases of advanced gastric cancer with diffuse carcinomatosis of the bone marrow showing a widespread osteoplastic bone metastasis].

Reported are 2 cases of an advanced gastric cancer with a "diffuse carcinomatosis of the bone marrow" (DCMB) showing a widespread osteoplastic bone metastasis. In a DCBM, a widespread invasion of cancer cells to the bone marrow occurs, causing leukoerythroblastosis, anemia, and disseminated intravascular coagulation. The local reaction in the bone, namely being osteoplastic or osteolytic, has not been discussed in previous papers. Our cases were characterized by an extremely high serum alkaline phosphatase, a "superscan" view of the bone scintigram, and a diffuse osteoplastic bone metastasis with an osteoid increase. Thus we think there are DCBM subtypes with a diffuse osteoplastic bone metastasis.

Establishment and characterization of a unique human cell line that proliferates dependently on GM-CSF, IL-3, or erythropoietin.

We have established a novel cell line, designated as TF-1, from a patient with erythroleukemia, which showed complete growth dependency on granulocyte-macrophage colony-stimulating factor (GM-CSF) or on interleukin-3 (IL-3) and carried a homogeneous chromosomal abnormality (54X). Erythropoietin (EPO) also sustained the short-term growth of TF-1, but did not induce erythroid differentiation. These three hematopoietic growth factors acted on TF-1 synergistically. Transforming growth factor-beta and interferons inhibited the factor-dependent growth of TF-1 cells in a dose-dependent fashion, and monocyte-colony stimulating factor and interkeukin-1 enhanced the GM-CSF-dependent growth of TF-1. Ultrastructural studies revealed some very immature features in this cell line. Although TF-1 cells do not express glycophorin A or carbonyl anhydrase I, the morphological and cytochemical features, and the constitutive expression of globin genes, indicate the commitment of TF-1 to erythroid lineage. When induced to differentiate, TF-1 entered two different pathways. Specifically, hemin and delta-aminolevulinic acid induced hemoglobin synthesis, whereas TPA induced dramatic differentiation of TF-1 into macrophage-like cells. In summary, TF-1 is a cell line of immature erythroid origin that requires GM-CSF, IL-3, or EPO for its growth and that has the ability to undergo differentiation into either more mature erythroid cells or into macrophage-like cells. TF-1 is a useful tool for analyzing the human receptors for IL-3, GM-CSF, and EPO or the signal transduction of these hemopoietic growth factors.

Cyclosporine G and D in experimental autoimmune uveoretinitis in the rat.

Two analogs of cyclosporine A (CsA), cyclosporine G (CsG) and cyclosporine D (CsD), were compared to CsA with regards to their effects on experimental autoimmune uveoretinitis (EAU) as well as their adverse effects on renal functions. When administered daily on days 0-14 after immunization with S-antigen, CsA was the most effective of all in inhibiting EAU followed by CsG: 20-30 mg/kg/day of CsG appeared to have the same effect as 5 mg/kg/day of CsA. The effect of CsD was the least. When administered from day 7 after immunization, CsA and CsG were also effective in inhibiting the development of EAU. As for the adverse side effects, CsA was the most nephrotoxic: the toxic changes were morphologically found with doses of CsA at 10 mg/kg/day or higher. CsG and CsD were not nephrotoxic even at 30 mg/kg/day. The effects of CsG on immune responses were very similar to those of CsA. Both agents exhibited selective inhibition on the cell-mediated immune responses to S-antigen, while having no effect on antibody production.

An autopsy case of myelodysplastic syndrome (MDS): diagnostic problems between MDS and the other haematopoietic disorders including acute myelofibrosis.

We report an autopsy case of myelodysplastic syndrome (MDS) in a 35-year-old male, who presented with pancytopenia and bleedings. Bone marrow specimens disclosed myelofibrosis and hypercellular marrow with more than 60% atypical erythroblasts in the bone marrow cells. Type I or type II blasts were less than 10% of the peripheral blood and bone marrow cells during the clinical course. At autopsy, infiltration by myeloid and erythroid cells and megakaryocytes was noted in the liver, spleen and lymph nodes. According to the FAB classification, this case might be classified into refractory anemia with excess of blasts (RAEB) or RAEB in transformation. However, the remarkable neoplastic proliferation of three haematopoietic cell lines also indicates acute myeloproliferative disorder such as acute myelofibrosis or acute panmyelosis.

Establishment of a human megakaryoblastic cell line (T-33) from chronic myelogenous leukemia in megakaryoblastic crisis.

A megakaryoblastic cell line, termed T-33, was established from the peripheral blood of a patient with Philadelphia chromosome-positive chronic myelogenous leukemia in megakaryoblastic crisis. T-33 cells have been maintained in RPMI 1640 medium containing 10% fetal calf serum in a single cell suspension with a doubling time of 24-36 h for over 2 years. Giemsa-banded karyotypes were female hyperdiploid with a modal chromosomal number of 51, all cells including Philadelphia chromosome. The cells showed strong positivity for periodic acid-Schiff and alpha-naphthyl acetate esterase, and weak for alpha-naphthyl butyrate esterase, but were negative for myeloperoxidase. Flow cytometric analysis of cell surface markers showed the existence of HLA-DR, MY-7, MY-9, and a platelet antigen (Yukb), and no markers for T- or B-lymphocytes. Most of the cells fixed with acetone were positive for Factor VIII, platelet glycoprotein IIb-IIIa, IIIa (Yukb), and Ib, but negative for glycophorin A and hemoglobin. Ultrastructural platelet peroxidase was demonstrated in 2-3% of cells and the percentage of positive cells increased up to 20% after the treatment with 12-O-tetradecanoylphorbol-13-acetate. The cells contained small dense granules negative for platelet peroxidase, their number increasing threefold after 12-O-tetradecanoylphorbol-13-acetate treatment. Such treated cells frequently showed a complex of the demarcation membrane in the cytoplasm. T-33 responded thrombin to exhibit calcium influx. This cell line may be useful for the study of the early stage of megakaryocytic differentiation in human megakaryopoiesis.

Influence of major histocompatibility complex H-2LD class I molecules on spleen colony-forming units.

To test whether the major histocompatibility complex class I genes are involved in the regulation of hemopoiesis, the stem cell activities of BALB/c-H-2dm2 (Dm2) mice, which are defective in the expression of H-2L antigens, have been compared with those of the wild-type, BALB/c-Kh, in in vivo and in vitro stem cell assays. In spleen colony-forming unit assays, Dm2 as hosts consistently supported a smaller number of colonies than did BALB/c-Kh. However, both Dm2 and BALB/c-Kh supported a comparable number of colonies in in vitro granulocyte-macrophage colony-forming unit and erythroid colony-forming unit assays. These observations together suggest that the mutation in Dm2 has not affected the hemopoietic potential of the stem cells but may probably affect the hemopoietic microenvironment for the development of the stem cells.

Histopathologic studies on myeloproliferative sarcoma virus (MPSV) induced leukemias and hemangiosarcoma in Jar-2 rats.

It is well known that MPSV induces myeloproliferative syndrome (MPS) in mice. Intravenous one shot inoculation of myeloproliferative sarcoma virus (MPSV) with Friend murine leukemia virus (F-MuLV) as a helper in newborn Jar-2 rats (on the second neonatal day) yielded hematopoietic malignancies in all the treated rats (25/25 rats) after 2 weeks' latency. MPS appeared from the 14th day in 14 rats. In the midst of the myeloproliferative field of the spleen and bone marrow, myeloblastic or myeloblastic-erythroblastic foci were observed. From 19th day, acute myeloblastic leukemia occurred in 3 rats and erythroleukemia in 8 rats. MPSV induced first MPS which remained as such or later developed into acute leukemia. Myelofibrosis as seen in mice was not observed. In addition, hemangiosarcoma of the brain, spinal cord and spleen appeared in 15 rats from the 24th day, and were often multiple. MPSV can yield the tumor only in newborn rats, and target cells of MPSV are not only hematopoietic cells but also endothelial cells of the brain, spinal cord and occasionally spleen.

Systemic hemangiomatosis.

An autopsy case of unusual vasoformative tumor is presented. The distribution of this tumor was restricted to the organ of the hematopoietic system such as the spleen, liver, and whole bone marrow. Although it may be considered as a variant of certain splenic hemangiosarcoma because of widespread dissemination, both the benign-looking histological features and specific distribution of this tumor suggest its hamartomatous nature. Several additional similar cases in the literatures are briefly reviewed from the viewpoint of systemic hemangiomatosis.

Endotoxemia-induced diffuse myelitis and extensive patchy necrosis of the liver.

An autopsy case with endotoxemia-induced diffuse myelitis and extensive, grossly patchy necrosis of the liver occurring in a 70-year-old female was examined histopathologically and electron microscopically. Leucopenia with prominent leukemoid reaction (myeloblasts 20%) preceded the terminal fulminant hepatitis by two weeks. Soon after the terminal event, bacteremia and endotoxemia were detected and negativity for HB antigen was proved. Diffuse myelitis was characterized by devastation of hyperplastic bone marrow structure mottled with destructed sinus architecture and scattered exudative necrosis, resulting in the loss of mature granulocytes and erythropoiesis. Regenerative clusters of myeloblasts and prominent increase of megakaryocytes were observed. Electron microscopically, the bone marrow contained fibrin and platelets within the exudate of the marrow stroma. Extensive, grossly patchy necrosis of the liver microscopically consisted of well demarcated coagulation necrosis of hepatic parenchyma with scattered fibrin thrombi in the sinusoids at the boundary. There were no definite thrombi but occasional fibrin accumulation in the small blood vessels of the liver. Both extensive diffuse myelitis and extensive, patchy necrosis of the liver seemed to be quite rare in incidence. The pathogenesis of these combined lesions was discussed in relation with endotoxemia.

Histopathological identification of hepatic intramitochondrial crystalloid inclusions (IMCI).

A 45-year-old female with systemic lupus erythematosus underwent liver biopsies twice during the period of cefazolin administration and a complication of cefazolin-induced toxic hepatitis was strongly suggested. Histologically, a small number of spindle-shaped, esoinophilic crystalloid bodies were faintly noticed in the cytoplasm of hepatocytes in hematoxylin and eosin-stained sections, while they showed strong positivity for strains inherent to mitochondria as well as specific to phospholipid and sharply showed a spindle or needle shaped configuration. Moreover, they greatly increased in number in these sections. They were distributed throughout the the parenchyma but were prominent in the degenerated hepatocytes with slight fatty metamorphosis. Electron microscopically, they proved to correspond to the intramitochondrial crystalloid inclusions (IMCI). Despite many ultrastructural descriptions of the IMCI, precise histopathological examination of the IMCI referring to the staining property on the deparaffinated sections has not yet been described since the first report of the IMCI by Napolitano (1958).
