Effect of gamma-globulin preparations on phagocytic function of whole blood.

Effect of gamma globulin preparations on opsonic activity in whole blood from non-immunodeficiency was studied using chemiluminescence as the parameter of phagocytic function of granulocytes. Poly (ethylene) glycol treated, Fc-intact preparation clearly enhanced chemiluminescence (luminol-dependent) of blood cells phagocytosing zymosan, Escherichia coli (E. coli), or Pseudomonas aeruginosa (P. aeruginosa), but pepsin-treated preparation showed no effect. Whole blood added with intact preparation at the final concentration of 4 mg/ml showed enhanced CL induced by E. coli and P. aeruginosa in many individuals, especially in infancy, although in adult age suppressive effect was often observed. In six patients with pediatric malignancy and three newborns with suspected septicemia, CL induced by E. coli or P. aeruginosa was measured after the administration of 150 mg/kg of intact preparations, and 4/6 of malignancy showed increased CL by E. coli, and all infants showed increased CL by E. coli and P. aeruginosa. These results suggest that intact gamma globulin preparation can increase phagocytic ability of whole blood in non-immunodeficiency via its opsonins, and may justify the administration of intact gamma globulins in non-immunodeficiency for the purpose of treating bacterial infections in some selected cases.

A case of Japanese Burkitt's lymphoma associated with EB virus.

A 6-year-old girl was admitted for cervical lymphadenopathy. She was diagnosed histologically as having Burkitt's lymphoma with partial involvement of follicles. Immunological study of a biopsy specimen demonstrated that many cells were surface immunoglobulin (lambda light chain)-positive, and chromosomal analysis revealed a translocation of (8;14) (q24;q32) in all blastic cells. Her serum antibody titers to Epstein-Barr virus (EBV) capsid antigen and to early antigen were 1:320 and 1:160, respectively, and most of the cells from the lymphoma biopsy specimen were EBV-associated nuclear antigen (EBNA)-positive. In addition, EBV DNA/DNA reassociation kinetic analysis showed 22 EBV genome equivalents per cell. Complete remission was obtained by CHOP therapy (consisting of cyclophosphamide, adriamycin, vincristine, and prednisolone) combined with cervical irradiation. She is in complete remission now (17 months after the onset).

Chemiluminescence of whole blood. 1. A simple and rapid method for the estimation of phagocytic function of granulocytes and opsonic activity in whole blood.

Luminol-dependent chemiluminescence of whole blood (whole blood CL) was developed to estimate the phagocytic function of granulocytes and the serum opsonic activity simultaneously. Whole blood (0.1 ml) was examined directly and results were obtained within 20 minutes. Phagocytic function of granulocytes can be estimated from the peak CL of whole blood and the number of granulocytes in a specimen, and the opsonic activity from the amount of time the peak CL is shown after the addition of nonopsonized CL inducer. Thus this method, using several types of CL inducer, offers much information concerning the functions of the phagocytic system in whole blood.

Human monocyte-derived chemotactic factor for granulocytes.

Production of the chemotactic factor for granulocytes (CFG) by human mononuclear cells was studied using the agarose plate method. CFG was produced by mononuclear cells placed in an agarose well, and a chemotactic gradient was formed in the surrounding area. The degree of CFG production by mononuclear cells was estimated by the chemotactic response of indicator granulocytes which were placed in separated agarose wells in the same dish. CFG production was observed after 3 h culture in a well where LPS or anti-beta 2 microglobulin serum stimulated mononuclear cells were placed. CFG did not show chemotactic activity for monocytes. PHA, Con A, PWM and Sepharose-Protein A failed to activate mononuclear cells to produce significant amounts of CFG compared with non-stimulated cells. There was a dose-response relationship between the chemotactic activity of the CFG produced and the number of phagocytic mononuclear cells in well culture. Thus it was concluded that the CFG-producing cells are not lymphocytes, but phagocytic mononuclear cells or monocytes.

Characteristics of impaired chemotactic function in cord blood leukocytes.

Mobilities of cord blood granulocytes were studied using the agarose plate method and Boyden's chamber method. In the agarose plate, granulocytes of cord blood were shown to have moderately decreased responses in chemotaxis and chemokinesis induced by Escherichia coli-derived chemotactic factor and/or zymosan-activated serum, whereas they were shown to have a normal capacity of random mobility. Although their distance and index of chemotaxis or chemokinesis in the agarose plate were significantly less than those of adult granulocytes, response rate in both types of mobility were evidently higher compared with those in patients with chemotactic defect. Furthermore, there is a difference between chemotactic responses of cord blood granulocytes to E. coli-derived chemotactic factor and to zymosan-activated serum in the agarose plate method. Using the latter, a more distinguishable difference between chemotactic responses of cord blood granulocytes and adult granulocytes was shown. The Boyden's chamber method tended to show a more significant difference between chemotactic responses of granulocytes of cord blood and adults than in the agarose plate method.

Development of tissue factor activity in mononuclear cells cultured in vitro.

Tissue factor activity was detected in human mononuclear cells cultured with E. coli endotoxin in vitro. The effecient concentration of endotoxin inducing significant tissue factor development was in excess of 10(-3) microgram/ml, and a dose response type relationship was seen between them. The activity developed after culturing for 2 hr and increased up to 6 hr, and thereafter no significant increase was observed. Although the activity was detected both in cell extract and on cell surface, the main activity seemed to exist on the cell surface. No correlation was observed between the synthetic rate of nucleic acid and the rate of development of the activity. Although the activity was detected also in a granulocyte preparation, it was significantly less than that in mononuclear cells. The development of activity was observed when lymphocytes were cultured in RPMI 1640 medium, MEM, and in autologous serum. However, the activity was not observed when cultured in Hanks solution and PBS, in which the main difference from RPMI was the absence of amino acids, and in autologous serum containing sodium citrate which was added for elimination of Ca++ from serum. Moreover, actinomycin D suppressed the development of activity, and the same was noted at low culture temperature. These results suggest that some metabolic change of the cell membrane triggered by endotoxin may induce the development of tissue factor in cells.
