[Complications of tension-free vaginal tape procedure: a comparison of the vaginal and abdominal approaches].

OBJECTIVES: The objectives of this clinical study is to demonstrate our experience of the abdominal approach (from the abdominal wall to the vagina) for the tension-free vaginal tape (TVT) operation and to identify factors responsible for complications relating to the puncture. MATERIALS AND METHODS: One hundred and fifty-one cases of patients with stress incontinence who underwent the tension-free vaginal tape operation were analyzed. Among them, 75 cases underwent the TVT procedure with the vaginal approach (from the vagina to the abdominal wall) and the remaining 76 cases underwent the TVT procedure with the abdominal approach (from the abdominal wall to the vagina). The TVT procedure with abdominal approach was performed in almost the same manner as described in the guiding booklet provided by Gynecare. Student's t-test was used to detect the differences in ages and operating times between the abdominal and vaginal approach cases. Chi-square test was used to analyze the difference between complication rates of the two procedures. Multivariate logistic regression analysis was used to determine the relationship between complications related to the puncture and such factors as age, concomitant operation for genital organ prolapses, approach routes and past history of intra-pelvic surgery. RESULTS: Differences between the abdominal and vaginal approach cases as regards age of patients and operating time were not significant. Chi-square test showed that the difference between the complication rates of the two procedures was not significant. Complications relating to the puncture consisting of bladder perforation and intra-pelvic hematoma occurred in nine cases in the vaginal approach group. Since an abdominal guide passed very close to the urinary bladder in one case in the abdominal approach group, we counted this as a bladder perforation case. Seven cases out of these ten cases with complications had a past history of intra-pelvic operation. Multivariate logistic regression analysis showed that the risk factors associated with these complications were the vaginal approach procedure [odds ratio (OR) 11.4, 95%confidence interval (CI) 1.4-96.4] and past history of intra-pelvic surgery [OR8.2, 95%CI 1.9-35.5]. CONCLUSION: The difference between the complication rates of the two procedures was not sig- nificant. The vaginal approach puncture and the past history of intra-pelvic surgeries were the risk factors for the complications relating to the puncture.

[Neurological findings, neurophysiological examinations, and sural nerve biopsy in a case of Friedreich ataxia].

We report on a 19-year-old Russian man with Friedreich ataxia with an expanded GAA repeat. The symptoms include ataxia of the trunk and lower extremities, dysdiadochokinesia of the upper extremities with left side dominance, square wave jerks, dysarthria, decreased muscle tone, areflexia, hypesthesia, decreased vibration sense and weakness in the lower extremities, extensor plantar response, skeletal abnormalies, and hypertrophic cardiomyopathy. Somatosensory Evoked Potentials elicited by median nerve stimulation suggested involvement of the central pathways, including the posterior column with lateral dominance. Sural nerve biopsy showed a marked decrease in large myelinated fibers (120/mm2) and a moderate decrease in small myelinated fibers (1430/mm2) with normal density of unmyelinated fibers. Carbon dioxide laser stimulation of the upper limbs demonstrated "C-fiber component" toward Adelta fibers and a normal component toward C fibers. Immunohistochemical staining of a skin biopsy from the lateral malleolus for protein gene product 9.5 demonstrated a normal density (18/mm) of intraepidermal nerve fibers. To our knowledge, this is the first report using CO2 laser stimulation, skin biopsy, and sural nerve biopsy that unmyelinated fibers are not involved in Friedreich ataxia.

Respiratory response of cell-based sensors to toxicants measured by using pseudo-random signals.

Cell-based sensors using such as Pseudomonas putida and Bacillus subtilis were applied to examine the toxicity of chemicals. Both toxicant and substrate solutions were introduced into the sensor system according to M-series and anti-symmetric M-series pseudo-random binary signals, Xn (n=15, minimal pulse width 4 min, period 60 min) and Yn (n=30, minimal pulse width 4 min, period 120 min). Toxicants such as Ag+, formaldehyde, azide, and hypo-chlorite were used to demonstrate the proposed method. The individual responses of substrate and toxicant were obtained at a time by calculating the cross-correlation function between the input and the output signals. Effects of toxicant on the response of substrate and the response of toxicant itself can be observed at a time, so that the method appears to be useful in studying the behavior of microorganisms in the presence of toxicants.

[Safety and efficacy of a new vessel-sealing device for laparoscopic varicocelectomy].

PURPOSE: To evaluate the safety and efficacy of a new vessel-sealing device (LigaSure) for laparoscopic varicocelectomy. MATERIALS AND METHODS: To evaluate the safty, after elective varicocelectomy, the internal spermatic veins (n = 8) were sealed ex vivo with a 5-mm laparoscopic Maryland-style LigaSure (LigaSure Lap), and bursting pressures were measured. To evaluate the efficacy, a retrospective review of our clinical experience with LigaSure (n = 13) and clip (n = 13) in laparoscopic bilatelal varicocelectomy from June 2000 to October 2002 was performed and certain parameters were abstracted, including operative time, estimated blood loss and perioperative complications. RESULT: In the ex vivo study the mean varicocele vessele diameter was 2.5 +/- 0.8 mm (mean +/- SD) and bursting pressure was 449 +/- 95.2 mmHg. Six were burst in normal vessel walls at the average bursting pressure of 442 mmHg and two in the occlusions at the average bursting pressure of 508 mmHg. The difference was not statistically significant. Reliable vessel sealing was achieved in all the patients. The mean operative time showed significant decrease in the LigaSure group compared with the clip group, 70 +/- 20 minutes versus 117 +/- 27 minutes (p < 0.05). Estimated blood loss was minimal and no perioperative complications occurred in both groups. CONCLUSIONS: Our study suggests that a vessel-sealing device appears to be a safe and feasible alternative to the clip in laparoscopic varicocelectomy. It offers the advantage of reducing operative time.

[Predictive factors associated with successful varicocele repair a study of 139 infertile men with valicocele].

PURPOSE: To determine pretreatment parameters which predict improvements following varicocele repaire in semen quality. MATERIALS AND METHODS: We retrospectively evaluated a total of 139 infertile patients who underwent varicocelectomy from February 1995 to March 2000. A logistic regression analysis was performed to identify parameters associated with improvements in semen quality. Parameters evaluated included varicocele grade, age, testicular volume, serum testosterone, liteinizing hormone (LH), follicle-stimulating hormone (FSH), preoperative sperm density and sperm motility. RESULTS: Of 139 patients 71 (51.0%) improved sperm concentration and 59 (42.4%) improved sperm motility postoperatively. Overall, median sperm density significantly increased from 10 x 10(6)/ml preoperatively to 30 x 10(6)/ml postoperatively. Sperm motility also significantly increased 33% to 45%. In logistic regression analysis, varicocele grade (odds ratio [OR] = 5.7; 95% confidential interval [CI : 1.9-17), FSH level ([OR] = 0.76; [CI]: 0.60-0.96) and sperm motility ([OR] = 1.03; [CI]: 1.0-1.1) were independent predictive factors for improvement in sperm concentration. CONCLUSION: Varicocelectomy improves sperm concentration and motility. Our data suggest that patients with grade 3 varicocele, low serum FSH level and high sperm motility are more likely to benefit from varicocele repair in sperm concentration.

[A case of incomplete distal renal tubular acidosis undergoing repeated treatment by extracorporeal shock-wave lithotripay and transureteral lithotripsy for recurrent urolithiasis].

A 65-year-old woman was referred to our hospital for further examination of recurrent urinary stone formation. She had undergone 49 sessions of extracorporeal shock-wave lithotripay (ESWL) and 3 sessions of transureteral lithotripsy (TUL) under the diagnosis of idiopathic recurrent urolithiasis at another hospital. An excretory urography showed bilateral hydronephrosis and a retrograde urography revealed bilateral lower ureteral stricture. Ammonium chloride loading test demonstrated incomplete distal renal tubular acidosis. Potassium citrate therapy had begun and bilateral nephrostomies were required in order to prevent recurrent urinary tract infection and new stone formation. Literature was reviewed and discussion was made on the ureteral stricture after ESWL and TUL, and on incomplete distal renal tubular acidosis.

[A case of inverted papilloma of the prostatic urethra].

A 59-year-old male with a complaint of gross hematuria was found to have a pedunculated and non-papillary tumor on the prostatic urethra. The tumor was resected transurethrally. The pathological finding was inverted papilloma. Although the majority of the cases of inverted papilloma have been found in the bladder, we reported the 32nd case located on the prostatic urethra in Japan.

Tissue-specific expression of rhamnose-binding lectins in the steelhead trout (Oncorhynchus mykiss).

Tissue-specific expression of three L-rhamnose-binding lectins, named STL1, STL2, and STL3, in the steelhead trout (Oncorhynchus mykiss) was investigated. STL2 and STL3 mRNAs were restricted in the oocytes. In contrast, STL1 mRNA was detected only in the liver. The transcription of STL2 and STL3 started in previtellogenic oocytes. These results showed distinct expression profiles of rhamnose-binding lectins in the fish.

[Leiomyosarcoma and transitional cell carcinoma in the urinary bladder: a case report].

A 74-year-old man was admitted for asymptomatic macroscopic hematuria. He had undergone transurethral resection of bladder tumor (TURBT) due to transitional cell carcinoma 30 years ago. Pelvic CT showed two invasive bladder tumors. A 5 cm tumor was on the dome and a 1 cm tumor was on the left lateral wall. TURBT was performed. Pathological examination revealed that they were leiomyosarcoma and transitional cell carcinoma. Radical cystectomy plus bilateral cutaneousureterostomy was performed. He died of lung metastases and local recurrence after 6 months.
