[Three-dimensional finite analysis of the application of attachment for restoration of unilateral maxillary defect].

PURPOSE: To study the stress of the application of attachment for restoration of unilateral maxillary defect. METHODS: Using the 3-D FE model of the unilateral maxillary defect, two different 3-D FE models of the application of attachment framework for restoration of different unilateral maxillary defects (attachment on the mesial side of the central incisor or canine) were designed and established. RESULTS: The area of stress concentration of the two prostheses were on the palatal base near the position of the anterior teeth. The stress was on the posterior teeth of the healthy side. When the attachment was on the mesial side of the central incisor, the stress of the mesial surface of the central incisor was 1.306MPa, and the stress concentration was at the junction of the attachment and the lingual guide plane. When the attachment was on the mesial of the canine, the stress of the mesial surface of the canine was 0.797MPa, and stress concentration was at the corner of the front and bottom of the lingual guide plane. CONCLUSIONS: The application of attachment for restoration of unilateral maxillary defect meets the biomechanical demands without increasing the stress load on the abutment. And further study of the design of the attachment framework should be done.

[The construction and analysis of three-dimensional finite element model of human unilateral maxillary defect].

PURPOSE: To built a three-dimensional finite element model of the common unilateral maxillary defect, and to study the stress of the traditional maxillofacial prostheses for unilateral maxillary defect. METHODS: The three-dimensional finite element model of unilateral maxillary defect and maxillofacial obturator prosthesis were constructed by means of spiral CT scanning, digital image transfer, self-developed software MedGraphics, UG engineering design program and ANASYS software. Then 3-D finite element stress analysis was carried out, for the prosthesis. RESULTS: The three-finite element models of the unilateral maxillary defect and maxillofacial obturator prosthesis were established. In the traditional obturator prosthesis, the stress was on the posterior teeth of the healthy side; the area of stress concentration was on the palatal base near the position of the anterior teeth. CONCLUSIONS: The three-dimensional finite element models of the unilateral maxillary defect and maxillofacial obturator prosthesis have good similarity in geometry and biology. The obturator prosthesis is in accorded with the biomechanical demands.

STS gene in a pedigree with X-linked ichthyosis.

To investigate the gene mutation in a pedigree with X-linked ichthyosis (XLI) and to explore the relationship between the mutation and its clinical manifestations, genomic DNA of affected members, the normal member of the pedigree and 50 unrelated normal members was extracted with a whole blood genomic DNA extraction kit and the DNA was used as a template for the polymerase chain reaction (PCR)-mediated amplification of exon 1 and exon 10 of the STS gene. hHb6 (human hair basic keratin) gene was used as the internal control. Our results showed that the STS gene was deleted in affected members in the pedigree with X-linked ichthyosis. The normal member of the pedigree and 50 unrelated normal members had no such deletion. The proband and his mother had products in the internal control after PCR amplification. The blank control had no product. It is concluded that deletion of the STS gene existed in this pedigree with X-linked ichthyosis, and it is responsible for the unique skin lesions of X-linked ichthyosis.

[Retention and rehabilitation of complex maxillofacial defects based on bar-clip and stud attachment].

PURPOSE: To evaluate the application of precise attachment in restoration of complicated maxillofacial defect. METHODS: A framework with bar-clip and stud attachment was designed and fabricated. Maxillofacial prosthesis with this framework was applied to treat a patient with defects of nose, entire upper lip and the corresponding maxillofacial areas. The retention force was tested with Universal Testing Device. RESULTS: After treatment, the patient got a better appearance, normal function of mastication and clear pronunciation. The dislodgement test showed the retention force was 40N. CONCLUSION: Application of precise attachment in complicated maxillofacial defects can get satisfactory result.

[Fabricating facial prostheses using CAD/CAM and rapid prototyping technique].

At present, the treatments of hemifacial microsomia such as the missing of ear and eye still rely on the skill of technician to make the wax model of contralateral apparatus of patient in China. In this paper, CAD/CAM and rapid prototyping (RP) techniques are integrated to successfully create a silastic prosthesis by using the patient's data of CT or laser scanning. The clinical results suggest that a high accuracy has been achieved in shape, size, and protrusion of the facial prostheses, which indicates that the application of RP techniques in conjunction with CAD/CAM is a suitable approach for fabricating facial prosthesis.

[Investigation of steroid sulfatase gene in two pedigrees with X-linked ichthyosis].

OBJECTIVE: To investigate the gene mutation in two pedigrees with X-linked ichthyosis (XLI) and explore the relationship between the mutation and clinical manifestations. METHODS: Genomic DNA of the affected and normal members of the pedigrees and 50 unrelated normal subjects from different regions was extracted with a whole blood genomic DNA extraction kit for use of the template for PCR amplification of exon 1, exon 2 and exon 10 of the steroid sulfatase (STS) gene. RESULTS: The STS gene was partially deleted in the affected members in the pedigrees with XLI, leaving only exon 1 but not the other exons. The normal member of the pedigree and 50 unrelated normal subjects had no such deletion. CONCLUSION: Partial deletion of the STS gene exists in the two pedigrees with XLI, which is responsible for pathological skin changes characteristic of XLI.

[The research on 3-D maxillofacial soft tissue reconstruction by spiral CT].

PURPOSE: To evaluate the role of spiral CT in 3-D reconstruction of maxillofacial soft tissues. METHODS: Spiral CT was given to a patient who had his right ear absent. The data was recorded in DICOM format. Digital image was reconstructed on GE advantage workstation in SSD way. RESULTS: Spiral CT can quickly and precisely scan and reconstruct the whole head, the reconstructed images of facial soft tissues and apparatus were very clear, but the site of pupil can not display. CONCLUSION: Except for the pupils, spiral CT is a useful way for maxillo-facial soft tissue reconstruction. It can be widely used in hospitals.

[Evaluation of the speech outcomes in patients with unilateral maxillary defect rehabilitated with maxillary obturator prosthesis].

PURPOSE: Evaluation of the outcomes in 15 patients with unilateral maxillary defect before and after maxillofacial obturator prosthesis. METHODS: 15 patients with unilateral maxillary defect were included in this study, who received obturator prosthesis for maxillary rehabilitation. The pronunciation of the examined phonetics such as /a/, /o/, /e/, /i/, /u/ were transferred into CSL4400 before and after treatment. The mean value of the first, second, third formant were measured. Paired t test of SPSS11.0 was used for statistical analysis. RESULTS: There was statistically significant difference on F2 and F3 before and after treatment (P<0.05). After treatment the value of F3 was increased significantly. CONCLUSIONS: The soft palate was reconstructed after obturator treatment. The space between the nasal and oral cavity was sealed,through which a similar normal oral cavity was obtained. Except F1, F2 and F3 of all the vowels changed after treatment. Maxillofacial prosthesis can improve the speech function of patients with unilateral maxillary defect effectively.

[The research on maxillofacial soft tissue reconstruction by 3-D laser scanning].

PURPOSE: To evaluate the role of 3-D laser scanning on reconstruction of maxillofacial soft tissues. METHODS: 3-D laser scan was given to a patient who had his right ear absent and reconstruct the image. RESULTS: 3-D laser scanning can reconstruct facial soft tissue quickly and precisely, but the image of the ear was not very clear. CONCLUSION: In the living, laser scanning can reconstruct all the maxillofacial soft tissues except for the ear. It can be widely used in clinic and research.

[Research and development for the CAD system of maxillofacial prosthesis].

OBJECTIVE: To establish a CAD system of maxillofacial prostheses. METHODS: The maxillofacial prosthesis CAD system, which included mirror technique, boolean operation and use of FreeForm software, was introduced in detail by use of two clinical cases of auricular and orbital prostheses. RESULTS: The CAD system of maxillofacial prosthesis was successfully established. The 3-D digital images of defected sides of two cases were quickly and conveniently obtained, which were symmetrical and harmoniously match to the deficient sides. CONCLUSION: The established CAD technique is a useful system in maxillofacial area. It may be applied in clinic in future.

[Using laser scanning to 3-D reconstruct an ear model].

OBJECTIVE: The purpose of the study was to get the ear data from a laser scanning so that a. STL file can be promoted though which a 3-d ear reconstruction can be held. METHODS: A laser scanning manufactured by France was used to get the data of an auricular model, then the data was turned into a. STL file so that a 3-d image of ear was got. RESULTS: The result shows that a 3-d ear reconstructed image can be got quickly and precisely. The digital image shows that the dimension, shape and long axis of the ear is really similar to the ear model. CONCLUSIONS: This study suggests that laser scanning is helpful and effective to get the data of ear. Through this method, it makes it possible to be ready for the technique of rapid prototyping and provide a new, convenient, effective and safe method to auricular prosthesis.

[The analysis of masticatory efficiency after maxillofacial prosthetic treatment for unilateral maxillary defect].

OBJECTIVE: To evaluate the clinic results of maxillofacial prosthetic treatment for unilateral maxillary defect. METHODS: Masticatory efficiency was tested before and after maxillofacial prosthetic treatment for unilateral maxillary defect. RESULTS: There was significant difference between one week, one month, three months after treatment and before treatment(F = 0.635, P = 0.002). Masticatory efficiency was increased by 59.98% after one month. CONCLUSION: Maxillofacial prosthetic treatment can significantly increase the masticatory efficiency of unilateral maxillary defect. It remains stable one month later.

[An experiment study on repairing residual dental root with a horn-shaped orifice by different materials].

When a traditional method is used to repair residual dental root with horn-shaped orifice, this kind of dental root often has to be extracted since the success rate in clinic is very low. The main reason is that the stress often concentrates on the neck and root apex, therefore the wall at the orifice is too thin to bear this kind of pressure. A new repairing method is proposed trying to preserve the residual dental root with horn-shaped orifice. Then compare the anti-fracture ability of this kind of tooth with that of no repairing. The results show that it is very effective to prevent the fracture of residual dental root repaired by chemical hard synthetic resin followed by making the post and core. However, there is no obvious difference between non-repaired and residual dental root groups repaired by traditional glass ionic adhesives. The results also provide the great possibility to preserve the residual dental root with horn-shaped orifice in clinic.

[The basic research on fabricating an ear model by means of rapid prototyping (FDM)].

OBJECTIVE: The purpose of the study was to reconstruct a 3-D auricular model. METHODS: Laser scanning was used to get the data of an auricular model, Fused Deposition Manufacturing (one of rapid prototyping technique) was chosen to fabricate an ear cast. The differences between the two models were compared. RESULTS: The result showed that the reconstructed model was precise. CONCLUSION: This study suggests that the technique of rapid prototyping is precise, convenient, effective and safe to auricular prosthesis.

Design and fabrication of auricular prostheses by CAD/CAM system.

PURPOSE: The purpose of this report is to describe a new technique for fabricating auricular prostheses by a computer-aided design/manufacturing (CAD/CAM) system. MATERIALS AND METHODS: A spiral CT was performed on a patient who had a right ear defect resulting from an accident. A 3-D image was reconstructed with the CT data. Through image ware, the image of the normal ear was extracted, mirrored, and Boolean operated with the image of the deformed side of the face. It was well modified and smoothed in the FreeForm model system so that it could precisely adapt to the deficient side of the face. A paper model ear was manufactured by rapid prototyping from the digitized data. Finally, silicone was poured into a silicone mold from the paper ear to create a silicone auricular prosthesis. RESULTS: The dimension, shape, and anatomic contour of the auricular prosthesis were quite similar to those of the normal ear and precisely matched the deformed area. CONCLUSION: The CAD/CAM system for creating auricular prostheses appears to be a practical technique.
