Effect of high level bovine parathyroid extracts on serum PTH, cAMP and urinary cAMP, Na+, and uric acid in stone-forming patients.

Patients with a history of idiopathic calcium oxalate stones, but without current stone formation, show distinctly higher levels of serum cAMP (cyclic adenosine 3',5'-monophosphate) after rapid injection of an extreme concentration of parathyroid extract (PTE). Extreme parathyroid hormone (PTH) levels, as induced by rapid injection of biologically active bovine PTEs, significantly affect the renal generation of cAMP and the elimination of electrolytes in a healthy control group. The stimulated increase of urinary cAMP, sodium, potassium, calcium and phosphate are observed to return to normal levels 120 min after extreme PTE injections for both groups studied.

[Thyroid hormones in metastasizing prostate cancer treated with polyestradiol phosphate].

Patients with prostate cancer demonstrate a significant change in thyroid hormones when treated with estrogens after orchiectomy. Low-dose estrogen treatment for more than six months stimulates the synthesis of thyroxine-binding globulin. As a consequence, serum level of thyroxine is also increased. Serum free thyroxine is distinctly lowered by estrogen treatment while the linear ratio of thyroxine and free thyroxine is disturbed.

The effect of high level CaCi2 injection on parathyroid hormone dependent cAMP in stone formation patients.

Patients with a history of idiopathic calcium oxalate stones but without current stone formation do not react to stimulation of parathyroid extract as expected after high dose calcium pretreatment. With Ca2+ pretreatment, these patients show higher serum and also renally generated levels of cAMP after rapid injection of extreme parathyroid extract-concentrations. Healthy controls show a modest increase of serum levels cAMP after similar stimulation with parathyroid extracts. In contrast to patients, calcium pretreatment of controls significantly decreases the renal cAMP response to parathyroid extract stimulation. Without Ca2+ pretreatment, both patients and controls show a strong increase in urinary and also in renally generated cAMP levels following parathyroid extract stimulation.

Polyamines, hydrolases, (PAP, LAP, SDH, plasmin) TSH, T3, T4 and C-peptide in benign hyperplasia of the prostate.

High concentrations of polyamines have been found in the normal human prostate. The profile of these amines appeared significantly changed in benign hypertrophy of the prostate. An increase of spermine and a fall of putrescine were always found in patients with a hypertrophied prostate weighing more than 30 g. Alterations of plasmin in these tissues seemed to reflect changes in the matrix; abnormalities of thyroid and pancreatic function documented by changes in the serum levels of TSH and c-peptide which are thought to be further evidence of a mesenchymal-epithelial interaction in the pathogenesis of benign prostate hypertrophy.

Synthesis and body distribution of alpha-aminoisobutyric acid-L-11C in normal and prostate cancer-bearing rat after chemotherapy.

This paper describes the synthesis of alpha-aminoisobutyric acid-11C (11C-AIB) and studies its body distribution in healthy and tumor-bearing animals. High tissue levels of 11C-AIB were found in organs with high metabolic activity (pancreas, liver, kidney). The prostate tumor 11C-AIB concentrations are significantly lower than that of pancreas, liver, and kidney, but increased when compared with normal prostate levels. Effective chemotherapy, which reduces tumor growth of two different prostate adenocarcinoma cell lines (R-3327-H, R-3327-G) also decreases 11C-AIB levels and the 11C-AIB prostate to tumor ratio.

[13,14-Dihydro-15-keto prostaglandin F2 alpha and haptoglobin in the serum of patients with urogenital tumors].

Prostaglandin F2 alpha can be measured as a stable degradation product, 13,14-dihydro-15-keto-prostaglandin F2 alpha (DHK-PGF2 alpha). Pathological serum levels of this lipid compound have been observed in prostate hyperplasia and treated testicular cancer patients. In renal cancer patients elevated DHK-PGF2 alpha shows a trend to normalize 12 months following nephrectomy in stage T0N0M0. Bladder cancer patients have slightly increased DHK-PGF2 alpha that differ in relation to preinvasive bladder cancer. In contrast serum haptoglobin levels were significantly pathological in patients with kidney and bladder cancer, suggesting involved cancer metabolism. Patients with prostate and testis cancer are without distinct relation to the tumour stage. After specification of haptoglobin types no coincidence to urogenital tumours could be detected. Correlated aberration of PGF2 alpha and haptoglobin displayed different patterns depending on the type of urogenital tumour.

[Peptide hormones LH, FSH, TSH, prolactin, beta-HCG and PTH in patients with urogenital tumors].

Serum levels of the peptide hormones, LH, FSH, TSH, prolactin, beta-HCG and PTH are observed to significantly modulate in patients with urogenital tumors, beta-HCG appears elevated in patients with chorioncarcinoma, teratoma and embryonal carcinoma and is shown to increase serum prolactin. TSH and prolactin are measured in pathological range in patients with renal carcinomas. Following nephrectomy serum TSH persists in high levels only in patients with advanced diseases, while PTH is more frequently suppressed in these patients. Serum LH and prolactin show a more distinct change of values in infiltrating bladder cancer, when compared to that of noninvasive bladder carcinomas. Prostate benign hyperplasia and cancer demonstrate similar ranges for peptide hormones, that will be changed by estrogen treatment in its LH, TSH and prolactin pattern. Serum levels of peptide hormones are affected by a complex system of ectopic hormone production, synthesis of peptide analogues, metabolic changes due to age and treatment.

Glycopeptides (FSH, LH, TSH, prolactin) and glycoproteins in patients with genitourinary cancer.

The present investigation has analyzed 249 patients suffering from urogenital cancer in respect to aberrant concentration of prolactin, TSH, LH and FSH. A simultaneous pattern of acute phase reactants (alpha 1-glycoprotein, alpha 1-antitrypsin, beta 2-glycoprotein and haptoglobin) was related to the content of these hormones. Abnormal results of prolactin and TSH were present in 30% of all patients, whereas frequent aberrations of alpha 1-acid glycoprotein and alpha 1-antitrypsin were detected in 60--80%. Acute phase reactants changed to more pathological values when correlated to elevated prolactin.

Reevaluation of C-reactive protein in cancer sera by radioimmunoassay and radial immunodiffusion. I. Diagnostic value and use in battery of conventional tumor markers.

C-reactive protein (CRP) has been found to be ubiquitous in sera when determined by a highly sensitive methods such as radioimmunoassay. Patients with some tumors appear to have much higher serum levels of CRP than others; in these, CRP seems to reflect the tumor burden. There is a positive correlation with TNM staging and the disease activity a the time of determination. CRP shows a high degree of 'uniqueness' when correlated with other routinely used cancer markers such as carcinoembryonic antigen, placental-like alkaline phosphatase and gamma-glutamyltranspeptidase.

The effect of alpha-difluoromethyl-ornithine on tumor growth, acute phase reactants, beta-2-microglobulin and hydroxyproline in kidney and bladder carcinomas.

Multiple oral doses of alpha-difluoromethyl-ornithine (alpha-DFMO), 18-24 g/day for up to 2 months, were administered to 2 patients with invasive and metastatic carcinoma of the bladder and to 3 patients with metastatic renal cancer in an open study. The moderate antigrowth effect of alpha-DFMO in these patients was concluded from the results of serial kidney, liver and bone scintigrams, from the low frequency of a local recurrence of bladder carcinoma and from the survival rate of 4 of 5 patients after 12 months. Increased excretion of hydroxyproline and beta2-microglobulin were measured 1 month following alpha-DFMO treatment suggesting initial tumor necrosis and degradation. Treatment was poorly tolerated with gastrointestinal side effects (e.g. gastritis, vomiting, diarrhea) in all subjects, erythropenia in 2 subjects (associated with thrombocytopenia and leucopenia in 1 subject each), and audiovestibular symptoms in 2 patients. All side effects were reversible and normal function returned 2-4 months after stopping therapy.

Parathyroid hormone, cAMP, electrolytes and uric acid after high dose CaCl2 in patients with idiopathic stone formation.

High dose levels of calcium chloride significantly decrease serum level of parathyroid hormone (PTH) in both patients with history of idiopathic stone formation and healthy controls. Suppressibility of parathyroid glands is obtained by injection of 10 ml 10% calcium chloride without any restriction in calcium diet. Serum levels of calcium measured 14 mg/dl 10 min. after high dose calcium chloride injection and distinctly affect cAMP in urine in patients when compared to healthy controls. The elimination of electrolytes and uric acid is elevated 40 min. after calcium application in both patients and controls and returns to normal values 120 min. later. No stone formation has been observed in an eighteen mouth follow up of all subjects studied.

alpha-Difluoromethylornithine (alpha DFMO) and phenoxybenzamine hydrochloride in the treatment of chronic non-suppurative prostatitis.

In a single-blind, placebo-controlled, cross-over study in 11 patients with chronic non-suppurative, culture-negative prostatitis, alpha-difluoromethylornithine (alpha DFMO) (18 g/day) was administered orally for 1 month. Ten patients completed the study. In part II of this investigation, 60 mg/day phenoxybenzaminehydrochloride, an alpha-adrenergic receptor blocking compound (alpha ABC), was studied in 11 patients with chronic non-suppurative prostatitis in an open-phase design. Several patients had a decrease in prostate size and associated pain symptoms with alpha DFMO treatment. One patient with gross hematospermia prior to treatment had a remission on alpha DFMO and a return hematospermia on subsequent placebo treatment. Six subjects with microscopic hematuria post-prostatic palpation had a decrease in RBC counts while on alpha DFMO treatment. There was a 16% fall in hematocrit and hemoglobin following alpha DFMO treatment, which returned to pretreatment values within 2 months after stopping the drug. alphaABC distinctly improved symptoms of chronic prostatitis. Five patients remained disease free for a period of 3-6 months and 10 patients had a decrease of RBC counted in the urine. alpha ABC was accepted by the patients despite the fact of frequent orthostatic dysregulation and aspermia.

[Clinical significance of peptide hormones LH, FSH, TSH, prolactin, HCG, parathormone, calcitonin and prostaglandin F2 alpha in kidney neoplasms].

Peptide hormones LH, FSH, TSH, e-peptide, prolactin, HCG, PTH, calcitonin and prostaglandin F2 alpha were evaluated in 25 patients with renal adenocancer. Prolactin elevation found in 45% was unrelated to the stage of the disease, while aberrant TSH and FSH were more sensitive in indicating distant metastasis. Prostaglandin F2 alpha, measured as a stable degradation product 13,14-DHK-PGF2 alpha, was frequently elevated and normalized following tumor nephrectomy within 12 month. Serum PTH was decreased in patients with tumor dissemination suggesting resorptive hypercalcemina, whereas none of the patient had circulating HCG levels. Therefore even patients without any paraneoplastic syndromes have a frequent change of peptide hormones directly or indirectly caused by renal cancer.

Plasma lipid bound sialic acid in patients with prostate and bladder cancer.

Plasma lipid bound sialic acid (LSA) was measured in patients with prostate and bladder cancer to determine the usefulness of this biochemical marker in teh staging of malignant disease and in monitoring the efficacy of therapy. Patients with advanced stages of prostate cancer with bone metastases exhibited LSA levels significantly higher than normal subjects. Patients with bladder cancer showed elevations in LSA both in early noninvasive and in advanced stages of the disease. In both types of cancer, patients treated successfully and clinically free of disease did not have elevated LSA levels, whereas patients failing to respond to treatment had persistently high values.

[Parathormone and renal excretion in healthy controls and patients with nephrolithiasis (author's transl)].

Idiopathic nephrolithiasis with recurrent ca-oxalat stones are investigated for occult renal dysfunction or for disturbed clearance of PTH. After injection of PTE a significant increase of renal excretion of Na+, K+, Ca2+ and uric acid was observed normalizing within 120 min after injection. Healthy controls showed similar changes, and following PTH the cAMP generating capacity of patients with recurrent stones was in normal range. The turn over rate of PTH was 30-40 min without differences in the collectives studied.

13,14-Dihydro-15-keto-prostaglandin F2 alpha in patients with urogenital tumors.

13,14-Dihydro-15-keto-prostaglandin F2 alpha (13,14-DHK-PGF2 alpha) represents a stable product of degradation after pulmonary flow and it is shown to be reliably measured by radioimmunoassay. In patients with urogenital tumors, serum levels of 13,14-DHK-PGF2 alpha are distinctly elevated when compared to a control group. The rate of synthesis of this compound in urogenital tumors, however, appears to be different.

Interrelations of PTH, cAMP, creatinine and uric acid in nephrectomized patients under DNA or PTH application.

Patients with reduced renal mass (i.e., nephrectomized) and slightly decreased renal clearance (70-80%) have been treated by 4 g DNA to study uricosuria. Prompt or prolonged renal excretion of uric acid and creatinine have been observed with indirect effects on parathyroid secretion. Urinary cAMP, however, failed to correlate to the change of parathyroid function. Reversely, PTH injection significantly increases uricosuria suggesting interrelated secretion of uric acid and cAMP of the proximal tubules. In some respects this model can be used to investigate increased DNA metabolism and parathyroid function.
