The underlying mechanism of search asymmetry is still unknown. Many computational models postulate top-down selection of target-defining features as a crucial factor. This feature selection account implies, and other theories implicitly assume, that predefined target identity is necessary for search asymmetry. The authors tested the validity of the feature selection account using a singleton search task without a predefined target. Participants conducted a target-defined and a singleton search task with a circle (O) and a circle with a vertical bar (Q). Search asymmetry was observed in both tasks with almost identical magnitude. The results were not due to trial-by-trial feature selection, because search asymmetry persisted even when the target was completely unpredictable. Asymmetry in the singleton search was also observed with more complex stimuli, Kanji characters. These results suggest that feature selection is not necessary for search asymmetry, and they impose important constraints on current visual search theories.
A 32-year-old female patient developed erythema nodosum-like lesions at needle prick sites after acupuncture therapy. Over the next few days, she developed similar new lesions over the extremities, trunk and face along with flu-like symptoms. There were neither genital ulcerations nor eye lesions. A skin biopsy specimen from an extremity lesion showed the characteristic findings of erythema nodosum. Treatment with oral potassium iodide at a dose of 750 mg/day was effective, and there has not been any recurrence to date. We diagnosed this case as erythema nodosum induced by a synergism between acupuncture therapy and a flu-like infection.

Secondary bile acids have been implicated as an important etiological factor in colorectal cancer. We investigated the effects of ursodeoxycholic acid (UDCA) and deoxycholic acid (DCA) on the growth and cytotoxicity in HT29 human colonic adenocarcinoma cells. Proliferation assay, cell cycle analysis and cell death characterization by bile acids were performed. Both UDCA and DCA reduced their proliferation rate of HT29 over 48 h in a concentration- and time-dependent manner compared with control cultures. In terms of cell cycle effects, however, UDCA induced G2/M arrest, while DCA induced G1 arrest in a concentration- and time-dependent manner. As for the effects of each bile acid on cell toxicity, UDCA induced early apoptosis and DCA induced both early apoptosis and necrosis. Bile acids play an important role in regulating cell survival and cell death in colon adenocarcinoma cells.
The inhibitors of apoptosis (IAPs) family regulate apoptosis by preventing the action of the central execution phase, and function as mediators and regulators of the anti-apoptotic activity of the v-Rel and NF-kappaB transcription factor families. The targeting of IAPs may be a promising strategy, but it is not well elucidated in human hepatocellular carcinomas (HCCs). We have therefore investigated the effects of the down-regulation of IAPs (XIAP or survivin) on the TNF-related apoptosis-inducing ligand (TRAIL) and chemotherapeutic agents that induced apoptosis in human HCC cells. To inhibit the IAPs gene expression, we designed small interfering RNA (siRNA) against the X-chromosome-linked IAP (XIAP) or survivin and investigated their efficacy in the suppression of the XIAP or survivin expression in two HCC cells (SK-Hep1 and HLE), and their consequent antitumor potential. We found that the designed siRNAs against the XIAP and survivin downregulated the protein expression of respective genes by almost 50%. The suppression of IAPs resulted in a significant decrease in procaspase-3 levels, especially by suppression of the XIAP. The apoptosis cell count was small in cells transfected with control siRNA and siRNA against the XIAP or survivin, but after treatment with 10 ng/ml of TRAIL, the apoptosis cells increased 2-3 times by the suppression of IAPs as control. The cytotoxicity of doxorubicin and camptothecin was augmented by the suppression of the XIAP in SK-Hep1 cells, whereas the suppression of survivin did not affect cytotoxicity. In conclusion, downregulation of the XIAP or survivin enhances cell death by TRAIL and increases sensitivity against some chemotherapeutic agents in HCC cells. In particular, the XIAP may be a potential target to increase therapeutic sensitivity.
Palliative care has been provided in our sub-acute hospital ward and multi professional conferences have also taken place in order to enhance care. In this paper, we present a 72-year-old female who has suffered from relapse of lung cancer and was able to return home because of an execution of multi-professional conferences. We also present considerations that were carried out in multi-professional conferences. We found some of the important factors for carrying an efficient conference are advance preparations in ahead, punctuality of the conference, a conference place that is kept in the ward, coping with nurse calls and participation of nutritionists. A timely conference, sharing information among the team and the education about a dying process are some of the future subjects.

The aim of this study is to evaluate the discriminative properties of the Multidimensional Quality of Life Questionnaire for HIV infection (MQoL-HIV) and to determine those factors contributing to the health-related quality of life (HRQoL) of HIV-1 infected persons living in Japan. The MQoL-HIV, the Nottingham Health Profile (NHP) as a generic instrument, and the Center for Epidemiologic Studies-Depression Scale (CES-D) as a psychological measure were administered in 375 patients as a multiple-centre study. The score distribution of the MQoL-HIV showed a unimodal distribution. The Cronbach's alpha coefficient scored more than 0.7 in seven out of 10 domains, but was low in both the physical functioning and sexual functioning domains. There was a strong correlation between the CES-D and MQoL-HIV index scores (R-0.73). Relatively high coefficient values were found between psychiatric and nervous symptoms and the index score (R=-0.60). In total, the MQoL-HIV may possess discriminative properties.

The objective of this study was to elucidate the utility of ion-trap GC/MS/MS for the analysis of pesticides in extracted matrices from various agricultural products. Identification and quantitative analysis of pesticides in matrices were performed by quadrupole GC/MS and ion-trap GC/MS/MS. Chlorpyrifos was added to the matrix of spinach, soybean in the pod or corn, and aldrin, dieldrin, endrin, alpha-BHC, beta-BHC, gamma-BHC, delta-BHC, p,p'-DDD, p,p'-DDE, o,p'-DDT and p,p'-DDT were added to each matrix of green tea, black tea or oolong tea. Although most of the pesticides in the matrix could not be determined by quadrupole GC/MS-Scan analysis at 0.1 microgram/mL, every pesticide was identified from the mass spectrum using ion-trap GC/MS/MS at the same concentration. The quantitation limit of every pesticide in each matrix by ion-trap GC/MS/MS analysis was higher than that by GC/MS-SIM analysis. The calibration curves obtained by GC/MS/MS were linear in the range of 0.01-0.25 microgram/mL of each pesticide. The recoveries of each pesticide from four kinds of samples spiked at the levels of 0.01 ppm to 0.02 ppm in extracts were 61.2-138.3% with SD values in the range from 1.2 to 15.4%. This study revealed that ion-trap GC/MS/MS was useful for the identification and quantitative analysis of low-level pesticides residues in matrices of agricultural products.
In the present study, we quantitatively investigated the relationship between the signal intensity in a vessel and the duration of contrast enhancement as well as scan timing in 3D contrast-enhanced MR angiography using an elliptical centric phase-encoding technique. A tube phantom filled with Gd-DTPA, acting as a vessel, was taken out from the field of view during data acquisition, by using the "pause" function of our MR scanner (GE Signa, 1.5 Tesla), thereby simulating the presence and absence of a vessel. The shortening of the duration of enhancement corresponds to the delay of scan timing from the optimal point in the phase-encoding of the centric-ordering system. The signal intensity in a vessel (1-5 mm in diameter) decreased as the duration of enhancement became shorter and the diameter of the vessel decreased. When the number of partitions was 16 or 32 in a 128-mm-thick slab, the signal intensity obtained by the elliptical centric phase-encoding technique was almost the same as that obtained by the conventional centric phase-encoding technique. However, when the number of partitions was increased (64-124), and if the duration of enhancement was short, the signal intensity obtained by the elliptical centric phase-encoding technique was higher than that obtained by the conventional centric phase-encoding technique. In conclusion, in terms of the duration of enhancement and the delay of scan timing, the elliptical centric phase-encoding technique is superior to the conventional centric phase-encoding technique when the number of partitions in a slab for 3D MR angiography is increased.
OBJECTIVE: The goal of this study was to determine the frequencies of allelic variants of CYP2B6and CYP3A5 in the Japanese population. METHODS: Genotyping of CYP2B6 (*2, *3, *4, *5, *6, and *7) and CYP3A5 ( *2, *3, *4, *5, and *6) was carried out in 265 unrelated Japanese subjects by polymerase chain reaction (PCR), restriction fragment length polymorphism and allele-specific, real-time PCR assays. RESULTS: Allele frequencies for CYP2B6*2, *3, *4, *5, *6, and *7 in 256 Japanese subjects were 0.047, 0, 0.093, 0.011, 0.164, and 0, respectively. Ethnic variation in allele frequencies relative to that in Caucasian subjects was observed for CYP2B6*4 (0.093 vs 0.040), *5 (0.011 vs 0.109), *6 (0.164 vs 0.256), and *7 (0 vs 0.030). Allele frequencies for CYP3A5*2, *3, *4, *5, and *6 in 265 Japanese subjects were 0, 0.740, 0, 0.004, and 0, respectively. The frequency of the CYP3A5*1 allele is 2.8 times higher in Japanese than in Caucasians. CONCLUSIONS: Our results contribute to a better understanding of the molecular basis of ethnic differences in drug response, which may help to improve individualization of drug therapy and offer a preliminary basis for more rational use of drugs that are substrates for CYP2B6 and CYP3A5 in the Japanese population.
OBJECTIVES: The routine detection of polymorphisms affecting drug sensitivity in patients before treatment is important in the identification of drug responders or nonresponders, and patients at increased risk of drug toxicity. Here, we present an assay for the simultaneous and rapid genotyping of five polymorphisms influencing drug sensitivity. DESIGN AND METHODS: We used a hybridization probe assay on the LightCycler to detect five single nucleotide polymorphisms (SNPs): INPP1 (973C>A), ADRB2 (R16G and Q27E), HTR2A (102T>C), and mtDNA (1555A>G). Two fluorescent labeled hybridization probes were designed for the simultaneous detection of the five SNPs and detection of the variant alleles was performed by melting curve analysis. RESULTS: All five SNPs were detected with a single thermocycle protocol within 40 min. The genotypes determined in this assay were identical to those obtained with conventional PCR and restriction fragment length polymorphism analysis. CONCLUSIONS: To our knowledge, we report here for the first time a method for simultaneous detection of five SNPs, on a single thermocycle protocol by the LightCycler. This method is rapid, highly sensitive, and high-throughput, and is thus suitable for routine clinical use and large-scale epidemiologic studies.
The prick test is a useful skin test for diagnosing immediate hypersensitivity response. Sometimes it is necessary to perform prick tests on patients who have already received antihistamines or corticosteroids. It is, however, occasionally uncertain whether the results of prick tests are reliable. In this study, the inhibitory effects of prednisolone (10 mg/day) and fexofenadine (120 mg/day) on the response to prick tests induced with histamine and compound 48/80 were examined. During a 7-day-continual drug administration, prick tests were performed 8 h after drug administration. The inhibitory effects of fexofenadine on both the histamine- and compound 48/80-induced skin responses were exhibited on the 1st day and persisted from 24 to 36 h after the final administration. The histamine-induced wheal responses were not inhibited by prednisolone, while the compound 48/80-induced flare and wheal responses were significantly inhibited on the 5th day of drug administration. These responses returned to the baseline level 24 h after the last drug administration. Thus, the results of skin tests performed during administration of antihistamines and corticosteroids should be carefully interpreted.
To investigate the association between NAT2 genotypes and the incidence of isoniazid (INH)-induced adverse reactions, in the hope of identifying a pharmacogenetic approach that could be useful in the prediction and prevention of adverse reactions in Japanese patients, we retrospectively studied the genotypes of NAT2 in 102 Japanese patients treated with INH (without rifampicin co-administration). The subjects were classified into three groups according to their genotypes: rapid-type, intermediate-type, and slow-type. The clinical conditions of the patients were followed-up in order to evaluate the development of any adverse drug reactions (ADRs) and correlate them with patient genotypes. Six out of the 102 patients (5.9%) developed various ADRs following INH treatment. These reactions included nausea/vomiting, fever, visual impairment, and peripheral neuritis. We found a statistically significant difference between the incidence of ADRs and NAT2 genotype. The incidence of ADRs was significantly higher in the slow type than in the other two types, as 5 out of the 6 ADR patients were of the slow-type, and the other one was of the intermediate-type, while no patients of the rapid-type developed any ADRs. The results indicated that the genes coding for slow acetylation were associated with the incidence of serious ADRs following INH treatment. Our findings suggest that determination of NAT2 genotype might be clinically useful in the evaluation of patients at high risk of developing ADRs induced by INH.
In our study of three-dimensional contrast-enhanced MR angiography, we performed a computer simulation to quantitatively investigate vessel visibility according to scan timing. To construct the simulated MR images, we varied the position (scan timing) and range (enhancement-duration) of k-space data assumed to be acquired during contrast enhancement. In the present study, either the sequential or centric phase-encoding order in k(y) and k(z) on k-space was assumed to be used. When scan timing was shifted from the optimal timing, the visibility of thick vessels decreased, and the signal intensity in thin vessels was higher than that in thick vessels. We found that the appropriate setting of scan timing was an important factor in the visibility of thick vessels. Meanwhile, we also noted that extending the enhancement-duration (or shortening the scan time) could increase the visibility of thin vessels. Our results and the simple technique used for simulation are considered to be useful for the study of three-dimensional contrast-enhanced MR angiography.
