[Wegener's granulomatosis--diagnostic problem].

UNLABELLED: Wegener's granulomatosis there is a small--and middle--vessels vasculitis. The pathomorphological diagnostic criteria is known as Wegener's triad: 1) necrotizing granlomatous inflammation of upper and/or lower respiratory tract, 2) systemic or focal necrotizing vasculitis involving arteries and vein, and 3) focal segmental necrotizing crescentic gromerulonephritis. According to the current theory of pathogenesis of Wegener's granulomatosis, Staphylococcus aureus is involved. The rise in ANCA level during vascular inflammation is very important for monitoring disease. The main localization of Wagener's granulomatosis is in lung, nasal sinusites, nose, kidneys and nasal pharynx. MATERIAL AND METHODS: The history of 60-years old female patient who was being treated in Department of Otolaryngology for chronic bilateral otitis media for many years is presented. During hospitalization the sinusites, rhinitis and renal failure additionally were found. The CT sinuses and head, histopatological and ANCA exam in patient were performed. RESULTS: Based on clinical, physical, additional exam Wegener's granulomatosis was diagnosed. CONCLUSIONS: 1) Surgical procedure is not indicated method of treatment of Wegener's granulomatosis, because it can increase the pathologic process. 2) The prognosis of Wegener's granulomatosis depends mainly on the ability to diagnose the early disease and the aplication of adequate treatment.

[Evaluation of intensity of angiogenesis in granulation tissue in chronic otitis media--preliminary report].

In chronic otitis media molecular pathogenetic mechanisms are still unknown, however, angiogenesis may play a role. The aim of the study was to determine the intensity of angiogenesis in granulation tissue in chronic otitis media of different clinical course. Twenty six granulation tissue specimens (twenty two--from chronic otitis media, four--from prolongated otitis externa) taken during surgery were analyzed. The angiogenesis (microvessel density) was measured in paraffin-embedded tissue by an immunohistochemical method, by staining for endothelial cells with a monoclonal antibody against CD 34. The presence of CD 34 was found in all specimens. The expression was more intense in tissue samples from the group with good clinical course (good healing and without recurrence) than those in the group with poor healing and recurrence (mean number of dots for mm2 589,2 vs 533,3, respectively) and from the group without bacterial infection by Pseudomonas aeruginosa than those with this infection (mean number of dots for mm2 645,5 vs 440,8, respectively), but differences were not significant. In conclusion it is suggested that angiogenesis may contribute to different clinical course of chronic otitis media.

[Malignant neoplasms of external and middle ear].

INTRODUCTION: Neoplasms of external and middle ear are rare, which cause several problems in diagnosis and therapy. The purpose of the study was to analyze retrospectively patients with malignant neoplasms of the ear. METHODOLOGY: The study was carried out on 53 patients treated for malignant ear neoplasms in single institution during 25 years (1978-2002). RESULTS: The most frequent neoplasm was squamous cell carcinoma--36 cases (67.9%), then basal cell carcinoma--9 cases (16.9%). Neoplasm primarily involved auricle in 26 patients (49.1%), external auditory canal in 15 patients (28.3%) and middle ear in 12 patients (22.6%). They were classified according to Stell and McCormick (1985) as follows: stage I--29 tumours (54.7%), stage T2--15 tumours (28.3%), stage T3--9 tumours (17%). Facial nerve paresis occurred in 15 patients (28.3%). The most often treatment modality was surgery--32 persons (60.4%), then surgery followed by radiotherapy--17 persons (32.1%). The characteristics of neoplasms related to the site of location were described. The difficulties in precise histopathologic diagnosis and extent of disease were pointed out. RESULTS: Neoplasms of external and middle ear constitute a group of various histopathological and clinical tumours, which differ in diagnostic difficulties, treatment and prognosis. A diagnosis was often made in advanced stages of neoplasms, especially for middle ear tumours, that diminished a possibility of effective treatment.

[Impact of tinnitus on quality of life in the patients' self-assessment].

Tinnitus may have multidirectional impact on patients and influence unfavourably their quality of life and psychology. The aim of the study was to investigate the aetiology, audiological parameters, and tinnitus characteristics and to evaluate the impact of tinnitus on patients life and emotional status by using the self-report questionnaire. A group of 157 consecutive patients referred to the Otiatry Clinic Medical University in Lodz were enrolled in the study. It was found that tinnitus negatively impact on 2/3 of patients and 1/3 of them consider it as annoying. The most frequent aetiological factor was noise exposure (40.8%). There was no correlation between tinnitus loudness and degree of hearing loss on the one hand and self-perceived handicap and annoyance of tinnitus on the other. It was stated that younger age and higher tinnitus pitch-match frequencies may influence patients' quality of life.

[Impact of tinnitus on emotional state of patients according to General Health Questionnaire by Goldberg].

The aim of the study was to estimate the emotional state and the prediction of the emotional disorder of the patients who suffer from tinnitus. The General Health Questionnaire by Goldberg (GHQ) was used to this study. The study was performed in 129 patients aged 20-78 years who had been suffering from tinnitus for more than 3 months. Based on the results of the examination the patients were divided into two groups. The first group consisted of 58 patients without any emotional disorders. The second group consisted of 71 patients who would be supposed to suffer from the emotional disorders in the future. There were no statistical significant differences in the gender, age and the result of the audiometric tests between the groups of the patients. An analysis showed that the results of the GHQ were the statistical significant higher in the second group especially in the part where the symptoms of a depression were estimated.

[Neurilemmoma of the neck--case report].

Neurilemmoma is a mild tumor growing from the Schwann's cells of the external part of the nerve trunk. Authors, in their works describe a case of a 32-year-old man with a neck tumor on its right side. The tumor was identified as the neurilemmoma on the grounds of a postoperative histopathological examination. Thin-needle biopsy that was made twice during the history of the patient that lasted two years did not reveal the character of the tumor.

[Glandular neoplasms of the external auditory canal--clinical and morphologic observations].

In the article cases of glandular neoplasms of the external auditory canal were presented. Ceruminous adenoma was diagnosed in three persons, adenoid cystic carcinoma--in three patients and ceruminous adenocarcinoma in one patient. All of them were treated surgically or by combined method with radiotherapy. Difficulties with exact histopathological diagnosis and with choice of a method of treatment were stressed. Classification of the glandular neoplasms of the external auditory canal was also presented underlying a histopathological and clinical diversity of these tumors, which had previously been incorrectly qualified in one group ceruminoma.

[Prognostic factors in sudden deafness].

Sudden sensorineural hearing loss is still a diagnostic and therapeutic dilemma and is very difficult to predict recovery in it. Different factors may influence a prognosis like severity of hearing loss, duration of symptoms before treatment, presence of vertigo, type of audiogram and age of patients. The aim of the study was an evaluation of the hearing improvement in sudden deafness in relation to some of these elements. A retrospective analysis of 119 patients treated for sudden sensorineural hearing loss during 10 years was done. Clinical otolaryngological, neurological and ophthalmological examination, audiologic and ENG tests were carried out. Hearing improvement was obtained in 51 patients (43%). Hearing recovery was recorded in 38 patients (66%) in whom duration of disease before treatment was no longer than 7 days, in 9 patients (25%) with a period 8-14 days and in 4 patients (16%)--with period 15-30 days (66% vs 25% vs 16%, p < 0.001, 25% vs 16% p = 0.39). Hearing improvement was more frequent in patients with initially mild (51.6%) than severe (38.7%) and profound hearing loss (25%) (51.5% vs 25%, p < 0.05). Hearing recovery was observed in 18 patients (33.3%) with vertigo and in 33 patients (50.8%) without vertigo (p = 0.056). In analysis of age (five groups: until 30 years, 31-40 years, 41-50 years, 51-60 years, over 60 years) in comparison with hearing recovery it was not found any significance (45.5% vs 45.8% vs 46.4% vs 37% vs 38.9%, p = 0.94). It was stated that in patients with sudden deafness duration of the disease before treatment and level of hearing loss may significantly influence an outcome, also vertigo may worsen a recovery, contrary age of the patients does not seem as important prognostic factor.

[Angiosarcoma of the temporal bone].

A case of 55-yrs old female was reported, in whom angiosarcoma of temporal bone had been diagnosed. A mode of treatment was surgery including partial petrosectomy and labyrinthectomy. Radiotherapy was not given because of a possibility of further cerebrospinal liquid leakage after dura mater rupture, which was seen during first operation. Local recurrence had developed about 2 months after surgery and a patient died 10 months after the time of diagnosis. It was stressed that angiosarcoma is a very rare neoplasm and in our case it was presented as few--symptomatic lesion despite of it's high extent in temporal bone. A review of literature concerning angiosarcoma was also reported.
