MR features of angiosarcoma in a patient with Maffucci's syndrome.

Maffucci's syndrome is a rare sporadic congenital disorder associated with multiple enchondromas and soft tissue hemangiomas. These bone and soft tissue tumors have the potential of malignant transformation to various histologic types: chondrosarcoma, osteosarcoma, fibrosarcoma, and angiosarcoma. We report a rare case of Maffucci's syndrome with soft tissue angiosarcoma of the leg to illustrate MRI findings with pathologic correlations.

Pulmonary lymphangioleiomyomatosis in a male patient with tuberous sclerosis complex.

Pulmonary lymphangioleiomyomatosis (LAM) is a rare disease of unknown etiology that occurs almost exclusively in women of reproductive age. High-resolution computed tomography (HRCT) reveals the striking feature of diffuse cystic changes throughout the lung parenchyma. The correct diagnosis may be delayed by several years after the onset of symptoms because of the rarity of the disease and the need for chest CT scans to identify the lung involvement. We describe a case of pulmonary LAM in a male patient associated with tuberous sclerosis complex (TSC), in whom the early stage of disease could be depicted by chest HRCT scans.

Performance assessment of phased-array coil in breast MR imaging.

PURPOSE: To compare the performance of the phased-array coil (PAC) with that of the single-loop coil (SLC) in magnetic resonance (MR) imaging of the breast. MATERIALS AND METHODS: MR imaging was performed with a 1.5T MR imager. A phantom study was performed with the right element of the two coils to obtain their signal-to-noise ratio (SNR). MR images of the breasts of 12 patients with breast lesions were obtained with the PAC and SLC, and these images were reviewed by five readers in a blind evaluation employing a scoring system for assessing overall image quality. RESULTS: In the phantom study, the SLC exhibited a SNR 1.82 times higher than that of the PAC at the center of the coil; however, the SLC exhibited an inhomogeneous sensitivity profile and its SNR varied with the distance from the center of the coil in the horizontal and vertical directions. In most of the 12 patients, the MR images obtained with the PAC showed more noise than did those obtained with the SLC, and the PAC obtained lower scores than the SLC in the assessment of overall image quality; however, the difference was significant (p < 0.05) only in coronal imaging. On the other hand, the uniformity of fat saturation in the MR images obtained with the PAC was judged to be significantly superior to that obtained with the SLC (p < 0.05). CONCLUSION: Compared with the SLC, the PAC exhibited a lower SNR and was less advantageous at depicting the breast. However, the PAC provided more homogeneous fat saturation and might be useful for reducing artifacts.

Dynamic computed tomography and color Doppler ultrasound of renal parenchymal neoplasms: correlations with histopathological findings.

BACKGROUND: We evaluated whether color Doppler ultrasound (US) had diagnostic accuracy equal to dynamic computed tomography (CT) and whether performing dynamic CT and Doppler US together would be more informative in preoperative diagnosis of renal solid tumors. METHODS: A total of 110 renal solid tumors smaller than 7 cm were evaluated with dynamic CT and Doppler US. We compared the enhancement and the color flow patterns with the histopathological subtypes. RESULTS: Eighty-seven (95.6%) of 91 clear cell carcinomas showed rich enhancement in the cortical nephrographic phase (CNP) and 82 (90.1%) of them had color flow in the Doppler US. Of the total of 110 tumors, nine (8.1%) did not show color flow in spite of rich enhancement in the CNP. Conversely, eight (7.2%) of the 110 tumors showed color flow in spite of poor enhancement, including two chromophobe cell carcinomas and two metastatic renal tumors. CONCLUSIONS: The enhancement pattern in dynamic CT and the color flow pattern in Doppler US were different among the subtypes of RCC. Color Doppler US had diagnostic accuracy equal to dynamic CT in most patients with renal solid tumors. Although Doppler US may play a unique role in the diagnosis of some renal parenchymal solid tumors, it is sufficient to perform dynamic CT alone for diagnosis of clear cell carcinoma.

Localized relapse in bone marrow in a posttransplantation patient with t(6;9) acute myeloid leukemia.

We report a 38-year-old woman with t(6;9) acute myeloid leukemia who relapsed with localized leukemic cell growth in the bone marrow after she had undergone allogeneic bone marrow transplantation. The localized cell growth was first recognized by an apparent discrepancy in the DEK-CAN fusion transcript levels between the aspirates from the left and right iliac bone marrow. Magnetic resonance imaging of the iliac bone revealed localized cell accumulation in the left side. The nonhomogeneous and localized leukemic cell growth in this case may have been due to the graft-versus-leukemia effect following allogeneic transplantation with donor lymphocyte infusion. Serial monitoring of molecular markers for leukemia at different sites or magnetic resonance imaging of the bone marrow may be of value in detecting this type of relapse.

[A case of advanced gastric adenocarcinoma with mild elevation of serum SCC that responded remarkably to adjuvant chemotherapy of ADM, CDDP, ETP and 5-FU (ACVF)].

A 65-year-old man was referred to our hospital because of weight loss. Endoscopic examination and computed tomography (CT) revealed an advanced gastric cancer with multiple abdominal lymph node swellings. Distal partial gastrectomy was performed but lymph node resection was not done, since it was not thought to be curative. Adjuvant chemotherapy was performed for 4 courses with a regimen of ADM 20 mg/m2 day 1, CDDP 50 mg/m2 day 1, ETP 100 mg/day days 3-7, 5-FU 600 mg/m2 every other day on days 3-29. After 3 courses of ACVF therapy, the patient's serum CEA and SCC level normalized and the lymph node metastases became undetectable by CT scan. No severe side effects were observed at any time during the administration of these medications. In this case, serum SCC level was elevated even though histologic examination did not reveal squamous cell carcinoma but poorly differentiated adenocarcinoma. On immunohistochemical analysis, these tissues were stained diffusely with CEA, locally with AE1 + 3, and partially with PAS or Alcian blue. We speculate that this tumor could have developed the potency of SCC secretions without structural change into squamous metaplasia.

Malignant endocrine tumor of the pancreas associated with von Recklinghausen's disease.

We describe a case of malignant endocrine tumor of the pancreas associated with von Recklinghausen's disease. A 66-year-old woman with familial von Recklinghausen's disease was admitted to our hospital with weight loss and lower abdominal pain. Endoscopy revealed a submucosal tumor in the second part of the duodenum. Contrast-enhanced computed tomography showed a heterogeneous low-density tumor, measuring 20 x 15 mm, in the head of the pancreas, and metastatic lymphadenopathy. Angiography showed faint tumor staining without encasement or apparent increase in vascularity. Pancreatoduodenectomy was performed. Macroscopically, there was a solid tumor, measuring 20 x 18 mm, in the head of the pancreas. Histologically, a malignant endocrine tumor was shown, with direct invasion to the duodenum, and lymph node metastases. This is the thirteenth case of pancreatic malignant tumor and the fourth case of pancreatic endocrine tumor associated with von Recklinghausen's disease reported in the world literature.

Hepatic nodules with early enhancement during computed tomography portography: report of six cases.

AIM: To study the clinicopathologic characteristics of hepatic nodular lesions with high attenuation (increased portal blood flow) compared with surrounding hepatic parenchyma on computed tomography (CT) during arterial portography (CTAP). METHODS: For six lesions found in six patients demonstrated as a high-attenuated mass by CTAP, CT during hepatic arteriography (CTHA; n = 3 patients), digital subtraction hepatic arteriography (n = 6) and conventional helical CT (n = 6) were evaluated retrospectively and compared with histopathologic findings (n = 4). Pathologic diagnosis was atypical adenomatous hyperplasia, nodule-in-nodule hepatocellular carcinoma (HCC) in one resected lesion each and overt HCC in two biopsied lesions. Two patients did not undergo any therapy and were followed up. RESULTS: The average size of lesions was 2.2 cm (range 1.2-3.5 cm). The CTAP revealed high attenuation in all six lesions; entirely within the lesion (n = 4 lesions) or peripherally with a central low attenuation (n = 2). In contrast, CTHA showed low attenuated lesions; entirely within the mass (n = 2) or peripherally with a central high-attenuated spot (n = 1). Hepatic arteriogram revealed only two hypervascular lesions; entirely and partially in one each. In the arterial phase of helical CT, all but one lesion were iso- or hypo-attenuated. In two patients who were followed up to 39 and 55 months without therapy, neither tumor growth nor hemodynamic change of the lesion was recognized on CT. CONCLUSIONS: Even though the incidence of hepatic nodular lesions demonstrated as high attenuating on CTAP is low, all but one lesion in the current series showed iso- or hypo-attenuation on CTHA and/or helical CT, suggesting the hemodynamics are reciprocal between CTAP and CTHA. One exceptional lesion that showed high attenuation on both CTAP and conventional CT was pathologically advanced HCC. Based on the follow-up study of two untreated patients, this kind of lesion with high attenuation on CTAP seems to grow slowly.

Sensitivity of superselective arteriography for small hepatocellular carcinoma compared with proximal arteriography and computed tomography during superselective arteriography.

PURPOSE: To obtain successful arterial chemoembolization for hepatocellular carcinoma (HCC), we evaluated the sensitivity of proximal arteriography, superselective (subsegmental or more distal branch) arteriography and computed tomography (CT) during superselective arteriography and assessed the method for the injection of contrast medium. METHODS: Thirty-two patients with 38 HCCs (< or = 5 cm) with a mean diameter of 2.2 cm underwent digital subtraction arteriography of proximal and superselective arteriography. In addition, they also had helical CT during superselective arteriography. The contrast medium was injected with a mechanical injector (n = 6 lesions) or by hand (n = 32) for superselective arteriography and CT during superselective arteriography. The amount of contrast medium used for superselective arteriography and CT during superselective arteriography with the mechanical injector was 3.5 times and 9 times that with manual injection, respectively. RESULTS: Overall, 31 lesions (81.6%) were detected by proximal arteriography, 25 (65.8%) by superselective arteriography and 35 (92.1%) by CT during superselective arteriography. CT during superselective arteriography was significantly superior to superselective arteriography (P = 0.005). In both studies, manual injection of contrast medium had a significantly higher sensitivity than mechanical injection (P = 0.013). CONCLUSION: To detect small HCC, CT during superselective arteriography showed significantly higher sensitivity than superselective arteriography. Manual injection of contrast medium was significantly superior to mechanical injection. Therefore, manual injection CT during superselective arteriography is recommended for accurately targeted, transarterial chemoembolization therapy.
