Effect of cyclosporin a on immediate early gene in rat global ischemia and its neuroprotection.

The expressions of the immediate early genes, c-fos and c-jun, and their product proteins C-FOS, C-JUN, and P-JUN were examined in the hippocampal CA1 subfield after global ischemia and reperfusion in rats treated with cyclosporin A. More than 90% neuronal cell death was seen in hippocampal CA1 7 days after global ischemia in control animals, but only 5% cell death after ischemia was seen in the CsA-treated animals. The expressions of c-fos and c-jun mRNA in the control animals were detected with an increase from 1 to 48 h after ischemia. On the other hand, they showed significant suppression in the CsA-treated animals. Increased expressions of C-FOS were found 1, 24, and 48 h after reperfusion in the control animals. In the CsA-treated animals C-FOS expression was found to increase, but the expression level reduced to a statistically insignificant level within 48 h after the ischemia. C-JUN and P-JUN expressions increased in control animals, but were almost completely suppressed in the CsA-treated animals. The present study demonstrated that the suppressant effects of CsA on IEGs and their products might have causal relationship to the dramatic protecting effect of the drug against delayed neuronal cell death.

Corneal higher-order aberrations induced by overnight orthokeratology.

PURPOSE: To evaluate corneal higher-order aberrations induced by overnight orthokeratology for myopia. DESIGN: Prospective, noncomparative, consecutive, interventional case series. METHODS: A prospective study was conducted in 64 eyes of 39 patients with overnight orthokeratology for myopia, who were followed up for at least 3 months and attained uncorrected visual acuity of 20/20 or better. Corneal height data were obtained with computerized videokeratography (TMS-2N, Tomey), and wavefront aberration was derived using Zernike polynomials. Higher-order aberrations of the cornea were calculated for 3- and 6-mm pupils. RESULTS: Orthokeratology significantly reduced manifest refraction from -2.60 +/- 1.13 (mean +/- SD) diopters to -0.17 +/- 0.31 diopters (P < .0001, paired t test). Root-mean-square (RMS) of third-order (coma-like) aberrations significantly increased by orthokeratology for both 3-mm (P < .0001, paired t test) and 6-mm (P < .0001) pupils. Fourth-order RMS (spherical-like) aberrations increased significantly by the treatment for both 3-mm (P < .0001) and 6-mm (P < .0001) pupils. Vertical coma significantly changed from positive to negative for both 3-mm (P = .0323) and 6-mm (P < .0001) pupils. Horizontal coma significantly increased to the positive direction for both 3-mm (P < .0001) and 6-mm (P < .0001) pupils. Increases in the third- and fourth-order RMS showed significant positive correlations with the amount of myopic correction for 3-mm (Pearson correlation coefficient, r = .452, P = .0001 for third-order RMS, r = .381, P = .0017 for fourth-order RMS) and 6-mm (r = .499, P < .0001, r = .455, P = .0001) pupils. CONCLUSIONS: Corneal higher-order aberrations significantly increased, even in clinically successful orthokeratology cases. The increases in the higher-order aberrations correlated with the magnitude of myopic correction.

Steepening of corneal curvature with contraction of the ciliary muscle.

PURPOSE: To measure the changes of corneal curvature during contraction of the ciliary muscle. SETTING: Department of Ophthalmology, St. Luke's International Hospital, Tokyo, Japan. METHODS: Twenty-eight eyes of 14 healthy volunteers under 40 years old were enrolled in this prospective randomized controlled study and divided into pilocarpine and control groups. Intraocular pressure (IOP), pupil diameter, and corneal topography were measured before and 40 minutes after instillation of topical pilocarpine 4% or balanced salt solution. Corneal topography was analyzed for the mean ring-power of Placido rings 1 through 25, average corneal power (ACP), and for spherical equivalent, regular astigmatism, asymmetry, and high-order irregularity by Fourier analysis. RESULTS: Pilocarpine had no effect on IOP, but it did cause a significant decrease in mean pupil diameter. Simultaneously, pilocarpine increased the mean ring powers for Placido rings 1 through 4 and the ACP (+0.13 diopters (D) +/- 0.17 [SD]; P=.017). By Fourier analysis, the mean spherical component for the central 3.0 mm of the cornea increased in the pilocarpine group (+0.08 +/- 0.15 D; P=.020). There were no changes in components of regular astigmatism, asymmetry, and high-order irregularity. CONCLUSIONS: The central cornea steepened in curvature and increased in power owing to contraction of the ciliary muscle. The results suggest that changes in corneal curvature increase refractive power during accommodation.

Retinopathy after percutaneous transluminal coronary angioplasty and stent insertion for acute myocardial infarction.

PURPOSE: To report microvascular retinopathy after percutaneous transluminal coronary angioplasty and stent insertion for acute myocardial infarction. DESIGN: Observational case series. METHODS: Retrospective review of seven patients with a retinal microangiopathy following coronary catheterization, angioplasty, and stent placement. RESULTS: All patients had bilateral cotton-wool spots in peripapillary distribution; four also had retinal hemorrhages in both eyes. None complained of visual symptoms. Lesions resolved spontaneously within several months in six patients. CONCLUSION: Although angioplasty and stent placement are less invasive than cardiac surgery, they appear to cause a retinopathy similar to postpump retinopathy.

Changes in corneal curvature in accommodation.

PURPOSE: To determine the changes in corneal curvature in accommodation. SETTING: Department of Ophthalmology, St. Luke's International Hospital, Tokyo, Japan. METHODS: Twenty-eight eyes of 14 healthy volunteers with a mean age of 38.9 years (range 28 to 65 years) were enrolled in this prospective study. Corneal topographies were taken before and during accommodation. Subjects achieved maximal accommodation by gazing at a target letter "A" in front of the eye. Keratometric values (K-values) obtained by topographies before and during accommodation were compared. RESULTS: Steepened corneal curvatures during accommodation were observed with the difference-plot view. Maximum K-values for the central 3.0 mm, 5.0 mm, and 7.0 mm corneal diameters significantly increased by a mean of 0.62 diopters (D) +/- 0.83 (SD) (P =.0005, paired t test), 0.60 +/- 0.90 D (P =.0015), and 0.72 +/- 0.65 D (P<.0001), respectively. Respective minimum K-values significantly increased by a mean of 0.62 +/- 0.83 D (P=.0005), 0.67 +/- 0.68 D (P<.0001), and 0.64 +/- 0.62 D (P<.0001). No statistically significant relationship was found between age and change in K-values. CONCLUSION: The results suggest that changes in corneal curvature in accommodation participate in the mechanism of accommodation.

Corneal higher order wavefront aberrations after hyperopic laser in situ keratomileusis.

PURPOSE: To evaluate the changes in corneal higher order wavefront aberrations after hyperopic laser in situ keratomileusis (LASIK). METHODS: In a prospective case series, 15 eyes of 12 patients who had hyperopic LASIK were evaluated. Corneal topography was obtained before and after hyperopic LASIK with a Nidek EC 5000 laser using 5.5/8.0 ablation zones. Using anterior corneal height data, the changes in corneal higher order wavefront aberrations were calculated. RESULTS: The surgery significantly increased both corneal coma-like aberration (preoperative/6 months postoperative, 0.054/0.147 [172% increase] for 3-mm pupil and 0.381/1.076 [182% increase] for 6-mm pupil) and corneal spherical-like aberration (preoperative/6 months postoperative, 0.039/0.067 [72% increase] for 3-mm pupil and 0.297/0.959 [223% increase] for 6-mm pupil). The surgery significantly decreased Zernike coefficient 12 and the polarity of corneal spherical aberration changed from preoperative positive value to negative postoperatively. For a 3-mm pupil, the achieved changes in spherical equivalent refraction significantly correlated with the induced changes in the corneal coma-like aberration (R = 0.629, P = .010), but not with those in corneal spherical-like aberration (R = 0.408, P=.133) or Zernike coefficient 12 (R = -0.301, P = .282). For a 6-mm pupil, the achieved changes in spherical equivalent refraction significantly correlated with the induced changes in the corneal spherical-like aberration (R = 0.862, P < .0001) and Zemike coefficient 12 (R = -0.872, P < .001) but not with those in corneal coma-like aberration (R = 0.449, P = .094). CONCLUSIONS: Hyperopic LASIK significantly increases corneal coma-like and spherical-like aberrations and changes corneal spherical aberration from a positive to negative value.
