[The role of stereotaxic biopsy in the therapeutic approach to malignant gliomas (author's transl)].

Analysis of 770 biopsies of intra-cerebral neoplasms under stereotaxic conditions and performed in Bordeaux, Rennes and Marseilles. The study emphasized the relative benign nature of this examination (1% deaths, 1.4 % severe sequellae) bearing in mind the severity of the underlying disease. In a very large number of cases (from 7 to 38% according to Schools) the diagnosis made before biopsy was fundamentally altered by this examination. The quasi-certitude of the nature of the lesion, and the possible facilitation of surgery by the precision of the lesional topography, and finally the possibility of a therapeutic gesture of evacuation in certain cases, is felt to justify intra-cranial biopsy performed under stereotaxic conditions.

The carnivore remains from the Sima de los Huesos Middle Pleistocene site (Sierra de Atapuerca, Spain).

Remains of carnivores from the Sima de los Huesos site representing at least 158 adult individuals of a primitive (i.e., not very speleoid) form of Ursus deningeri Von Reichenau 1906, have been recovered through the 1995 field season. These new finds extend our knowledge of this group in the Sierra de Atapuerca Middle Pleistocene. Material previously classified as Cuoninae indet, is now assigned to Canis lupus and a third metatarsal assigned in 1987 to Panthera of gombaszoegensis, is in our opinion only attributable to Panthera sp. The family Mustelidae is added to the faunal list and includes Martes sp. and a smaller species. The presence of Panthera leo cf. fossilis, Lynx pardina spelaea and Felis silvestris, is confirmed. The presence of a not very speloid Ursus deningeri, together with the rest of the carnivore assemblage, points to a not very late Middle Pleistocene age, i.e., oxygen isotope stage 7 or older. Relative frequencies of skeletal elements for the bear and fox samples are without major biases. The age structure of the bear sample, based on dental wear stages, does not follow the typical hibernation mortality profile and resembles a catastrophic profile. The site was not a natal or refuge den. The hypothesis that the site was a natural trap is the most plausible. If the Sima de los Huesos functioned as a natural trap (without an egress out), the human accumulation cannot be attributed to carnivore: activities and must be explained differently.

[Influence of gestational age and postnatal kidney maturation on the kinetics of gentamicin].

The current schedule for gentamicin administration to newborn infants, in doses of 2.5 mg/kg every twelve hours during the first week of life was evaluated in pre-term newborns under 35 weeks of gestational age (GA). Pharmacokinetic studies in steady state conditions were performed at the third and seventh days of therapy (periods A and B respectively) in nine pre-term (GA 30 to 34 weeks) and ten full-term newborns. Minimal gentamicin blood concentrations at period A were 1.96 +/- 0.32 micrograms/ml in term newborns and 2.51 +/- 0.48 micrograms/ml in pre-term infants (p less than 0.005) and, at period B, 1.49 +/- 0.37 micrograms/ml and 2.33 +/- 0.34 respectively (p less than 0.001). Gentamicin excretion showed good correlations with gestational age (r: 0.654; p less than 0.005) and creatinine clearance (r: 0.628; p less than 0.005).

A multisite field test of the acceptability of physical activity counseling in primary care: project PACE.

Primary health-care providers have been encouraged to counsel their patients about regular physical activity, but there are significant barriers to effective counseling. In this study a program of training and materials was tested for acceptability to providers, office staff, and patients. Primary care providers and office staff were trained to use the Physician-based Assessment and Counseling for Exercise (PACE) materials in four geographical sites in the United States. The program was tested in a variety of settings and with diverse patient populations. The acceptability of the program during a five-month study period was evaluated through structured interviews. The training was effective in preparing the providers to counsel, and the program was generally acceptable to providers, office staff, and patients. Counseling was provided in less than five minutes by 70% of providers, and most patients reported following the recommendations given. The PACE program assists providers in overcoming barriers to counseling patients about physical activity. The PACE program is potentially an important part of a national effort to enhance the adoption and maintenance of physical activity among adults.
