Arterial and ductus venosus Doppler in fetuses with absent or reverse end-diastolic flow in the umbilical artery: longitudinal analysis.

OBJECTIVE: To evaluate alterations of arterial and ductus venosus blood flow velocities during deterioration and their interdependence. METHODS: 37 high-risk pregnancies complicated by umbilical absent or reverse end-diastolic flow velocities (AREDV) were monitored by measurement of the pulsatility index of the umbilical artery, middle cerebral artery and ductus venosus waveforms. RESULTS: The mean observation period was 16.5 days. There was a significant change in the pulsatility of waveforms in all vessels over the observation period. Compared with the ductus venosus, pulsatility of waveforms in the middle cerebral artery diverged from the normal range 2.2 weeks earlier. Increase in pulsatility in the umbilical artery was concomitant with venous but not with cerebral flow alterations. In addition, there was a correlation between the increase of venous but not arterial pulsatility and perinatal acidemia. Variability of pulsatility index values of the umbilical artery and the ductus venosus but not of the middle cerebral artery increased towards delivery. CONCLUSIONS: Ductus venosus Doppler allows detection of further deterioration in centralized fetuses with umbilical AREDV. However, striking short-term variability has to be taken into account when considering this parameter.

Feasibility and safety of needle catheter jejunostomy for enteral nutrition in surgically treated severe acute pancreatitis.

BACKGROUND: The role of the gut in the development of septic complications and promising clinical results have led to a shift from the parenteral to the enteral route for nutrition support of patients with acute pancreatitis. In patients undergoing surgery for severe necrotizing pancreatitis, the application of a needle catheter jejunostomy might be useful. However, there is a shortage of clinical data on its feasibility and possible harmful effect. METHODS: Between January 1999 and December 2002, 13 patients were operated for severe acute necrotizing pancreatitis. At the time of surgery, needle catheter jejunostomy was performed using a standard technique. Enteral nutrition was initiated without a strict protocol by feeding small amounts of a standard diet and carefully monitoring patient tolerance. As long as necessary, patients were also fed parenterally in order to achieve target caloric goals. RESULTS: No major tube- or feeding-related complications were observed. A single case of tube dislodgement caused by manipulation during relaparotomy for lavage occurred and was rectified during the same operation. CONCLUSIONS: In patients undergoing surgery for severe acute pancreatitis, needle catheter jejunostomy for long-term enteral nutrition can be applied with no additional risk.

The spatial and temporal disappearance of different oral rabies vaccine baits.

Bait disappearance can give valuable information for the assessment of oral vaccination campaigns of foxes against rabies. In this study, the spatial and temporal disappearance of three different vaccine baits under almost identical conditions was investigated. In the study area, 350 baits were placed at previously marked positions during two different periods; late autumn and early spring. The distribution of baits was in accordance with the method as recommended by the European Union; a density of 20 baits per km2 along flight lines 500m apart. Bait disappearance was checked 1, 3, 5 and 7 days after distribution. At least 80% of the baits had disappeared within one week after distribution. No difference in bait disappearance was observed between the two selected periods. However, a significant higher bait disappearance was observed in forested areas when compared to open agricultural areas. Furthermore, the differences in bait disappearance between the three type of baits tested were relatively small and not significant.
