Oral and maxillofacial surgery: considerations in cleft nasal deformities.

Oral and maxillofacial surgery approaches and techniques are important considerations in the comprehensive management of cleft nasal deformities. Establishing a proper bony base for the soft tissues can involve secondary procedures such as alveolar cleft grafting and orthognathic surgery. The cleft deformity itself, but particularly previous surgery, usually dictate modifications of orthognathic surgery techniques. Distraction osteogenesis potentially allows for earlier intervention as well as more extensive bony changes with decreased surgical risk.

Dacron mesh tray and cancellous bone in reconstruction of mandibular defects.

We used a Dacron-urethane mesh tray filled with cancellous bone for mandibular reconstruction in 17 patients. Five patients with traumatic defects and two with benign tumors developed solid, functional mandibles. Among ten patients with squamous cell carcinoma, eight had successful reconstruction; one required a second procedure. Mandibles of two of three patients with osteoradionecrosis were successfully reconstructed, but only one of three primary reconstructions was initially successful. The Dacron-urethane mesh tray has the advantage of being stiff but malleable. It easily fits the defect, is radiolucent, and may be used either before or after radiotherapy. We recommend delayed reconstruction, adequate soft-tissue coverage, good immobilization of the mandible, no intraoral contamination, especially with osteoradionecrosis, and hyperbaric oxygen in patients who have been irradiated or have osteoradionecrosis.

Epsilon aminocaproic acid in hemophiliacs undergoing dental extractions: a concise review.

On the basis of our review of the literature, it can be concluded that the advantages of E-A.C.A. include (1) safe execution of oral surgical procedures in persons with hemophilia of varying degrees of severity, (2) decrease or total elimination of the need for specific factor replacement therapy, (3) extremely positive cost effectiveness, and (4) reproducibility of results at different hemophilia centers.

Multiple dental extractions using general anesthesia for a patient with Down and Eisenmenger syndromes and periodontal disease.

A patient with Down syndrome and severe retardation and Eisenmenger syndrome sought dental treatment at the Oregon Health Sciences University Hospital Dental Service. Eisenmenger syndrome is a form of cyanotic congenital cardiovascular pulmonary disease. The cardiac structural abnormalities associated with Eisenmenger syndrome preclude the use of conscious sedation and predispose a patient to the development of bacterial endocarditis. A prophylactic antibiotic regimen is recommended for patients with Eisenmenger syndrome who are undergoing procedures that produce a transient bacteremia. This article reviews the appropriate measures necessary for the safe anesthetic, operative, and perioperative care of a severely compromised patient. It is based on an understanding of the pathophysiology associated with Eisenmenger syndrome.
