Contrast-enhanced three-dimensional MR angiography with an elliptical centric view for the evaluation of intracranial aneurysms.

The purpose of this study was to assess the feasibility of high spatial resolution, selective arterial phase, 3D contrast-enhanced (CE) MR angiography with first pass bolus, software-trigger, elliptical centric view ordering in the detection of intracranial aneurysms. Our study included nine consecutive patients with ten intracranial aneurysms. 3D TOF MR angiography and 3D CE MR angiography were carried out with a 1.5-T MR scanner. 3D CE MR angiography was performed with an automated bolus detection algorithm and elliptical centric view order using ultrafast SPGR with a spatial resolution of 0.63x0.83x0.5 mm and imaging time of 55 s. Observers detected seven of ten aneurysms on 3D TOF MR angiograms and nine of ten aneurysms on 3D CE MR angiograms. 3D CE MR angiography clearly revealed an IC-PC aneurysm with a relatively smaller neck, a broad-based small aneurysm originating from tortuous and dilated MCA bifurcation, and a residual aneurysm and parent vessels adjacent to metallic aneurysmal clips, which had relatively low signal intensities on 3D TOF MR angiograms. 3D CE MR angiography was found to be a good and promising technique for detecting intracranial aneurysms with small necks and slow flow, vasculature with aneurysmal clips and tortuous vasculature with disturbed flow.

Psychological distress experienced by families of cancer patients: preliminary findings from psychiatric consultation of a Cancer Center Hospital.

OBJECTIVE: The purpose of the present study was to obtain preliminary findings regarding psychiatric disorders and background characteristics among Japanese family members of cancer patients. METHODS: We investigated the psychiatric diagnosis and background factors of family members of cancer patients by analyzing the consultation data of patients referred to the Psychiatry Division, National Cancer Centre Hospital East, Japan. RESULTS: Of a total of 1469 psychiatric consultation, 47 (3.2%) family members were referred, and 85% of them were spouses. The most common patient cancer site was the lung. Approximately one-half of the referred family encountered patients' end-of-life issues. The most common psychiatric diagnoses were adjustment disorders, followed by major depression. CONCLUSIONS: These preliminary findings suggest that psychosocial support for family members is not fully delivered and development of a comprehensive support system for caregivers of cancer patients is an urgent issue in the clinical oncology setting in Japan.

A case of diffuse neurofibrillary tangles with calcification.

We report a 79-year-old female with atypical senile dementia with Fahr-type calcification. The patient started to show memory disturbance at the age of 75 years, followed by visual hallucination, stereotypy, personality changes such as irritability, aggression and disinhibition. Brain computed tomography (CT) demonstrated bilateral and symmetric calcification of the basal ganglia and thalamus. Magnetic resonance imaging (MRI) revealed diffuse cortical atrophy pronounced in the fronto-temporal areas. On MRI T1-weighted images the calcified areas showed a mixture of low- and high-intensity signals. Based on the overlapping clinical symptoms of Alzheimer's disease and Pick's disease, together with the brain CT and MRI findings, we clinically diagnosed the patient as having 'diffuse neurofibrillary tangles with calcification' (DNTC). The characteristics of psychiatric symptoms and neuroradiological findings in DNTC are discussed.

Diffusion-weighted echo planar imaging of ovarian tumors: is it useful to measure apparent diffusion coefficients?

PURPOSE: Our goal was to test the hypothesis, as previously reported in other studies, that apparent diffusion coefficients (ADCs) provide specific information to diagnose ovarian tumors, especially to discriminate between benign and malignant lesions. METHOD: T1-and T2-weighted spin echo imaging and diffusion-weighted echo planar imaging were performed in 31 women with 61 cystic components of ovarian tumors. RESULTS: The lesions that showed typical watery intensity, hypointensity in T1-weighted imaging, and hyperintensity in T2-weighted imaging had similar ADCs, ranging from 1.54 to 1.84 x 10(-3) mm2 /s. The lesions that showed signal intensity different from typical watery intensity in conventional MRI tended to have low ADCs. In endometrial cysts, the mean ADC of the subgroup that showed typical watery intensity was higher than that of other subgroups. CONCLUSION: With conventional MRI, a tendency of ADCs could be predicted. ADCs may not provide additional information, especially to discriminate benign from malignant lesions.
