Therapeutic apheresis in myasthenia gravis patients: a six year follow-up.

Six years ago 4 patients suffering from myasthenia gravis (MG) types C and E according to Compston with failed drug therapy were initially treated 3 times (1 patient, a total of 11 times) by protein A immunoadsorption (Immunosorba, Excorim AB, Lund, Sweden). No further immunoadsorption treatments have been carried out. In addition, 3 patients were given a thymectomy. The present status of the patients was checked. We could see a beneficial effect in all MG patients. The patients are fit for work; each has an improved Besinger index. The patients were used as their own controls. A higher anti-AChRAb level 6 years after protein A immunoadsorption than at the beginning was seen in all patients, combined with less serious MG. In addition, their immunomodulation could be induced as seen in lymphocyte and inflammatory protein changes during the first 36 days after beginning immunoadsorption treatment. A larger population has to be investigated to verify these results.

Conservative treatment of acute Ormond's disease.

Ormond's disease is a chronic inflammatory process of the retroperitoneum, which sometimes takes a very acute course. We report the case of a man with acute bilateral hydronephrosis. Diagnosis was based on typical criteria seen by magnetic resonance imaging, which made biopsy unnecessary.

Impairment of pancreatic microcirculation in the early reperfusion period during simultaneous pancreas-kidney transplantation.

The most likely cause of graft pancreatitis is the ischemia/reperfusion injury which can be a major problem in simultaneous pancreas-kidney transplantation. Animal experiments suggest the important role in this process of an impaired microcirculation after reperfusion. We have investigated pancreatic microcirculation in the early reperfusion period during clinical pancreas-kidney transplantation. Tissue PO2 (PO2ti) was monitored by a PO2-sensitive electrode. After reperfusion (a.r.) samples were taken from the venous effluent of the pancreas and simultaneously from the radial artery. After an initial peak a transient fall of PO2 was found. Total blood flow and hemoglobin oxygen saturation (sHbO2) in the venous effluent increased until 90 min a.r. (107 ml/min, 97.1%) High venous sHbO2 and high PO2ti correlated with good graft outcome. These findings can be explained by an impairment of capillary perfusion (no reflow) and concomitant shunt perfusion. The data suggest the considerable relevance of pancreatic microcirculation in the early reperfusion period during clinical pancreas transplantation.

Up-regulation of cytochrome P450 1A2, 2C9, and 2E1 in chronic pancreatitis.

The oxidative metabolism of xenobiotics is effected mainly by cytochrome P450 enzymes (CYP), which are expressed as a family of genetically related enzymes primarily in hepatocytes. The pancreas is among the extrahepatic tissues expressing CYP, and it has been suggested that intermediates generated by them might be of pathogenetic significance for diseases of the pancreas such as chronic pancreatitis. We studied 10 surgical resection specimens by immunohistochemistry with polyclonal antibodies against recombinant human CYP 1A1, 1A2, 2C9, 2E1, and 3A and used tissues from 11 normal pancreata as controls. In addition, we assayed microsomal preparations for their capacity to metabolize verapamil. In normal pancreata weak to moderate expression of all enzymes was demonstrated immunohistochemically in up to 50% of duct epithelia, acinar cells, and islet cells. In contrast, in chronic pancreatitis an up-regulation was observed, with immunohistochemical positivity in some cases in up to 100% of duct epithelia and acinar cells. The oxidative capacity of microsomal preparations from chronic pancreatitis was higher than that of preparations obtained from control tissues; compared to liver microsomes, however, it was low. The up-regulation of CYP may have pathogenetic significance for chronic pancreatitis. Yet considering the pancreas' capacity for conjugation reactions, conceivably low levels of reactive intermediates could effectively undergo inactivation.

Prediction of in vivo drug interaction from in vitro systems exemplified by interaction between verapamil and cimetidine using human liver microsomes and primary hepatocytes.

Emphasis on drug safety is increasing as newly developed drugs become more potent. Interest in the prediction and description of drug interactions is growing accordingly. The study of potential interactions at a very early stage of drug development requires suitable in vitro models that describe drug interactions both qualitatively and quantitatively. The purpose of the work described here was to help assess the predictive value of in vitro drug interaction tests with liver microsomes and hepatocytes by means of the interaction between verapamil and cimetidine. The in vitro inhibition of verapamil metabolism by cimetidine observed during the studies was quantitatively similar to the results reported in published clinical studies after intravenous application. Studies using liver microsome fractions showed that the intrinsic clearances for the formation of various metabolites could be used to predict drug interactions. In addition, work with hepatocyte cultures revealed that an in vitro system covering both phase I and phase II reactions should be included in such studies to permit quantitative prediction of the various metabolic pathways. Both human hepatocyte cultures and human microsomes offer certain advantages for predicting the degree of drug metabolism and interactions in humans at the biotransformation level. Therefore, it seems likely that the simultaneous application of both systems will yield conclusions that most closely approximate the situation in humans.

[Programmed lavage as a basic principle in therapy of diffuse peritonitis].

The therapeutic concept of programmed lavage has been established in the treatment of severe diffuse peritonitis. This treatment was given to 30 patients from January 1995 to February 1997. The total lethality rate was 30% (11/30 patients). Ten patients had organ failure of three or four organ systems at the time of first laparotomy. In this group the lethality rate was 50%. On average 9.2 programmed relaparotomys were performed per patient. As early complications, small bowel fistulas were observed in three patients, bleeding in three patients and problems due to the laparostomy in four patients.

Long-term follow-up after open perineal cryotherapy in patients with locally confined prostate cancer.

OBJECTIVE: Appropriate therapies for locally confined adenocarcinoma of the prostate are available-but there is as yet no gold standard of therapy. For that reason, old therapeutic regimens are being revised. One such regimen is cryotherapy. METHODS: Between 1976 and 1989, 1,250 patients were seen with newly diagnosed prostate cancer. Sixty-six patients of stages T1c (n = 3), T2a (n = 32), T2b (n = 9), T2c (n = 11), T3a (n = 4), T3b (n = 5) and T3c (n = 2) were treated by open perineal cryotherapy. Mean age was 68 years. Three months after surgery, transurethral resection of the prostate and/or perineal biopsy was performed. RESULTS: In 66% of patients with stages T1c-T2b and in 87% of patients with stages T2c-T3c, positive biopsies were obtained. To date, 28 patients have died between 3 and 16 years after cryotherapy, with a mean survival of 7.2 years. The mean follow-up period of survivors (38 patients) is 8.5 years. Complications were: stress-incontinence in 10%, impotence in 10% and temporary rectoperineal fistula in 8%. CONCLUSIONS: Cryosurgery is an effective treatment for locally confined prostate cancer. With improvement of the procedure, clinical outcome may also improve.

Rapid HPLC assay for verapamil and its metabolites: use for application to in vitro studies.

An improved method using isocratic reversed phase HPLC is presented for the extraction and rapid determination of verapamil and its main metabolites in microsomal preparations and cell culture media. Possibilities for using the method to estimate cytochrome P450 enzymes in microsomal test systems and hepatocyte cultures are described. The studies show that primary hepatocyte cultures are suitable for studying the metabolism and interactions of pharmaceuticals in vitro and could be superior to microsomal systems in many cases.

[A new method for pseudocysto-duodenostomy as therapy in pancreatic pseudocyst in chronic pancreatitis].

Reference in the surgical literature to the use of pseudocysto-duodenostomy whether laterolateral by Ombredanne [6] or transduodenal by Kerschner [4], is uncommon. The author with the aid of specially designed three-jaw prong, now, prefer to use pseudocysto-duodenostomy. From 1970, 411 patients underwent surgery for complicated chronic pancreatitis. 67 of the 93 patients requiring an internal cysto-intestinal procedure were treated by pseudocysto-duodenostomy; 11 additional patients were treated by derivation in the first retroperitoneal transposed jejunal loop. Postoperative mortality for the first month was 0%. The actuarial survival rate at 5 years was 86.9%. These satisfactory results have encouraged us to compare this new operative method with cystojejunostomy. It allows pancreatic secretions to drain into their natural anatomical site. Compared with external drainage it avoids the often prolonged and costly complications.

[Steps for solutions and hindrances in progress in relation to an implantable bio-artificial hemofilter-intestine-hybrid kidney].

On the view of 1989 older and modern developments to a bioartificial implantable hybrid kidney are presented. To the realization proof of the implantable hemofilter-intestine-hybrid kidney the routes were described in an experimental rat model to the plasmakinetic simulation of the urea and sodium level as a marker of the hybrid kidney functions. The results of the bioartificial hemofilter-intestine-hybrid kidney development are presented, but significant detail problems must be solved, especially the hemofilter biocompatibility problem.

[The selective elimination function of the colon in a rat model for the substances water and urea in long-term experiments as a contribution to the development of an implantable hemofilter-intestine-hybrid kidney].

The investigations on the selective elimination and preservation function of the colon represented one of the numerous problems in the development of an implantable hemofilter-intestine-hybrid kidney. The assumption of an ideal reflection of urea on the intestinal mucosa and the ability of water absorption of the colon had to be checked. A fluid which is nearly analogous to the ultrafiltrate with different urea concentrations was infused into the colon of rats by a cecocutaneous catheter fistula during two-phasic long-term experiments. The renal and enteral urea elimination rates were defined. The apparent reflection coefficient for urea of the colon amounted 0.5 to 0.6 in an infusion rate of 80 ml/day. The ability of water absorption of the colon in rats of nearly 300 g of weight could be increased up to 200 ml/day during slow and continuous adaptation in the infusion quantity.
