[Current facts on ulcer disease].

In the second half of this century, some important changes characterized the peptic ulcer history. The overall incidence of ulcer disease markedly decreased, with some differences in the various geographic areas and according to socioeconomic development of population. Peptic ulcer seems now more frequent in women and in the elderly patients, in comparison with the past. The appearance of H2-Receptor Antagonists determined a decline of ulcer surgery, because the ulcer is become a medical disease. Pyloric stenosis also, after ten years of H2-Receptor Antagonists treatment, markedly decreased. Indeed, the problem of acute complications is still actual. The detailed analysis of patients hospitalized, shows significative decrease of acute complications in patients with chronic ulcer and a marked anamnestics. The efficacy of medical treatment in the control of acute ulcer complications, seems to be balanced with new etiopathogenetic factors, which are able to start the appearance of acute complications of the disease (increased assumption of ulcerogenic drugs; smoke and alcohol abuse; increased socio-politic engagement of women and improved resuscitations technics).

Usefulness of carcinoembryonic antigen measurement in gastric juice of patients with gastric disorders.

Due to conflicting reports on the role of CEA measurement in the gastric juice of patients with gastric malignancy, we have assessed gastric CEA levels in a variety of both nonmalignant and malignant lesions of the stomach. The average gastric CEA levels were higher in patients with gastric ulcer and gastric cancer than in healthy subjects and those with duodenal ulcer. In addition, the frequencies of abnormally high values were significantly (P less than 0.001) higher in gastric ulcer or gastric cancer than in healthy or duodenal ulcer groups, whereas there was no difference between gastric ulcer and gastric cancer groups. The evaluation of gastric CEA levels in relation to gastric inflammatory changes leads us to suggest a close relationship between gastric CEA values and the degree of gastric inflammation. Gastric lavage cytology did not provide a sensitive tool for detection of gastric disorders. This study suggests little value for gastric CEA determination in gastric cancer detection. However, we believe the association of rising gastric CEA levels with various types of gastritis justifies measuring gastric CEA to recognize "precancerous" mucosal changes of the stomach.

Surgical sphincterotomy in the early management of non-malignant obstructive jaundice. Indications and results.

Biliary obstruction by non-malignant lesions can be treated either by biliary-enteric anastomosis or by sphincterotomy or sphincteroplasty. In the present report, 153 jaundiced patients operated on by sphincterotomy, were retrospectively examined. Proper indications for surgical sphincterotomy have been considered as follows: stones in the common bile duct (49.7%), biliary stones with secondary stenosis of Oddi (26.8%), impacted ampullary stones (17.6%), primary stenosis of Oddi (3.9%), stenosis of Oddi by perivaterian chronic pancreatitis (2.0%). Preoperative early diagnosis was recognized by US, ERCP, PTC, CT. The overall operative mortality was 2.6% and determined by acute renal failure in 2.0% and acute pancreatitis in 0.6%. Postoperative complications occurred in 4.6% and consisted in acute pancreatitis in 3.3% and bleeding from sphincterotomy in 1.3%. One hundred and twenty-three patients (82.5%) were followed up by questionnaire and by both X-ray studies and ultrasonography. Good results were observed in 80.5%, fair results in 17.1% and poor results in 2.4%. The presents study suggests further indications for surgical sphincterotomy in the treatment of benign biliary obstruction. The effectiveness of this procedure in the early management of non-malignant jaundice is also stressed.
