The incidence of delayed wound healing and urinary tract infections was studied in 2 randomized groups of post-operative gynecological patients, one of which was given systemic antibiotic prophylaxis and the other not. 143 women were given Bactrim in a standard dose of 2 x 2 tablets or ampoules daily, commencing preoperatively during and continuing until the eighth postoperative day. The control group received placebos parenterally and orally for the same period. The incidence of complications in wound healing, of subfebrile and febrile states, urinary tract infections and micturition difficulties following removal of the catheter was significantly lower in the group who had undergone antibiotic therapy. 83% of this group, compared with 68% in the control group, were discharged after the normal hospital stay. In 51% of the control group a chemotherapy or antibiotic course had to be given secondarily to combat infection, the commonest indications being urinary tract infections or pyrexia for several days. The drawbacks were side effects of the systemic chemoprophylaxis, such as skin eczema (6%) and gastrointestinal disturbances, as well as a tendency to infections with problem bacteria requiring treatment with reserve antibiotics. It is concluded that routine postoperative antibiotic prophylaxis, despite its favourable effect on infections morbidity as a compromise between optimum treatment for the individual and general medical interest, must be prescribed on an individual basis.
The results of oestriol excretion, HPL activity and fetal heart rate monitoring are presented during the last three weeks before the birth or the intrauterine fetal death. Abnormal results were found in all cases at least two weeks before the intrauterine death. In the same periode, we observed a raise of abnormal values in cases with intrauterine growth-retardation. In cases with normal birth weight, the majority of values were normal and without a significant decrease.

In a randomized series of patients who underwent gynaecological operations, the prophylactic effect on thrombophlebitis of low dose Heparin was compared to that of oral anti-coagulants. One group (n = 221) received Heparin (Liquemin Roche) subcutaneously in a dosage of 5000 units 2 hours pre-operatively and every 12 hours until the eighth post-operative day. The control group (n = 237) continued to receive the conventional prophylactic medication with Sintrom by mouth from the second post-operative day to complete ambulation keeping the quick test between 20 and 30%. The two groups were more or less identical regarding several risk factors and the following results were obtained: 1. The number of cases of deep thrombophlebitis diagnosed by the I-125-Fibrinogen test was significantly less in the group receiving Heparin than in the group receiving Sintrom. Clinical examination showed the same correlation but only detected 30% of all cases of deep thrombophlebitis. 2. The clinical suspicion of pulmonary embolism was more frequent in the group of patients receiving low dosage Heparin. Most of the symptoms were mild and none of the patients died. 3. During the anti-coagulant treatment the incidence of secondary bleeding was the same in both groups. The prevention of deep thrombophlebitis by low dosage Heparin is a very effective and simple method with few side effects and is superior to the post-operative prophylaxis with oral anti-coagulants.
