Primary ovarian tumor undergoing surgical management during pregnancy.

The aim of this study was to evaluate the preoperative symptoms of patients with primary ovarian tumors undergoing surgery during pregnancy and non-pregnancy. We retrospectively analyzed the medical records of 71 pregnant patients who underwent surgery for primary ovarian tumors (pregnant) and 580 non-pregnant patients (non-pregnant) aged 15similar44 years old. In the non-pregnant group, 79.7% of the patients complained of abdominal pain at the first examination. However, in the pregnant group, 62.0% of the patients reported no symptoms and 31.0% of them reported abdominal pain (p<0.05). There were no significant differences in the percentage of ovarian malignancies between the two groups (8.5% vs. 6.6%). However, the incidence of the advanced stage of greater-than-or-equal Ic in ovarian malignancies in the non-pregnant group was 42.1%, while it was 0% (p<0.05) in the pregnant group. Ovarian tumors including ovarian malignancies were significantly more frequently diagnosed with no symptoms in the pregnant group than in the non-pregnant group.

Inhibition of excystation and metacystic development of Entamoeba invadens by the dinitroaniline herbicide oryzalin.

The effect of oryzalin on excystation and metacystic development of Entamoeba invadens strain IP-1 was examined by transfer of cysts to a growth medium containing the drug. Excystation, which was assessed by counting the number of metacystic amoebae after induction of excystation, was inhibited by oryzalin in a concentration-dependent manner. Metacystic development, which was determined by the number of nuclei in metacystic amoebae, was also inhibited by oryzalin because the percentage of 4-nucleate amoebae at day 1 remained unchanged at day 3. The addition of oryzalin after the induction of excystation decreased the number of metacystic amoebae, compared with control cultures. When cysts were incubated for 1 day in growth medium plus oryzalin, little increase in the number of metacystic amoebae was observed after removal of the drug. Excystation and metacystic development were further inhibited when the cysts were incubated for 30 min in encystation medium containing oryzalin before transfer to growth medium with the drug. When cysts were incubated for 30 min in encystation medium before transfer to growth medium without the drug, metacystic amoebae decreased in number. Pretreatment of cysts with oryzalin for 30 min in phosphate-buffered saline markedly reduced viability and prevented excystation in growth medium with or without the drug. The results indicate that oryzalin inhibits excystation and metacystic development of E. invadens, suggesting that it may be an inhibitor of Entamoeba infection.

Successful non-surgical treatment of brain abscess and necrotizing fasciitis caused by Bacillus cereus.

Musculoskeletal and central nervous system infections caused by Bacillus cereus are very rare. Only a few cases have been reported, whose clinical courses strongly suggested that surgical procedures combined with appropriate antimicrobial therapy are necessary to cure these infections. A 60-year-old man with severe neutropenia due to myelodysplastic syndrome, developing necrotizing fasciitis and brain abscess caused by Bacillus cereus is reported. Without performing any surgical procedures, the patient was successfully treated with systemic antimicrobial therapy combined with granulocyte colony stimulating factor, which contributed to the increase in the neutrophil count.

A functional polymorphism in fas (CD95/APO-1) gene promoter associated with systemic lupus erythematosus.

OBJECTIVE: To investigate whether Fas promoter polymorphisms show a genetic contribution to the development of systemic lupus erythematosus (SLE) in a Japanese population, and to study the functional difference in promoter activity of the polymorphisms. METHODS: In 109 SLE patients and 140 controls, the frequencies of A/G polymorphisms at -670 nucleotide position and G/A at -1377 nucleotide position were determined by allele-specific polymerase chain reaction (PCR) or PCR-single strand conformation polymorphism analysis. The functional significance of the -670A/G polymorphism in the Fas gene was evaluated by a combination of Fas transcriptional activity in the reporter gene assay and binding activity of signal transducer and activator of transcription (STAT) 1 protein in the electrophoretic mobility shift assay. RESULTS: SLE patients exhibited significantly higher frequency of A allele at nucleotide position -670 (p = 0.004). There was no significant difference in the nucleotide position -1377 in Fas promoter gene between SLE patients and controls. The electrophoretic mobility shift assay demonstrated that the oligonucleotide with -670A in the Fas promoter had a higher binding ability to a GAS binding protein, STAT1, than that with -670G, although there was no statistically significant difference in the reporter gene assay. CONCLUSION: Fas promoter -670A/G polymorphism was significantly associated with SLE, suggesting a possibility that Fas promoter contributes, at least in part, to the pathogenesis of SLE.

[Etanercept].

Etanercept is a protein comprised of the extracellular domains of two TNF receptors attached to a Fc portion of an IgG. Etanercept was approved for not only reducing signs and symptoms but inhibiting of structural damage in patients with active RA who had an inadequate response to one or more DMARDs. Moreover, combination therapy with methotrexate will be attractive for severely active patients. The proportion of patients who have discontinued therapy due to adverse events is approximately 4%. Etanercept has not raised the risk for serious infections(0.04/patient-year) as well as malignancies. There are sporadic case reports of aplastic anemia, demyelination, lupus-like conditions, which are not significant so far. Etanercept may contribute rheumatologists to manage patients with RA.

Entamoeba invadens: inhibition of excystation and metacystic development by aphidicolin.

The effect of aphidicolin, a specific inhibitor of the replicative DNA polymerases, on the excystation and metacystic development of Entamoeba invadens was examined. The protein profile of metacystic amoebae and their immunogenicity in the presence and absence of aphidicolin were also examined by sodium dodecyl sulfate-polyacrylamide gel electrophoresis and immunoblotting. Excystation, which was assessed by counting the number of metacystic amoebae after the induction of excystation, was inhibited by aphidicolin in a concentration-dependent manner during incubation compared to the controls. Metacystic development, when determined by the number of nuclei in amoeba, was also inhibited by aphidicolin, because the percentage of 4-nucleate amoebae in cultures with aphidicolin during incubation was higher than that in cultures without the drug. The addition of aphidicolin to cultures at day 1 of incubation reduced the number of metacystic amoebae thereafter compared to cultures without the drug. The inhibitory effect of aphidicolin on excystation and metacystic development was reversed by removal of the drug. Pretreatment of cysts with aphidicolin before transfer to a growth medium containing the drug had no further effect on the excystation and metacystic development. Cellular proteins of metacystic amoebae with 4 nuclei, which were predominant even at day 3 in the cultures with aphidicolin, reacted strongly with rabbit anticyst serum absorbed with trophozoite proteins. In contrast, those of metacystic amoebae with 1 nucleus, which were predominant at day 3 in cultures without aphidicolin, no longer reacted with the absorbed anticyst serum, suggesting change in the expression of proteins during metacystic development.

Immunological characteristics of patients with vernal keratoconjunctivitis.

PURPOSE: Previously, we have reported that local excision and immunosuppressive treatment are useful in treating patients with very severe vernal keratoconjunctivitis (VKC). METHODS: We measured serum levels of immunoglobulins, cytokines, and interleukin-2 receptors (IL-2R) and the concentration of IL-4 in tears of 10 patients with severe VKC, and made a comparison with data from 10 healthy controls. Brush cytology specimens were examined to determine the number of inflammatory cells and the human lymphocyte antigen (HLA)-DR expression in conjunctiva. RESULTS: The mean serum level of total immunoglobulin (Ig)E, but not IgG, was higher in severe VKC patients compared with healthy controls. Specific IgE positivity to housedust, dust mites, and cat antigens was observed in 60-80% of patients. The level of IL-4 and IL-2R in serum and IL-4 in tears before treatment was higher in patients with VKC compared with controls. Serum levels of IL-2 and interferon-gamma did not increase. Brush cytology specimens from patients expressed HLA-DR. CONCLUSIONS: These results suggest that VKC could be a combination of type I allergic disease and an inflammatory cell disease including activated T cells, especially Th2 cells. Therefore, the surgical removal of inflammatory cells combined with immunosuppressive therapy could be advocated as a method of treatment.

Altered signaling triggered by ligation of the TCR/CD3 receptor in T lymphocytes from patients with systemic lupus erythematosus: the road from anergy to autoimmunity.

Some of the signaling phenotypic abnormalities observed in peripheral blood T cells from patients with systemic lupus erythematosus resemble those seen in cells anergized by stimulation through the TCR/CD3 complex, in the absence of costimulatory signals, or by stimulation with altered ligands. The potential association of this "anergic" phenotype and the triggering of autoimmune responses in human lupus is discussed.
