[Localized amyloidosis of the lower respiratory tract].

Primary amyloidosis localised to the lungs can simulate different types of lung diseases such as tuberculosis, carcinoma and repeated infections in the lower respiratory tract. The possibilities of treatment are limited. The prognosis is usually good, except when there is diffuse pulmonary involvement. Such patients can develop progressive respiratory failure The mechanism of amyloidogenesis is unclear. At the cellular level the macrophage may play a central role.

Characterization of pleural effusions by flow-cytometric DNA analysis.

Pleural effusions from 58 patients were studied by cytology and flow-cytometry. Relative cellular DNA content was measured, giving an estimate of ploidy as well as cell cycle distribution. Twenty-one patients had non-malignant diseases while the rest had malignant diseases. Pleural effusions from 13 patients contained malignant cells. In 2 cytologically equivocal cases and in one cytologically negative case aneuploid cells were demonstrated in the histograms. The cell cycle distribution of the diploid tumour cells did not differ from that of the diploid cells in benign effusions. The demonstration of an aneuploid histogram by flow cytometry favours a cancer diagnosis, and can provide valuable and decisive information in addition to cytological results.

Occurrence of lung cancer in workers producing chromium pigments.

The results of a follow-up study on the incidence of lung cancer in 133 workers producing zinc chromate pigments are presented. By the end of 1972 three cases of lung cancer had occurred in a sub-cohort of 24 workers who had been employed for over three years. The same group of workers has now been followed up to December 1980 and three further cases of lung cancer were found. The observed/expected ratio was 44 in this group, virtually the same as at the end of 1972. Five of the six patients smoked. Only one had been exposed to chromates other than zinc chromates.

Pneumothorax during combination chemotherapy.

Two patients with pulmonary metastases from synovial sarcoma and germ-cell tumor developed first bilateral, then repeated unilateral pneumothoraces during combination chemotherapy. It is suggested that chemotherapy may occasionally cause pneumothorax.

Comparative pharmacokinetics of theophylline and aminophylline in man.

1 The pharmacokinetics of theophylline and aminophylline was compared after oral administration and intravenous infusion. 2 Theophylline (250 mg) and aminophylline (390 mg) were taken orally by eight healthy volunteers in a randomized cross-over study. 3 In another cross-over study theophylline and aminophylline were administered intravenously to six healthy volunteers at a dose corresponding to 5 mg/kg pure theophylline. 4 The protein binding of the theophylline in serum collected during the intravenous study was studied by ultrafiltration. The serum concentration of theophylline was measured by high pressure liquid chromatography. 5 Almost identical concentration-time curves were found for theophylline and aminophylline in both of the studies. No significant difference was found in the pharmacokinetic parameters or protein binding with the two preparations.

Propranolol in the treatment of migraine.

Beta-blocking drugs that prevent cranial vasodilatation are potentially valuable in the prophylaxis of migraine. Forty-nine patients with either classic or common migraine were treated with propranolol 160 mg/day for an average of six months. The first 30 of the patients to respond well to this treatment then participated in a double-blind cross-over trial with a placebo and propranolol. The mean frequency of headache attacks was significantly reduced by propranolol. None of the patients expressed a preference for placebo. Propranolol seems to be an effective prophylactic for common and classic migraine but the antimigraine properties of the various beta-blocking agents probably differ.
