[Mercury release from amalgam specimens].

Over 7 days specimens of 5 gamma 2-free Amalgams were stored in KSCN-containing and KSCN-free lactic acid-salt solution. After 1, 4 and 7 days samples of the solution were analyzed for their mercury contents using inductively coupled plasma atom emission spectroscopy. In KSCN-containing solution the release of mercury was 45.4 micrograms, in KSCN-free solution it was 70.7 micrograms/50 mm2. KSCN, a part of natural saliva, has the effect of inhibiting corrosion.

[Using force in psychiatry. Evaluation of restraint documentation in a psychiatric hospital].

The documentation of restraints in a psychiatric hospital was examined over the period of a year as regards frequency and duration of restraints as well as the reasons for them. Based upon this statistical evaluation, the conditions which lead to the use of force in psychiatry are discussed in general, taking into account disease-related causes and--above all--institutional and environmental factors as well as causes related to interaction. Final considerations investigate if and how an increase in staff and concrete support for the everyday running of a ward can diminish the potential for violence, thus rendering restraints unnecessary as far as possible.

[Echocardiographic functional parameters of the left ventricle as a prognostic indicator in coronary heart disease].

The value of two-dimensional cross-sectional echocardiography for the estimation of the left ventricular function had been investigated in 241 consecutive patients with suspected coronary artery disease (CAD). The day before left heart catheterization the left ventricular volumes (EDV, ESV) as well as the global left ventricular ejection fraction (EF) were calculated from the RAO-equivalent in the 2D-echo and in addition the classification of the EF was visually performed from different cross-sections. The coronary angiography showed in 208/241 patients hemodynamically effective stenoses (lumen restriction greater than 50%). For 192/208 patients there were diagnostically usable 2D-echograms as well as clinical data over an observation period of 3 years and 7 months. The 2D-echo correspond quite well to the levocardiography for the calculation of the EDV with r = 0.75, with r = 0.85 for the ESV, and with r = 0.80 for the EF. The mere visual evaluation of the EF out of the 2D-echo agreed well in 84% of the cases to the quantitative determination. During the observation period 18/192 patients died; 17/18 of these patients of cardial causes. Out of the patients with normal EF in the 2D-echo only 3.5% died, with slightly reduced EF 10% died. With highly reduced EF mortality was with 40% significantly increased (p less than 0.001). Thus in patients with CAD unfavourable long-time prognosis may be quickly recognized by their markedly reduced left ventricular function in the 2D-echocardiogram, which shows favourable correspondence to invasive data.

[Comparative SEM study of root canals prepared with Intraendokopf 3-LDSY and 3-LD].

A total of 211 root canals were prepared in extracted teeth using either hand instruments or two engine-driven instruments (KaVo Intraendokopf 3-LDSY and 3-LD). The teeth were investigated by light and scanning electron microscopy. The best results in terms of smoothing the canal wall were observed in the teeth prepared with hand instruments. The Intraendokopf 3-LD was found to be useful in supporting manual methods in certain limited cases, such as curved narrow canals which are hard to access. 3-LDSY, however, proved to be less efficient because it tended to bind in the canal so that the full working length could not be reached.

[Spontaneous remission of lung metastases in renal cell carcinoma].

Pulmonary metastases were first noted in a 47-year-old man, 15 months after a radical right nephrectomy and local radiotherapy. They progressed over the subsequent ten months despite progestagen administration. But they regressed slowly after the progestagen had been discontinued so that eight months later they were no longer demonstrable radiologically. The remission has so far persisted for 3 1/2 years.

Fetal bradycardia in labor.

The prognostic significance of terminal bradycardia is analysed in this study. Amongst 12,145 intrapartal directly recorded cardiotocograms, 135 heart rate curves free of deceleration (1.1%) were found with an abruptly occurring fetal bradycardia. The mean duration of bradycardia was 5.7 +/- 3.1 minutes, the mean heart rate during the phase of bradycardia was 86.2 +/- 9.9 beats/minute. A premature amniotomy (40%) was significantly correlated with the bradycardia. The mean pH in the umbilical arterial blood was 7.261 +/- 0.072. pH values of less than 7.200 were measured in 21.7% and pH values under 7.150 were measured in 4.2%; a severe acidosis (pH less than 7.100) was not present in any case. The fall in fetal pH in the umbilical arterial blood per minute of bradycardia averages -0.006 units. In the bradycardia group, only 5.2% of all babies were slightly depressed and only 0.7% were severely depressed; 5.9% of the babies had slight difficulties in postpartal adaptation. It is concluded from the data that as a rule the impending spontaneous delivery (head rotated correctly, effective bearing-down efforts) can be waited for in the occurrence of terminal bradycardia (without prior decelerations). The fall in heart rate should not automatically occasion the institution of hazardous delivery maneuvers which may entail a greater danger to the mother and baby than the fetal bradycardia itself.

[Mechanical preparation of root canals using a Canalfinder as compared with hand-operated instruments].

In an in vitro study of 134 teeth the efficacy of manual canal preparation was compared with that of the mechanical method using the Canalfinder system. The superiority of the mechanical instrument in narrow and curved root canals is particularly evident in the buccal segment. Since, however, the canal lumen and the master point are not perfectly equal, it is necessary to finish the preparation with conventional hand instruments.

Topographical analysis of proliferation ([3H]thymidine labelling index and mitotic index) as compared with tumour growth and tumour weight in xenotransplanted melanoma. Changes due to local and systemic application of azelaic acid.

Xenotransplanted human melanoma was investigated by measuring the increase in tumour volume and in final tumour weight (macroscopical parameters) and histomorphological parameters of cell proliferation: Mitotic index (MI) and autoradiographic [3H]thymidine labelling index (LI). A total of 87 tumours, derived from a human melanoma metastasis and a primary nodular melanoma respectively, were analysed by these methods in two series. Topical treatment of the tumours with azelaic acid cream resulted in a statistically significant reduction in the increase in tumour volume and, in the first series, in a clear decrease in final tumour weight and in the MI, as compared with controls. The LI was decreased only in the superficial region of the tumours, i.e. at the site of treatment. Subtumoral injection of azelaic acid (disodium salt solution) was the second route of local therapy. It was followed by a significant reduction in the increase in tumour volume, of final tumour weight (first series) and in the MI. The average LI was clearly smaller than in the controls, especially at the tumour base, which was the site of injection (local effect). Systemic (intravenous) injection of azelaic acid (same concentration of the disodium salt solution) had no negative effect on the increase in tumour volume or final tumour weight, but was followed by a clear reduction of the MI. The average LI of this group was significantly smaller than in the controls as well. This effect was most impressive in the perivascular regions of large and small vessels, which fact can be interpreted as a sort of local effect via the blood stream after systemic application of azelaic acid.(ABSTRACT TRUNCATED AT 250 WORDS)

[Development of a findings and results data system for forensic medicine autopsy cases].

Based on the personal computer software "dBase III plus", we have developed a database program for storage and analysis of data obtained from forensic autopsies. It has been designed to handle administrative data as well as aspects of pathological and forensic findings. To assure the comparability of data with those of other institutes, the data were encoded using the SNOMED nomenclature. This system is characterized by a well differentiated, hierarchical structure, and is commonly used internationally. The entry of data is done by choosing among more than 3,000 terms, which are displayed in a system of selective menus. There terms can be supplemented at any time for specific purposes. Thus it is necessary to record the results of an autopsy in a standardized and well-defined way. The data-base can be used for statistical evaluation of autopsy results, but is also intended as a basis for the development of an expert system to help to determine the cause of death.
