Adenocarcinoma mimicking primary gastric lymphoma demonstrated by I-123 IMP scintigraphy.

A poorly differentiated adenocarcinoma of the stomach showed an unusual type of diffuse wall thickening of the entire stomach, similar to that seen in malignant lymphoma. The pathologic specimen revealed a "lymphoepithelioma"-like pattern of undifferentiated carcinoma with intense lymphocyte and plasma cell infiltration. I-123 IMP scintigraphy incidentally demonstrated increased uptake in the entire stomach on early imaging and moderate washout on delayed studies. SPECT images demonstrated diffusely increased accumulation in the entire gastric wall.

Incidental finding of herpes zoster infection by Tc-99m HMPAO.

Tc-99m HMPAO brain SPECT study incidentally demonstrated a large area of increased accumulation in the soft tissue of the face and fronto-parietal region of the head, corresponding to the distribution of the ophthalmic and maxillary branches of the trigeminal nerve, which had been infected by herpes zoster.

[An autopsy case of primary malignant melanoma of the esophagus--appraisal of radiation therapy].

A rare case of primary malignant melanoma of the esophagus is reported. Our case is of interest because an extraordinary effect was observed early during conventional radiation therapy. The primary lesion was irradiated with 60 Gy, it almost disappeared. Metastatic lesions were irradiated with 30 Gy, their volume decreased by more than 50%. At autopsy, these lesions were more or less unchanged. Histologically, cells from the main tumor were small and round. The growth of pleomorphic cells with large amounts of melanin granules was observed in the regions of irradiation. These findings suggest that the small round cells in malignant melanoma are radiosensitive and the pleomorphic cells with abundant melanin granules are radioresistant.

[Effects of PS-K as adjuvant therapy for primary lung cancer--an autopsy case].

A 57-year-old woman underwent left lower lobectomy for primary lung cancer (papillary adenocarcinoma, T2NOMO). Despite postoperative adjuvant immunochemotherapy with 5-Fu, Carboquone and PS-K, bone metastases were recognized at 9 months after the surgery. She received radiation therapy and was administered PS-K alone, 3.0 g daily. As a result, long-term good condition was maintained with a positive PPD skin test. The bone metastases developed slowly, and the patient survived 5 years and 8 months after the operation. At autopsy, no obvious metastatic lesions were identified except for the bone metastases. The cause of her death was thought to be acute renal failure due to severe hemorrhage from an esophageal ulcer. Moreover, she was doomed because of advanced polycystic disease.

[Effectiveness of CT in the determination of the extent of infiltration of head and neck cancer].

For cancer therapy, especially radiation therapy, it is very important to determinate the invaded region. Since computed tomography (CT) is introduced into medical radiography, it has become well known that it has a highly diagnostic value not only for the intracranial lesions, but also for extracranial pathological processes. In this paper, 4 cases (pinealoma, rhabdomyosarcoma of the ear, nasopharynx tumor and Histiocytosis X of the orbit) are presented. In each case, pre-therapeutic and posttherapeutic CT images are shown. CT is also very useful during and after therapy in patients with malignancy of the head and neck.

[Diagnostic imaging in cancer therapy: the genito-urinary system].

Diagnostic approach of the tumors of the genitourinary system with multiple imaging modalities was described. The way of staging of the diseases was emphasized. Angiography has been used for diagnosis of renal and renal pelvic tumors, but CT and ultrasonography are more useful than angiography for evaluation of the extension of the disease into the perirenal or pararenal space as well as detection of lymph-node metastasis. Invasion into the renal vein or the interior vena cava is also demonstrated by those new modalities. They are also useful for post-operative follow up. For the staging of the bladder tumor, transurethral ultrasonography or CT are useful, as they can detect the extravesical infiltration of the diseases. Bone scintigraphy is more effective in detection of bony metastasis, which is essential for staging of prostatic carcinoma. Although transurethral ultrasonography is more useful in the evaluation of the extension of the prostatic carcinoma, when the extension is advanced CT is better than ultrasonography. CT is also effective for diagnosis of bony metastasis.

Temporal bone radiography using the orthopantomograph.

Temporal bone radiographs obtained with an Orthopantomograph were compared with conventional radiographs. In acoustic neurinoma, cholesteatoma, otitis media, and middle fossa tumors, both methods demonstrated the abnormalities well. In two cases with lesions extending beyond the range of conventional projections, the broad orthopantomographic coverage was very valuable. Mastoid air cells, the mastoid process, petrous ridge, and internal auditory meatus were well demonstrated by both techniques. Orthopantomography was found to be superior in the demonstration of the petrous apex, while the superior semicircular canal was better demonstrated on the conventional views. Bilateral symmetry was particularly good and because of fewer films, radiation exposure was considerably less with orthopantomography. For many applications, orthopantomography is an adequate convenient substitute for conventional methods of examining the temporal bones.

Venous angioma of the neck in a child.

Venous vascular masses in the neck are rare entities in the pediatric population. Aneurysmal dilatation or ectasia of the superficial cervical vein, the so-called "venous angioma" is a very uncommon condition. A 12-year old female with venous angioma involving the right neck is described. The preoperative diagnosis was confirmed by angiography.
