Temporary distal occlusion for non-pharmacological preconditioning of harvested internal mammary artery grafts.

Perioperative spasm of the internal mammary artery (IMA) may result in early restriction of bypass flow. Therefore, pharmacological pretreatment is usually suggested. We present a technique of temporary distal occlusion of the transsected IMA using a metal clip. After 1 hour of storage in normal saline (SAL) there were significantly better flow rates compared with proximal application of a bull-dog clamp. No further increase in flow was noticed after submersion of the IMA pedicle in verapamil (VER). This technique easily provides sufficient IMA flow at the time of anastomosis, probably due to continuous endoluminal dilatation of the IMA by blood pressure, and thus avoids pharmacological pretreatment with vasodilators.

Normal outcome of a pregnancy with mosaicism for double trisomy in amniotic fluid cells.

True chromosomal mosaicism of double trisomy (48,XX, +7, +20) was detected in amniotic fluid cell cultures at 16 and 20 weeks of gestation. No aneuploid cells were found in chorionic villus samples (CVS) by semidirect preparation and long-term culture. High-level ultrasound did not indicate any structural abnormality of the fetus. At 38 weeks of gestation, a phenotypically normal girl was born. She is now 22 months old and normally developed. At birth, various samples were investigated by routine cytogenetic methods or by fluorescence in situ hybridization with the probe p7t1 (umbilical cord blood, placental tissue, umbilical cord fibroblasts, urine sediment) and no abnormal cells could be detected in any of those tissues.

In vivo morphology of woven, collagen-sealed Dacron prostheses in the thoracic aorta.

BACKGROUND: Long-term changes in knitted Dacron grafts inserted into the infrarenal aorta have been addressed by a number of studies indicating their potential for postoperative dilatation. In contrast, the behavior of woven, collagen-presealed, double-velour Dacron grafts used to replace the thoracic aorta is not known. METHODS: Forty-five patients were examined at a mean of 32.4 +/- 14.8 months after insertion of woven, collagen-coated, Dacron double-velour prostheses (Meadox woven with Hemashield, Meadox, Oakland, NJ) in the thoracic position under highly standardized conditions using spiral computed tomography. RESULTS: Compared with a manufactured diameter of 26 mm, all grafts showed an increase of 1 to 5 mm (mean, 3.0 +/- 1.2 mm [11.6% +/- 4.4%]; p < 0.0001) with greater enlargement of the ascending than of the descending aortic portions (p = not significant). A further statistically significant progressive dilatation failed to occur. Degenerative changes, including false aneurysm formation, could be excluded. CONCLUSIONS: Woven, collagen-coated Dacron prostheses are considered a safe replacement material for the thoracic aorta.

[Degenerative colorectal polyps].

In 23 patients with malignant colorectal polyps we reviewed the frequency of recurrence. In the group without subsequent local resection, 8 of 11 patients were without recurrent tumour after 2.7 (0.8-4.1) years, in the group with subsequent local or conventional colon resection, 10 of 12 patients after 5.3 (0.8-9.0) years. None of the patients showed any evidence of residual tumour or lymph node metastasis. All recurrent tumours (2 x carcinoma, 3 x benign polyps) could be resected curatively. There was no difference in recurrence in both groups. For safe histopathological examination of malignant colorectal polypi in respect of depth of invasion, free resection margins and lymphatic vessel invasion, intraoperative fixation and marking must be performed, since local recurrence was seen after more than 5 years, long-term follow up is necessary.

DNA cytophotometry and prognosis in ovarian tumors of borderline malignancy. A clinicomorphologic study of 80 cases.

Surgical specimens of 80 ovarian tumors of borderline malignancy (OTBM) were investigated by scanning DNA cytophotometry. Diploid or euploid DNA histograms were found for 21 tumors, whereas 59 OTBM showed noneuploid or aneuploid DNA patterns. All patients were followed-up after surgery for at least 3 years (mean observation period, 6.7 years). Follow-up showed 11 cases of recurrent disease and 6 deaths. DNA findings and several other morphologic and clinical details (including patient age, histologic type and stage of disease, and extent of therapy) were correlated to the postoperative course. Statistical analyses disclosed that, of these parameters, only DNA content significantly affected prognosis. Recurrences and deaths resulting from tumor exclusively were observed among patients with noneuploid or aneuploid OTBM, whereas none of the diploid or euploid tumors recurred (P less than 0.05). DNA cytophotometry thus might be regarded as an effective complementary means to assess the prognosis of individual OTBM cases.

[Significance of CEA and CA 19-9 for after care of curatively resected colorectal cancers].

The monitoring of CEA is valuable in the follow-up after curative resection of colorectal cancer and contributes to the early detection of local recurrence and distant metastases. The simultaneous detection of the CA 19-9 does not lead to a substantial advantage. The recurrences and especially the metastases detected by the monitoring of the tumour markers frequently are unresectable. In patients with elevated tumour markers without other evidences of recurrence or metastases the indication for a second-look operation should be considered cautious because elevated tumour markers are found in patients with benign diseases also.

[Aorto-iliac reconstruction after kidney transplantation with in situ transplant perfusion with HTK solution].

A case report of a 57 year old male is presented, who underwent kidney transplantation in 1987. Subsequently he developed arterial occlusive disease. A bifurcated prosthesis was implanted and the graft preserved by in-situ-perfusion with HTK-solution (Custodiol). The graft artery was anastomosed in an end to side fashion to the prosthesis. The postoperative course was uncomplicated with unimpaired renal function. We conclude that in-situ-perfusion with cold HTK-solution represents a simple method for kidney graft protection during reconstructive vascular procedures.

Prenatal deletion detection in a sporadic case of Duchenne muscular dystrophy without genotype information from the affected individual.

We describe the application of deletion screening by amplification of deletion-prone exons via polymerase chain reaction (PCR) in a family with a sporadic case of Duchenne muscular dystrophy (DMD). No DNA was available from the affected patient who died 12 years beforehand at the age of 18 years. Material obtained prenatally from two male fetuses exhibited an identical deletion. These findings effectively transformed a sporadic case into a familial case and a numerical carrier risk was substituted by obligate carrier status. Additionally an indirect genotype analysis was replaced by the possibility of direct DNA analysis. Genetic counselling, formerly based upon incomplete data, can now be aided by precise risk assessment.

[Theraplay--a direct communication play therapy].

This paper introduces the basic concepts and the application of the directive short-term play therapy called Theraplay. Goals and methods are illustrated by examples from the assessment (Marschak Interaction Method) and by sessions with a 6 year old regressive noncompliant mutistic twin.

99mTc-labelling of leucocytes with 99mTc-DPO: a complex developed for myocardial imaging.

The lipophilic 99mTc-DPO complex, developed as a myocardial imaging radiopharmaceutical, was used to label leucocytes. After an incubation of 0.1 ml 99mTc-DPO (8 micrograms DMPE*2HCl) with mixed leucocytes in plasma, the labelling efficiency was over 70%. During incubation in 5 ml plasma, a loss of activity was found between 20% (1 h) and 35% (3 h) caused by elution. Disturbances of cell viability could not be found with the help of the chemiluminescence test. The in vivo recovery was determined in three dogs and was 45%-50% (0.5 h), 30%-36% (1 h), and 18%-24% (3 h). Autologous 99mTc-DPO-leucocytes were used on seven patients with suspected osteomyelitis, there were four true negative and three true positive results. The target/nontarget ratio determined by ROI in the positive cases was 1.8 to 2.5 at 3 h after injection.

Maternal modulation of the inheritable meiosis I error Dipl I in mouse oocytes is associated with the type of mitochondrial DNA.

The ovulation of diploid oocytes, abnormally arrested at or during the first meiotic division, is an inheritable trait (DiplI) in mice and modulated by a maternally transmitted factor. By repeated backcrossing, mouse strains with identical nuclear encoded genes and differing only in their mitochondrial genomes can be created. NMB mice represent such a strain having acquired the nuclear genome of C57BL/6J but still possessing mitochondria and therewith mitochondrial DNA (mtDNA) of NMRI/Bom, their female progenitor. The strains NMB and C57BL/6J were used to characterize a new mitochondrial trait, namely the ability to modulate the expression of the inheritable meiosis I error Dipl I in oocytes. We show that an increased rate of ovulated diploid oocytes is associated with the mtDNA type of C57BL/6J. These results corroborate the assumption that mitochondria do play an important role in meiosis of mammalian oocytes and hence seem to be involved also in the orderly segregation of chromosomes.

Chromosome segregation at meiosis I in female T(2;4)1Gö/+ mice: no evidence for a decreased crossover frequency with maternal age.

The influence of age and hormones on chromosome segregation at meiosis I was studied in female mice heterozygous for the T(2;4)1Gö translocation. Females of two age groups (18-22 and 40-56 weeks old) were stimulated for ovulation with different doses of gonadotropins (1.5 IU PMS/1.0 IU HCG or 10 IU PMS/10 IU HCG). Analysis of metaphase II oocytes revealed the highest level of hyperhaploidy (1.8%) and presegregation (4.4%) in the young females receiving the low dose. Presegregation preferentially affected the small 4(2) marker chromosome. There was no significant interference of the tetravalent with disjunction of the nontranslocated normal bivalents. Moreover, no remarkable difference in the mode of segregation (adjacent I, II or alternate) was observed. Recombination within the interstitial pairing segments of the chromosomes involved in the translocation allowed us to calculate cross-over frequencies in ovulated oocytes. For both the large 2(4) and the small 4(2) marker chromosomes, this frequency was higher in old than in young T(2;4)1Gö/+ females. Our data do not support the production line hypothesis of Henderson and Edwards (1968) which claims that chiasma frequency in oocytes decreases with maternal age.
