Laryngeal configuration of 99 dysphonic patients.

The aim of this investigation was to study a wide range of dysphonic patients and determine the best matches among the laryngeal configurations on phonation for each patient and those previously established for control data, which were obtained from a professional voice user producing the whole range of voice qualities. Ninety-nine patients were selected and laryngeal photographs were produced for each patient. Fifteen laryngeal parameters were quantified and normalized. The data were sorted, scalar values assigned and a measure of similarity between configurations applied. The best, second-best, third-best and worst matches between each patient separately and the control data were then examined. Although 41% of the patients did not have particularly unusual configurations, 59% exhibited a narrowing of the laryngeal vestibule caused by epiglottic retraction, cuneiform fronting and/or false fold adduction. This suggests that clinical reports for patients with dysphonia should contain information not only on any lesions present but also on laryngeal configurations and, in particular, vestibular narrowing.

[Superoxide dismutase (SOD) activity measured by ESR-spin trapping method in canine vocal cords].

Recently, superoxide dismutase (SOD) has been regarded as a scavenger of superoxide radical which is a important factor of 'reperfusion injury' in ischemic heart or in brain disorder. Its low activity means cell susceptibility to the superoxide radical in tissues. The SOD activity was examined in ten canine larynges after phonation by a electron spin resonance (ESR)-spin-trapping method. The values of mean SOD activity at the middle part of the membranous vocal cord, the vocal process of the arytenoid and the subglottal portion were 67.84 +/- 11.06 U/ml/g, 121.94 +/- 19.46U/ml/g and 115.46 +/- 19.18U/ml/g, respectively. There was a significant difference between the value of the membranous vocal cord and those of the other two (P less than 0.01). The result suggests that the low SOD activity at the part of the membranous vocal cord might relate to a pathogenesis of the laryngeal polyp and vocal nodule. The reperfusion injury may participate in the cause of such vocal cord disorders which grow commonly at the middle part of the membranous vocal cord.

[Aggressive surgery for hepatocellular carcinoma in advanced stage and with recurrence].

Between July 1973 and September 1990, 160 patients with hepatocellular carcinoma underwent hepatic resection at Keio University Hospital. Hepatic resection was carried out for patients with advanced diseases as well as recurrence. The patients with advanced disease consisted of 5 patients with obstructive jaundice, 6 with tumor thrombi in the portal trunk, one with tumor thrombus in hepatic vein, inferior vena cava and right atrium, 8 with satellite nodules in both lobes. Nine (45%) of these patients survived more than 2 years. Seven patients underwent removal of recurrent tumors: 4 in the remnant liver, one each at the left adrenal gland, lung and chest wall. Two patients lived longer than 2 years with relief of pain.

Carcinoma, Hepatocellular

Clinical studies on the effectiveness of prazosin HCl (Minipress tablets) in the treatment of dysuria accompanying benign prostatic hyperplasia.

Prazosin HCl was administered in dosages of 1.5-4.5 mg/day for 4 weeks to 40 patients suffering from dysuria accompanying prostatic hyperplasia. Significant reductions were observed in the mean frequency of night-time and 24-hour urination. Additionally, significant improvements were recorded for the time for start of urination, time from start to finish of urination and related variables. The improvements correlated with dosage levels. Objective variables measured by urine flow rates also exhibited significant improvement. A statistically significant decrease in systolic without a decline in diastolic blood pressure was observed. Slight dizziness was observed in 1 patient and elevated laboratory test values in another, but neither warranted discontinuation of the medication; however, 1 patient did have two doses of medication held. Overall, the drug was judged beneficial to some degree in 95% of the patients.

Clinical evaluation of effects of prazosin in patients with benign prostatic obstruction. A double-blind, multi-institutional, Paraprost-controlled study.

Prazosin, a selective alpha 1-adrenoceptor blocker, was compared with Paraprost in a double-blind, multi-institutional study in patients with benign prostatic hyperplasia. The trial encompassed 77 evaluable patients in the prazosin group and 79 in the Paraprost group. Both drugs significantly improved obstructive and irritative symptoms. Residual urine (volume or percentage of total urine) was reduced by both prazosin and Paraprost. Average and maximum flow rates increased significantly in the prazosin group while those were unchanged in the Paraprost group. Side effects occurred in 1 patient treated with prazosin (1.1%) and 3 patients with Paraprost (3.3%). Thus, we confirm the clinical value and minimal side effects of prazosin in the treatment of bladder outflow obstruction resulting from benign prostatic hyperplasia.

New screening system for unruptured cerebral aneurysms--combination of an expert system and DSA examination.

We have designed a screening system to diagnose unruptured aneurysms, including the use of digital subtraction angiography (DSA). We surveyed 115 patients who had undergone clipping procedures after subarachnoid hemorrhage (SAH) and questioned them with regard to the subjective symptoms. Sixty-eight of 92 patients who returned the questionnaire reported, prior to rupture, headache, eye pain, and neck pain most frequently, and also impairment of extraocular movements, ptosis, visual field defects, and motor and sensory disturbances. Nineteen (47.5%) of 40 patients who had complete pain relief after surgery complained of headache from 1 week to 1 month before SAH. In addition, nine patients (22.5%) complained of headache for several years, and were also pain-free after surgery. For the indication of DSA, we employed an expert system based on fuzzy set theory. Seven groups of parameters are: Group 1, a basic questionnaire concerning age, sex, and past and family histories; Group 2, 15 warning signs selected on the basis of retrospective study; and Groups 3-7, detailed questions concerning each sign. Scoring weights assigned to each condition based on the results of the retrospective study, and threshold values were determined by several neurosurgeons. The certainty factors for intermediate hypotheses were calculated from these weights and threshold values and summed up, from which the conclusion was obtained. Twelve new cases of unruptured cerebral aneurysm were diagnosed using this screening system. This system may improve the ability to diagnose cerebral aneurysms before rupture.

Lambda bacteriophage-mediated transduction of ColE1 deoxyribonucleic acid having a lambda bacteriophage-cohesive end site: selection of packageable-length deoxyribonucleic acid.

An in vitro recombinant ColE1-cos lambda deoxyribonucleic acid (DNA) molecule, pKY96, has 70% of the length of lambda phage DNA. The process of lambda phage-mediated transduction of pKY96 generated a small amount of transducing phage particles containing ColE1-cos lambda DNA molecules of 80 or 101% of the length of lambda phage DNA, in addition to those containing original pKY96 DNA molecules. The newly isolated larger plasmid DNAs were transduced 100 times more efficiently than pKY96 DNA. Their structures were compared with that of a prototype pKY96 DNA, and the mechanism of the formation of these molecules is discussed.

Bacteriophage lambda

Hepatoma with obstructive jaundice due to the migration of a tumor mass in the biliary tract: report of a successful resection.

A 51-year-old man underwent extended right lobectomy for hepatoma of the right lobe with obstructive jaundice due to migration of the tumor mass in the common and bilateral hepatic ducts. Severe jaundice amounting to 32.6 mg% and suppurative cholangitis were relieved by drainage through the cannulation into the hepatic ducts of the bilateral lobes. Although hepatoma was combined with liver cirrhosis, the patient could tolerate extended right lobectomy. The patient is doing relatively well without jaundice 2 years after operation, but a recurrent tumor appeared on the celiac angiogram taken 1 year and 6 months following operation. This is the fourth report on hepatic resection of such cases, and difficult problems involved with that procedure are presented and discussed.

Carcinoma, Hepatocellular

Carcinoma of the proximal bile ducts.

The records of 31 patients with carcinoma of the proximal bile ducts were reviewed and the operative procedures, results, causes of deaths and autopsy findings were analyzed and correlated. Ten poor risk patients were treated by external drainage and died of jaundice or abscesses of the liver within six months. However, one patient survived 13 months after effective bilateral drainage. Twelve patients underwent intubation through the tumor into the hepatic ducts with or without postoperative irradiation. Four patients with irradiation survived 42, 15, 15 and ten months, while eight patients without irradiation died within six months. Autopsy findings of two patients who survived 15 months revealed metastases to the various organs. Four patients underwent resection. A patient who underwent resection of the common hepatic duct and hepaticoduodenostomy died of ascending cholangitis nine months postoperatively, while a patient treated by resection of the common hepatic duct with hepaticojejunostomy died of a recurrence 25 months postoperatively. Two patients underwent left hepatic lobectomy and resection of the right hepatic duct followed by hepatojejunostomy. One patient survived 25 months and died of a reccurrence, while the other patient died of abscesses of the liver ten months postoperatively.

Pathological study of lead poisoning in whooper swans (Cygnus cygnus) in Japan.

During spring 1989, thirty-three whooper swans (Cygnus cygnus) died at Lake Miyajima in Hokkaido, Japan; 15 were examined. The birds were diagnosed as having subacute lead poisoning due to ingestion of spent lead shot. The main gross findings were bile-stained liver, edematous or gelatinous bone marrow, bile-stained lining with hyperkeratosis and lead pellets in the gizzard, and proventricular impaction. Histopathologically, there was lead-hemolytic jaundice of the liver, hemosiderosis in the liver and spleen, and hypoplasia of the bone marrow with increased numbers of polychromatic erythroblasts. Acid-fast intranuclear inclusion bodies were seen in kidneys of seven swans. Under electron microscopy, inclusion bodies had frayed contours and consisted of high-electron-dense fine granules. The lead concentration of the liver ranged from 5.5 to 44.3 mg/kg wet weight. It was suggested that these changes resulted from excess breakdown of erythrocytes, inhibition of heme synthesis, and impaired erythropoiesis caused by lead shot.
