The multiscan appearance of eosinophilic granuloma.

A case of multifocal eosinophilic granuloma is presented in which a 99mTc-methylene diphosphonate bone scan revealed areas of increased, normal, and decreased uptake of the radionuclide. A radiographic bone survey demonstrated some but not all of the lesions seen on the bone scan; additionally, certain lesions were demonstrated only by the radiographs. A radionuclide bone scan is therefore unreliable as the sole diagnostic procedure for evaluating the distribution of eosinophilic granuloma and should always be complemented with a radiographic bone survey.

Aberrant jugular bulb presenting as a middle ear mass.

Case reports of 3 patients seen with a bluish mass behind the tympanic membrane are presented. The initial diagnosis was probable glomus tumor. In 1 patient, middle ear exploration confirmed the presence of a high-lying jugular bulb. In the other 2, venography demonstrated that the jugular bulb projected superiorly into the middle ear cavity. In all 3, polytomography demonstrated a dehiscence of the bony septum which normally separates the jugular bulb from the hypotympanum. When a bony dehiscence is seen in the absence of destructive changes, a diagnosis of aberrant jugular bulb is strongly suggested.

Hindgut duplication with rectovaginal fistula.

Complete colon duplication is an extremely rare congenital anomaly that occasionally presents diagnostic problems. This report presents a 23-year-old black woman with complete duplication of the colon and distal ileum, with termination of 1 colon into the vagina.

Decreased lung perfusion due to broncholithiasis.

Two patients each with symptoms of an endobronchial lesion showed decreased unilateral lung perfusion upon radionuclide imaging. Chest films were normal except for hilar lymph node calcifications. Bronchography and tomography revealed compression and obstruction of the bronchial tree by the calcified lymph nodes, accounting for the patients' symptoms. Incomplete bronchial obstruction or collateral air drift accounted for the normal appearance of the lungs on the PA chest films.
