Three different treatments of regional chemotherapy in colo-rectal malignancies and their results are presented. 1. Prophylactic chemotherapy with 5-FU--via the recanalized umbilical vein in patients without liver metastases (randomized study since 10/1980). 2. Intraportal adjuvant chemotherapy after resection of liver metastases. 3. Intraarterial chemotherapy in patients with unresectable liver metastases. The regional chemotherapy of the liver in colo-rectal diseases seems to increase the survival rate and the quality of life. The Port-A-Cath-system can be used repeatedly, however, there is a certain rate of complications to be expected.
A unique case of spontaneous rupture of a renal oncocytoma presenting with hemorrhagic shock and acute abdomen is reported.

Radiotherapy of the inoperable malignant bile duct obstruction is performed to prolong the survival time in comparison to the palliative drainage. The preferable method is a combination of intracavitary irradiation via a percutaneous transhepatic drainage (30-40 Gy at a distance of 1 cm) and a moving field technique (30-40 Gy). This combination allows a high local tumor dose and a relatively large treatment volume. After radiotherapy, the PTD is being exchanged by a large-bore biliodigestive prosthesis.
In a prospective randomized placebo controlled double blind study, the prophylactic effect of mesalazine (5-aminosalicylic acid. 5-ASA) as suppositories (3 x 250 mg/day) on radiation induced proctitis during radiotherapy for prostatic carcinoma was studied. The study ended when 16 patients had been included (5-ASA: eight, placebo: eight) because of severe side effects in the 5-ASA group. 75% of patients treated with 5-ASA reported symptoms of a severe proctitis while only one patient in the placebo group had similar complaints. The application of mesalazine as suppositories is not useful in preventing radiation induced proctitis during radiotherapy of prostate carcinoma.

Perforation of a peptic ulcer located on the posterior wall of the duodenum is very rare. We present here three cases with acute and free perforation of a posterior duodenal ulcer with a fatal outcome. To the best of our knowledge such an acute free perforation has not been previously published.
Twenty-five cases of Crohn's disease confined to the appendix were reported in eight hospitals in Israel during a 15-year period. Review of the histologic slides confirmed the diagnosis in 22 cases. Re-evaluation of these 22 patients included physical examination and radiologic studies of the small and large bowel. Rectosigmoidoscopy was performed in 16 patients. Signs and symptoms of Crohn's disease at other sites in the gastrointestinal tract did not occur during follow-up periods of two to 15 years (mean, 6.4 years) after appendectomy. This study and a review of the literature indicate that in most cases (93 per cent) Crohn's disease initially limited to the appendix is not a predictor of subsequent involvement of another portion of the bowel. It is concluded that the so-called Crohn's disease isolated to the appendix is a form of chronic granulomatous and follicular appendicitis of unknown etiology that is unrelated nosologically in the majority of the cases to Crohn's disease proper.
A 17-year-old girl with refractory, but nonmalignant, hypertension and hypokalemia is presented. A state of primary hyperreninism and secondary hyperaldosteronism was found to exist, together with a renin-producing Sertoli cell ovarian tumor. Upon removal of the tumor, this patient attained both a normotensive and a normokalemic state. A brief review is given of renal and nonrenal renin-producing tumors.

Two weeks after appendectomy a right hemicolectomy was performed on a 35-year-old man because of an ileocolic intussusception. The appendiceal stump was the leading point of the intussusception. Ileocolic intussusception is a very rare complication after appendectomy. The clinical symptoms, diagnostic features, and mode of treatment are discussed.
A transthoracic approach was used in the treatment of 2 patients with emergencies due to traumatic diaphragmatic hernia. Relevant symptoms began respectively 3 months and 8 months after closed injury. Strangulation and necrosis of abdominal organs herniated into the chest are associated with a high mortality. Awareness and early diagnosis will reduce mortality.
In a comparative study on the pathomechanism of myocardial hypoxia we determined a parameter each of platelet activity and thrombin activity in 46 patients with angina pectoris, i.e., 15 patients with dilatative cardiomyopathy (DCM), 15 patients with latent cardiomyopathy (LCM) and 16 patients with coronary heart disease (CHD) compared to 15 normal subjects (N). beta-thromboglobulin (beta TG) and fibrinopeptide A (FPA) were measured both at rest (I) and after symptom-limited maximal exercise (II) in plasma. In N, beta TG was not increased in any case, neither at rest nor on exertion (I: 20.5 +/- 6.1 ng/ml, II: 22.4 +/- 6.7 ng/ml). Patients with LCM did not differ significantly from N (I: 20.9 +/- 6.1 ng/ml, II: 22.7 +/- 7.9 ng/ml). beta TG values of some patients with DCM and CHD were increased at rest and especially under exercise (DCM I: 27.9 +/- 9.6 ng/ml, vs. N p less than 0.025; II: 49.9 +/- 45.5 ng/ml, vs. N p less than 0.01; CHD I: 25.2 +/- 7.7 ng/ml, vs. N p less than 0.05; II: 38.6 +/- 34.3 ng/ml, vs. N p less than 0.01). Patients with DCM developing significant angina under exercise showed a higher beta TG under these exercise conditions than those with mild or no angina (p less than 0.01). In patients with coronary heart disease, this correlation was not to be found. With regard to FPA the four investigated groups differed in an analogous way, as they did with regard to beta TG; but only a weak correlation within both values was shown.(ABSTRACT TRUNCATED AT 250 WORDS)
Two patients, one with bile duct stones above a strictured biliary-enteric anastomosis, and the second with posttraumatic intra- and extrahepatic bile duct strictures, were treated via the percutaneous transhepatic route. The stones were removed and the strictures dilated. Surgery was unnecessary in the first patient, and in the second, exploratory laparotomy confirmed the efficacy of percutaneous transhepatic dilatation.
