Infradian rhythms of alanine aminopeptidase excretion during gentamicin therapy.

Urinary excretion of alanine aminopeptidase (EC 3.4.11.2) was examined in 30 patients (22 women, 8 men, range 38-81 years; mean age 55.4) receiving gentamicin in a therapeutic dosage, which was based on the monitored blood creatinine concentration. In general, therapy lasted 10 days. The excretion of 24 individuals displayed significant infradian rhythms with periods between 2.2 and 8.1 days. In 10 of these 24 patients (42%) a circaseptan period was detected. The high portion of circaseptan rhythms might have been induced by the detrimental effects of gentamicin on the proximal tubule and the resulting processes of reconstitution.

[Experience with sisomicin in pediatrics (author's transl)].

Sisomicin, an aminoglycoside antibiotic, is especially effective against Escherichia coli, Klebsiella, Enterobacter, Citrobacter, Serratia, indole-positive and indole-negative Proteus species, Pseudomonas aeruginosa, Salmonella and Staphylococcus aureus. It has a bactericidal action. Although sisomicin is similar to the other aminoglycoside antibiotics, there is not complete cross-resistance to them. Our own pharmacokinetic investigations showed that a dose of 2--3 mg/kg body weight of sisomicin twice daily is necessary in the neonatal period. Infants should be given 2.5 mg/kg body weight three times daily, and school children 1.5--20 mg/kg body weight, likewise three times daily. Excretion of sisomicin in the urine is lower in children than in adults, amounting within 24 hours to only 10--20% in newborns, and 30--40% in school-children. Sisomicin induces excretion of some enzymes in higher quantities from the tubular part of the kidneys, especially alaninaminopeptidase. A report is given on 58 patients, especially newborns and prematures, who were treated for about seven days with sisomicin. The results obtained with a wide variety of infections (such as omphalitis, aspiration of amniotic fluid with broncho-pneumonia, phlegmons of the galea, and also pyelonephritis and mucoviscidosis with pulmonary complications) can be described as good, with a success rate of 85%. On only seven occasions were insignificant transitory side-effects, such as slight increase in transaminases, toxic-allergic exanthema and pain in the region in injection, observed.

Age Factors

[Hemofiltration of lysozyme in dialysis patients].

With the help of the haemofiltration on the capillary dialysator CDAK 4 the behaviour of lysozyme (muramidase) was investigated in 20 dialysis patients. In 6 of 55 cases a measurable lysozyme concentration in the haemofiltrate was found. A relation to the size of the serum lysozyme level could not be established. The influence of defective capillaries is discussed. Despite the in most cases not measurable lysozyme concentrations in the haemofiltrate a decrease of the serum lysozyme level taking place parallel to creatinine could be proved during dialysis. Examinations of the influence of the renal rest function in dialysis patients with a rest diuresis resulted in a lower lysozyme concentration in the serum than in patients with complete anuria. These findings allow conclusions to the behaviour of low-molecular proteins in the kidney of dialysis patients and to their losses on the dialysis membrane.

[Urinary excretion kinetics of enzymes and proteins in the application of therapeutic doses of gentamycin].

9 patients with chronic pyelonephritis were given gentamicin intramuscularly in an appropriate dosage for a period of 10 days. The urinary excretion of cells, protein, alanine aminopeptidase (EC 3.4.11.2),N-acetylglucosaminidase(EC 3.2.1.30) and alpha-galactosidase (EC 3.2.1.23) was determined daily. The most striking enzyme increase occurred in alanine aminopeptidase. The enzyme and protein output appears to be rhythmic and discontinuous. A 2--3-day rhythm is suggested. Rhythmic alterations of the digestion activity of the lysosomes are assumed to be the cause of these changes.

[Metabolic changes in renal insufficiency].

In the chronic renal insufficiency is shown a decrease as well as an accumulation of some metabolites in the organism. There appear dietarily conditioned insufficiency states and also decreased rates of synthesis of 1,25-OH cholecalciferol , of the renal erythropoietic factor and of histidin. Apart from nitrogenous metabolic end products also the molecules with a middle molecular weight (300-1.500 Dalton) are accumulated. From this, among others, the trade-off hypothesis may be derived, which characterizes the biochemical distrubances in renal insufficiency as a sequel of the accumulation of metabolic regulators. In the analysis of the middle molecule fraction by means of combined chromatographic techniques we succeeded in isolating an inhibitor of glucose utilisation and in defining of some of its biochemical qualities. With the help of this survey the authors demonstrate a new way of thinking and a promising direction for the solution of still unclear probelms of renal insufficiency.

[Urinary alanineaminopeptidase with reference to age and sex].

In 200 healthy test persons of either sex between 15 and 94 years the excretion of the alanine aminopeptidase (EC 3.4.11.2) and of creatinine with the urine was tested. Female test persons excrete smaller quantities of enzymes with the urine than male ones. In the two sexes the efflux of the alanine aminopeptidase only slightly increases with growing age. With regard of the enzyme activities to the creatinine in the urine the curve of excretion is clearly deformed, since in the senium significantly smaller quantities of creatinine are excreted. When an enzyme/creatinine quotient is used reference values specific to age must be used.

Adolescent

[Diagnostic value of LDH-isoenzyme determination in the urine].

In patients with chronic pyelonephritis (n = 17), nephrotic syndrome (n = 7) and hyperthyroidism in comparison to a reference group of healthy persons (n = 17) total activity and isoenzymes of the lactate dehydrogenase in the urine were determined. Only in patients with pyelonephritis is not regularly increased activity of the total lactate dehydrogenase a clear increased excretion of the isoenzymes IV and V is established. The isoenzyme pattern shows a dependence on the content of leucocytes in the urine and normalizes in renal infections only in a therapeutic effect.

[D-thyroxin in the therapy of type IIa and IIb hyperlipoproteinemias].

21 patients with hyperlipoproteinaemias of type IIa and IIb were treated with D-thyroxin (4 mg/die) after a placebo phase. The observed changes of the cholesterol and triglyceride level in the serum are described. Hypermetabolic and cardiac side effects did not appear in the patients. The possible sites of action of D-thyroxin in the intermediary lipid metabolism are discussed.

[Excretion of alanine aminopeptidase in the urine after administration of lysosomotrophic substances in patients with latent or florid pyelonephritis].

It is described a test for the presence of diffuse nephropathies which is based on the fact that after application 20 ml of the renal diatrizoate X-ray contrast medium Visotrast or of 500 ml of a 10% mannitol solution in patients with preexisting renal diseases appear reversible, diagnostically relevant increases of the alanine aminopeptidase excretion with the urine. In persons with healthy kidneys the excretion of enzymes changes insignificantly. The increased excretion of alanine aminopeptidase is also observed in patients with latent pyelonephritis and is traced back to the increased formation of an osmotic nephropathy in patients with preexisting nephropathies.

Alanine

[Alanine aminopeptidase and creatinine excretion under various conditions of diuresis in man].

In 10 female test persons Volhard's water and concentration experiment was carried out. It exists a highly significant correlation between the alanine aminopeptidase activity and the creatinine concentration in the urine. The relation of the two factors to the urine minute volume is hyperbolic. The creatinine in the urine may be regarded as a favourable reference parameter for the excretion of alanine aminopeptidase.

[Aminopeptidases in the serum and urine of patients with hyperthyroidism].

In patients with hyperthyroidism the serum activities of the leucine aminopeptidase (LAP) and the alanine aminopeptidase (AAP) as well as the alanine aminopeptidase excretion in the urine were determined. A significantly increased activity of the leucine aminopeptidase in the serum and an increased excretion of alanine aminopeptidase in the urine were found. The AAP in the serum did not show a significant increase of activity. On account of the changes in the serum and in the urine before and during therapy a low-grade participation of the hepatobiliary and renal systems which are clinically not uppermost is to be assumed. Increased excretion of AAP and hyperthyroidism coincide nearly without exception in out patients. Correlation-statistical investigations make it probable to regard the increased excretion of AAP in the urine as an indirect parameter of the peripheral metabolism in hyperthyroidism.

[Acute poisoning with L-triiodothyroinine, L-thyroxine and phendimetrazine bitartrate with suicidal intention].

It is reported on an acute intoxication after application of 8 mg L-thyroxin, 2 mg L-triiodotyronine (200 tablets Thyreotom) and 4.5 g hendimetrazine bitartrate (100 tablets Sedafamem). The clinical picture is described. An endangering of the vital function of the organism was not observed. The symptoms disappeared after 4 days. The pharmacological aspects of the intoxication after application of the hormone of the thyroid gland, its significance for the pathogenesis of the thyreotoxic crisis and its therapy are discussed
