[Retinal changes in patients with familial adenomatous polyposis].

PURPOSE: The aim of study was to evaluate retinal changes in patients diagnosed with familial adenomatous polyposis (FAP) and in patients with family history of FAP. MATERIAL AND METHODS: The study was conducted on 51 patients diagnosed with FAP and 35 subjects with family history of FAP. RESULTS: In 44 patients diagnosed with FAP (86.2%), typical pigmentation of fundus lesions were observed, in 1 patient atypical fundus changes were evident. However, in the group of patients with family history of FAP, the presence of typical fundus lesions was observed in 9 patients (25.7%). In the period of at least 6 month observation, no changes in fundus lesions were present in both groups. CONCLUSIONS: The occurrence of retinal changes typical for FAP, especially in patients with family history of the disease, should imply the higher risk of FAP.

Expression of keratinocyte growth factor and its receptor in adaptive changes of ileorectal pouch mucosa.

OBJECTIVE: Total proctocolectomy with formation of an ileo-anal pouch is a well-established surgical procedure for patients with ulcerative colitis (UC) or familiar adenomatous polyposis (FAP). The pouch mucosa undergoes adaptive changes, with inflammation of the ileal reservoir being the most common complication. The aetiology is unknown. The keratinocyte growth factor (KGF) has not only been shown to promote intestinal wound healing and re-epithelialization but also to have some immunomodulatory properties. This study was designed to investigate a putative involvement of KGF in observed histomorphological changes in the pouch mucosa. MATERIALS AND METHODS: Multiple biopsies were obtained from age-matched and sex-matched patients. Biopsies were stained with H&E and scored for inflammation and morphological changes. mRNA expression levels of KGF and KGF-receptor (KGFR) were determined using competitive RT-PCR, protein expression and phosphorylation was analyzed by Western blotting. KGF and KGFR were localized in tissue samples by immunohistochemistry. RESULTS: Expression of KGF and KGFR was significantly increased in inflamed and adapting mucosa. Patterns of expression did not significantly differ in pouch mucosa from UC or FAP patients. Protein expression correlated with the mRNA results and KGFR was shown to be activated in adapting pouch mucosa. KGF was detected on subepithelial cells, mainly on fibroblasts, whereas expression of KGFR was restricted to epithelial cells. CONCLUSION: Expression of KGF and KGFR is significantly increased in the pouch mucosa, suggesting an involvement of this growth factor in tissue repair and adaptive changes. Topical application of KGF might alleviate the inflammatory and promote the adaptive process in the ileo-anal pouch mucosa.

Effects of gastrectomy or colectomy on liver metabolism and liver morphology in an experimental rat model.

BACKGROUND: It is sometimes necessary to translocate part of the gastrointestinal tract into the site of the organ earlier resected. This occurs after total esophageal, stomach or colon resection, which can lead to disturbances in the anatomical and functional balance of the gastrointestinal tract and the body as a whole. The aim of this study was to evaluate the effects of different resective surgical procedures performed on the proximal and distal GI tract in terms of total body weight, selected liver function tests, and ultrastructural changes in the liver in laboratory rats. MATERIAL/METHODS: The total of 57 resective procedures consisted of 31 total gastric and 26 total colon resections. The control group consisted of 12 animals on which only laparotomy was performed. Body weight, stool character, and the general condition of the animals were evaluated from the time of the operation. Twenty-six blood samples were collected for analysis of hemoglobin, total plasma protein, and liver aminotransferases levels. Thirty liver tissue samples from the 3 groups were taken and examined. RESULTS: We observed statistically significant decreases in total body weight, hemoglobin, and total plasma protein levels in the group where reconstructive procedures following gastrectomy were performed. In the group in which reconstructive procedures followed colectomy, a statistically significant decrease in total body weight was observed. Aminotransferase level changes were neither characteristic nor significantly different in any of the three groups involved in the investigation. CONCLUSIONS: Abnormalities of liver structure were observed at the ultrastructural level. Any correlation between these changes, general condition, and selected biochemical parameters requires further controlled clinical and experimental studies.

Diagnostic difficulties in pouchitis--experience from a single institution.

The aim of the study was to analyze the clinical, endoscopic and histological signs of pouchitis in patients operated on because of ulcerative colitis (UC) and familial adenomatous polyposis (FAP). Between 1984 and 2002, the Department of Surgery carried out 218 restorative proctocolectomies in patients with UC and 120 in FAP patients. The follow-up to assess the intensity of inflammatory changes included 110 patients: 72 UC and 38 FAP. During the endoscopic examination, samples were taken for routine histological examinations. The histological assessment was based on the so-called Moskwitz's Histological System. Acute pouchitis is diagnosed if the score is > or =4, and chronic condition--if the score is > or =4. We used a modified PDAI scale to analyze the symptoms, taking into account the microscopically detectable histological features of chronic pouchitis. The clinical symptoms of pouchitis were revealed in 28 patients (25.5%), endoscopic symptoms in 36 patients (32.7%), and histological parameters in 48 patients (28-58.3% with > or =4 score of acute pouchitis and 20-41.7% with > or =4 score of chronic pouchitis). The signs of acute and chronic pouchitis were evident in endoscopy and histology, with no clinical features observed in many cases. Patients with symptoms of chronic pouchitis require a continuous follow-up.
