[Glassman palatine expansion. Experiences with mono- and bimaxillary dysgnathia operations].

The median palatine suture has long been regarded as having the greatest resistance to dilatation of the maxilla. In 1984 Glassman [3] presented a conservative-surgical method of division of the palatineal suture in which only the lateral and anterior support of the maxilla is debilitated surgically. An orthodontic apparatus cemented in place preoperatively is already used intraoperatively for dilatation of the maxilla. In the period from 1991 to 1997, 16 patients with a leptomaxilla and various dysnathic findings have been operated on at our clinic by the method described by Glassman. Sometimes the maxilla was dilated intraoperatively using the apparatus cemented in place by the method of Derichsweiler and dilatation was continued post-operatively until the described result was achieved. After successful dilatation of the maxilla and a stabilizing phase, a mono- or bimaxillary operation was performed. The use of this method led to the desired result in 15 patients. Dilatation of the maxilla was objectivized by determining the pre- and postoperative width of the anterior and posterior dental arch using models and X-ray of the occlusal overlay of the maxilla. In one patient who was operated on at the age of 38 years, a fracture of the alveolar process of the maxilla occurred unilaterally due to the completed ossification of the median palatine suture. The method of surgically aided dilatation of the maxilla at the level of Le Fort I plane is suitable for patients up to the age of 30. In older patients, the median palatine suture should be transsected as well.

Prognostic relevance of DNA image cytometry in oral cavity carcinomas.

OBJECTIVE: To evaluate the relation of DNA content of squamous cell carcinomas of the oral cavity to clinicopathologic features by image cytometry. STUDY DESIGN: A series of 52 patients with operable oral cavity carcinomas at stage T1-4 and NO-3 were studied. The tumors were classified according to the TNM classification and graded histopathologically. RESULTS: A positive correlation between tumor size and ploidy status was observed. There was a significant correlation between ploidy status and histologically proven existence of cervical lymph node metastases (61% nondiploid versus 29% diploid, P < .03). A relationship between histopathologic grade and ploidy status could not be proven. The cumulative five-year survival rate among patients with diploid DNA findings as opposed to those with nondiploid findings was significantly increased (P < .03). CONCLUSION: When assessing the prognosis of squamous cell carcinomas, DNA cytometry analysis can be used as an additional method. The significantly higher N stage, a higher frequency of metastases and a significantly lower survival rate with nondiploid tumors underline the importance of this examination method.

[Nuclear magnetic resonance tomography study of the position of the discus articularis after dislocation fractures].

16 patients with a total of 25 condyle neck fractures, of which 17 were fractures with luxation, 4 fractures with dislocation and 4 diacapitular fractures, were checked clinically and using x-ray and nuclear magnetic resonance diagnosis. Except for the diacapitular fractures, all fractures were treated surgically and fixed functionally stable using mini plates or lag screw osteosyntheses. The examination results show that in the majority of cases a regular discus position and joint function appears along with the repositioning of the condyle neck.

Effects of stimulated repopulation on oral mucositis during conventional radiotherapy.

The effect of local conditioning of human oral mucosa by silver nitrate solution (3%) on epithelial proliferation rates was tested in 11 healthy volunteers by in vitro labelling of biopsies with tritiated thymidine. Compared to control biopsies from 13 volunteers, stimulation over 3 days, 3 times per day, yielded a significant (p = 0.006) increase in the epithelial labelling index (LI) from 4.75 +/- 0.32% to 6.85 +/- 0.65%, i.e., by 44%. The increase in the absolute number of labelled cells per mm epithelial length was dependent on the overall cell density at the various intraoral sites and varied between 45% in the maxillary vestibule and 91% at the floor of the mouth. In an analysis of variance, stimulation turned out to be the most important source causing the effect (p = 0.011 for LI and 0.015 for labelled cells per mm). In a radiotherapy trial with conventional postoperative treatment with 5 x 2 Gy/week to a total dose of 60 Gy in 6 weeks, the left buccal mucosa in 10 patients with squamous cell carcinomas of the head and neck was conditioned (3% silver nitrate, 3 times per day, 5 days before and the first 2 days of radiotherapy) while the contralateral mucosa, receiving an identical dose, served as individual control. Mucositis scores according to the EORTC/RTOG or the Dische system showed that the time course and severity of the mucosal response was almost identical in both cheeks, which is in clear contrast to a previous clinical study (Maciejewski et al. Radiother. Oncol. 22, 7-11, 1991). Differences in radiation dose intensity, i.e., weekly dose, in these studies are discussed as a tentative explanation for the different clinical findings.

[Clinical, radiographic and axiographic control after traction-screw osteosynthesis of fractures of the mandibular condyle region].

Clinical, radiographic and electronic axiographic follow-up examinations were performed in 45 patients with a luxation or highly displaced fractures treated with traction screw fixation from 1984 to 1989. Axiographic findings gave an excellent objective evaluation of joint function. The excellent results achieved with traction screw fixation suggest that this method can be recommended for condyleal fractures of the mandible.

[Follow-up with bone scintigraphy using tomography emission technique (SPECT) after reconstruction of the mandible with iliac crest transplants].

Fifteen patients who underwent mandibular reconstruction with iliac bone grafts where studied by single photon emission computed tomography (SPECT). Six of these patients underwent mandibular reconstruction with revascularized iliac bone grafts. Long-term follow up data by free bone grafts and revascularized bone grafts where very different. The single photon emission computed tomography (SPECT) is an excellent method for the assessment of the vascular patency and bone viability.

[10 years of dental care of hemophiliacs].

Increased consciousness of oral hygiene, improvement of sanitation degree and decrease of the number tooth extractions have been proved a success and an expression of high compliance within the scope of long-term interdisciplinary care of haemophilic patients. At the same time both patients registration and concentration at an out-patient department facilitate cooperation with the paediatricians and internal specialists being in charge of them, in addition a change-over from children to adult dispensary care is ensured. Haemophilic patients suffering from hepatitis B or HTLV-III/HIV antibodies carrier may be registered immediately. All patients undergoing dispensary care are subject to preventive test obligations.

[The cryodevitalization of the mandible in animal experiments].

The mandible of minipigs may be devitalized cryosurgically. With the used cryotechnical method deep temperatures down to -131 degrees C and freezing speeds of 62 degrees C/min can be reached. The bone scintiscanning proved suitable method for establishing the progress of the revitalisation.

[Bone scintigraphy following cryodevitalization of the mandible].

A tumor of the mandible can be destroyed successfully by cryogenic devitalisation. The revitalisation process was observed in 6 miniature swines by means of nuclear medical bone exploration. The bone metabolism decreased clearly postoperatively, and indeed laterally more than lingually. An increased blood flow and bone metabolism was recognized as an expression of the repair process at the border to the healthy bone after 7 days, later also lingually whereas no reaction was found laterally. The extent of the devitalised bone was 0.5-1.0 cm bigger than the cryosurgical part.
